Toxicity testing of atracurium.

The toxicity of atracurium has been investigated in subacute tests in rat, dog and monkey, in a fetal toxicity test in the rabbit and in a paranatal study in the cat. No specific adverse effects were found despite administration of supraparalysing doses. It appeared not to cross the placenta at term and the neonatal kitten seemed resistant to its neuromuscular blocking action. In the dog, cat and pig, in which physiological monitoring was undertaken, i.v. injection of three to four times the paralysing dose had little or no effect on arterial pressure or heart rate. The solution did not cause

ULTRASOUND IN THE DIAGNOSIS AND MONITORING OF AMYOTROPHIC LATERAL SCLEROSIS: A REVIEW.

Neuromuscular ultrasound is complementary to electrodiagnostic (EDx) testing and is useful in enhancing the diagnosis of mononeuropathies, peripheral nerve trauma, and demyelinating polyneuropathies. There is increasing interest in using ultrasound both to aid in the diagnosis of amyotrophic lateral sclerosis (ALS) and to monitor its progression. In this article we review the relevant literature on ultrasound in ALS. Ultrasound is more sensitive than EDx in identifying fasciculations in patients with ALS. It can detect decreased muscle thickness, increased muscle echointensity and echovariance

Measurement and mobilisation of localised oedema in haemodialysis.

BACKGROUND: The presence of localised oedema can make measurement and removal of excess fluid in haemodialysis challenging. OBJECTIVES: To evaluate (i) the effectiveness of intermittent pneumatic compression and neuromuscular electrical stimulation at mobilising oedema and (ii) the impact of localised fluid on bioimpedance measured fluid status. DESIGN: A single centre, cross-over study design. Participants were monitored weekly during mid-week dialysis sessions. Four sessions with each of the interventions and no interventions, with washout periods between, were included. PARTICIPANTS: Six

Proteomic Profiling of Myofiber Repair Annexins and Their Role in Duchenne Muscular Dystrophy.

Myofiber regeneration and membrane repair play crucial roles in maintaining the continuous physiological functioning of the neuromuscular system. A swift and efficient repair mechanism enables the rapid restoration of sarcolemmal integrity following cellular impairment in damaged skeletal muscles. Members of the annexin family of proteins, which are characterized by the peripheral binding to acidic phospholipid membranes, are intrinsically involved in this myofiber repair process. The biochemical and proteomic profiling of dystrophinopathy, a severe and highly progressive neuromuscular disorde

286th ENMC international workshop: Muscle imaging: artificial intelligence, automatic segmentation and imaging data sharing in neuromuscular disease. Hoofddorp, The Netherlands, 7-9 March 2025.

Quantitative muscle MRI (qMRI) has emerged as a promising non-invasive biomarker for assessing neuromuscular diseases (NMDs). However, clinical implementation is limited by the significant time required for manual muscle segmentation, which restricts analysis to limited muscle regions rather than comprehensive whole-muscle assessment. The 286th European NeuroMuscular Centre (ENMC) workshop brought together 18 international participants from 10 countries to establish consensus on optimal qMRI acquisition protocols and automated analysis tools, revealing that while most centers utilize qMRI tech

Demographic-Driven Electromyography Analysis: Advancing Personalized Biosignal Interpretation.

The integration of rehabilitation robotics and surface electromyography (sEMG) offers a powerful approach for monitoring and enhancing recovery in patients with neuromuscular disorders. However, variability in baseline sEMG readings across individuals can limit its effectiveness. Factors such as age, height, and weight influence these baselines, and there is a lack of personalized baselines that account for demographic differences. This study proposes a novel model to estimate individualized baselines for one important sEMG parameter, Root Mean Square (RMS). Demographics and physiological data

The effect of prolonged walking on leg muscle activity patterns and vulnerability to perturbations.

Understanding the consequences and ecological relevance of muscle fatigue is important to guide the development of strategies to preserve independence. However, few studies have examined walking-related fatigue and the effects on walking instability. Our purpose was to investigate the effects of prolonged walking on leg muscle activity and vulnerability to balance perturbations. Eighteen healthy young adults completed a 30-min walking trial at their preferred walking speed while leg muscle activities were recorded. Before and after the 30-min walk, participants responded to five 5% body weight

Anaesthesia for thymectomy in adult and juvenile myasthenic patients.

PURPOSE OF REVIEW: Myasthenia gravis, a chronic disease of the neuromuscular junction, is associated with an interaction with neuromuscular blocking agents (NMBAs). As thymectomy is often the method of choice for its treatment, anaesthetic management requires meticulous preoperative evaluation, careful monitoring, and adequate dose titration. The frequency of video-assisted thoracoscopic extended thymectomy (VATET) is also increasing, making the use of NMBA obligatory. The number of cases of the juvenile form has also increased over years; airway management in juvenile one-lung ventilation is

Lactic acidosis associated with severe neuromuscular weakness and stavudine therapy.

Nucleoside analogue reverse-transcriptase inhibitors (NRTIs) especially stavudine, used for the treatment of HIV infection have been rarely associated with lactic acidosis syndrome (LAS) and severe neuromuscular weakness mimicking Guillain Barre syndrome. A 36-year-old man presented with a one-week history of nausea, vomiting, epigastric pain, dyspnea associated with progressive muscle weakness and numbness in glove and stocking pattern. He had symptomatic HIV infection, diagnosed 2 years before the admission and was treated with GPOvir (lamivudine, stavudine and nevirapine). Physical examinat

Which muscle relaxants should be used in day surgery and when.

PURPOSE OF REVIEW: After myorelaxants, myalgia and residual curarization may complicate recovery. Local anaesthesia and minimally invasive airway management make myorelaxants disputable in many outpatient procedures; nevertheless, neuromuscular blockade may be necessary to facilitate intubation or maintain muscle relaxation. Agent selection criteria are discussed. RECENT FINDINGS: Reduced hospital time is not associated with central neuraxial or peripheral nerve block. To reduce the risk for residual block, neuromuscular monitoring is mandatory. Use of reversal agents should not be restricted…

Neuromuscular dysfunction, independent of gait dysfunction, modulates trabecular bone homeostasis in mice.

OBJECTIVES: To clarify the effects of neuromuscular dysfunction on hindlimb loading, muscle atrophy, and bone homeostasis. METHODS: We quantified changes to hindlimb loading, muscle atrophy, and bone morphology following either Botulinum toxin A (BTxA) induced muscle paralysis or peripheral nerve injury (PNI) in mice; two in vivo models that we anticipated would differently alter gait and mechanical loading patterns due to their distinct effects on neuromuscular signaling. To confirm the expected behavioral effects of PNI, we assessed mechanical allodynia of the ipsilateral hindlimb using von

[Comparative study of the effect of muscle relaxants on the intraocular pressure].

Control of intraocular pressure is a major concern in ophthalmic anesthesia, especially in the presence of glaucoma or penetrating eye injury. The use of succinylcholine under these conditions is controversial. This study was undertaken to assess the effects of the neuromuscular blocking agents succinylcholine, pancuronium, metocurine, vecuronium, and atracurium on the intraocular pressure of dogs. The study was performed on five "conditioned" mature male foxhounds. The dogs were intubated immediately after induction with thiopental 10 mg/kg and ventilated with an O2/N2O mixture (FIO2 = 0.33).

Early Neurophysiological Monitoring of Train of Four Assists in the Detection of Pseudocholinesterase Deficiency.

A craniotomy with cortical and subcortical mapping was planned for a 64-year-old male with a large right frontotemporal brain mass. Total intravenous anesthesia was performed, and 200 milligrams of succinylcholine was administered at induction. A train of four prior to head pinning (52 minutes after succinylcholine administration) revealed zero of four twitches in the left hand and foot. The patient did not regain spontaneous breathing despite discontinuation of infusions and the surgeon canceled the case at 108 minutes from induction. The patient was safely extubated at 270 minutes. Intermittent abdominal pressure ventilation management in neuromuscular diseases: a Delphi panel Consensus.

BACKGROUND: Intermittent abdominal pressure ventilator (IAPV) use started in the 1930s for ventilatory assistance with muscular dystrophy patients. Later, the device was perfected and expanded for other neuromuscular disorders (NMD). In recent years, the morbidity and mortality tracheotomies and trach tubes related renewed the interest around IAPV. However, there are no guidelines for its use. This study aimed to establish a consensus among physicians involved in its practice to provide IAPV suggestions for the treatment of patients with NMD. METHOD: A 3-step modified Delphi method was used t

Neuromuscular diseases and Covid-19: Advices from scientific societies and early observations in Italy.

The risk of a severe course of COVID-19 is increased in patients suffering with Neuromuscular disorders (NMD) due to the following comorbidities: muscular weakness of the chest and diaphragm, use of ventilator supports and/or presence of tracheostoma, weak airway clearance, cardiac involvement, rhabdomyolysis, comoribities, steroid and immunosuppressant treatments. NMD display varying levels of disability in people with the same diagnosis, thus it is difficult to give COVID-19 related general recommendations. Present advicess were designed for patients, caregivers, general neurologists and non

Myopathology in congenital myopathies.

Congenital myopathies are clinically and genetically a heterogeneous group of early onset neuromuscular disorders, characterized by hypotonia and muscle weakness. Clinical severity and age of onset are variable. Many patients are severely affected at birth while others have a milder, moderately progressive or nonprogressive phenotype. Respiratory weakness is a major clinical aspect that requires regular monitoring. Causative mutations in several genes have been identified that are inherited in a dominant, recessive or X-linked manner, or arise de novo. Muscle biopsies show characteristic patho

Frequency-dependent changes in neuromuscular responses to cyclic lumbar flexion.

Repetitive lifting in the workplace has been identified to be a cause of low back disorders. Epidemiologic data further supports an hypothesis that higher repetition rate (i.e. frequency) is an added risk factor. The objective of this study was to provide experimental data testing the above hypothesis. An in vivo feline model was subjected to 20-min of cyclic lumbar loading at frequencies of 0.1 Hz and 0.5 Hz while monitoring the EMG from the L-3/4-L-5/6 multifidus muscles and the creep at the L-4/5 level. Seven hours of rest were allowed after the cyclic flexion/extension was terminated. Duri
