Fifteen-minute consultation: assessment of a child with suspected shunt problems.

Cerebrospinal fluid diversion procedures have saved more lives than any other neurosurgical procedure. However, they do have a unique set of complications which a paediatrician in a district general hospital may encounter. Timely recognition and appropriate referral to the neurosurgeon is vital in order to avoid serious consequences and to have a favourable outcome.

Impact of moderate alcohol consumption on visuo-motor skills in neurosurgical residents.

Alcohol adversely affects cognitive and motor functions that are critical for surgical performance. Although its general physiological and neurocognitive effects are well documented, evidence regarding its impact on complex, high-precision tasks-such as surgical procedures-remains limited. In this study, nine neurosurgeons performed a series of standardized surgical tasks assessing fine motor skills, gross motor control, and hand-eye coordination under three conditions: sober, acutely influenced (blood alcohol concentration [BAC] ~ 1.0 g/kg), and during the elimination phase several hours afte…

Congress of Neurological Surgeons Systematic Review and Evidence-Based Guidelines Update for the Role of Audiologic Screening in the Diagnosis and Management of Patients With Vestibular Schwannomas.

BACKGROUND: Vestibular schwannoma (VS) represents a benign tumor of the vestibulocochlear nerve that presents with otologic dysfunction. Although MRI remains the most common technique for imaging diagnosis of VS, there are no unifying guidelines to suggest when a practitioner should obtain a screening MRI for new otologic complaints to rule out VS. OBJECTIVE: We aim to assess the diagnostic yield of MRI in the detection of VS in patients presenting with asymmetric sensorineural hearing loss (SNHL), unilateral tinnitus, and sudden SNHL. METHODS: The questions from the previously published gui

Will ChatGPT be Able to Replace a Spine Surgeon in the Clinical Setting?

OBJECTIVE: This study evaluates ChatGPT's performance in diagnosing and managing spinal pathologies. METHODS: Patients underwent evaluation by two spine surgeons (and the case was discussed and a consensus was reached) and ChatGPT. Patient data, including demographics, symptoms, and available imaging reports, were collected using a standardized form. This information was then processed by ChatGPT for diagnosis and management recommendations. The study assessed ChatGPT's diagnostic and management accuracy through descriptive statistics, comparing its performance to that of experienced spine sp

Impact of Experience on Outcomes After Endoscopic Transsphenoidal Surgery for Acromegaly.

OBJECTIVE: Acromegaly is a syndrome of metabolic alterations secondary to increased growth hormone levels from a somatotroph pituitary adenoma. A multidisciplinary approach beginning with surgery, followed by adjuvant radiation or medical therapy for residual disease, is considered standard of care. Several factors affect the likelihood of remission after surgery, but the impact of surgical experience on remission rates has not been adequately assessed. METHODS: Retrospective review of 203 patients, divided into 2 eras (era 1, 102 patients; era 2, 101 patients) of patients who underwent trans

Meditation music improved the quality of suturing in an experimental bypass procedure.

BACKGROUND: Neurosurgeons are vulnerable to additional noise in their natural operating environment. Noise exposure is associated with reduced cognitive function, inability to concentrate, and nervousness. Mediation music provides an opportunity to create a calmer environment which may reduce stress during surgery. METHODS: A pilot study was performed to find a suitable task, meditation music of surgeon's choice, and operation noise and to reach a certain level of training. For the main experiment, two neurosurgeons with different microsurgical experience used real operation noise and meditat…

Chiari I Malformation and Sleep-Disordered Breathing.

Sleep-disordered breathing (SDB) is a frequent symptomatic feature of pediatric Chiari I, reported in at least 24% of patients presenting to neurosurgeons. Here the epidemiology, natural history, pathophysiology, and diagnosis of SDB in Chiari I is reviewed. Diagnosis requires polysomnography, which should be pursued in young or symptomatic patients or those with prominent imaging findings. Review of case series of surgical decompression suggest that surgical decompression can improve SDB in selected patients.

Mediastinal paraganglioma as a large dumbell tumor: A case report.

Mediastinal paraganglioma presented as a large dumbbell tumor is a rare entity. We report a case of a 47 year old woman who suffered from spinal pain and sporadic lower limb paresis. The imaging studies showed a voluminous mass occupying the posterior mediastinum with right foraminal extension. For excellent results, a combined effort was necessary including thoracic and neurosurgeons teams. Complete resection was successfully performed without laminectomy. The operative course was uneventful.

Ventriculoperitoneal shunt management in children undergoing liver transplantation.

There is a lack of data to guide neurosurgeons on the management of ventriculoperitoneal shunts (VPS) in patients undergoing abdominal transplant operations. We present the cases of two pediatric patients with VPS undergoing liver transplantation who were successfully managed with externalization of the VPS at time of transplantation, with subsequent re-internalization once cleared by the transplant surgery team. We present this as an effective management strategy in patients undergoing liver transplantation.

Perioperative management of face-to-face craniopagus twins separation.

Craniopagus conjoining represents a complex and challenging issue for neurosurgeons as well as for anesthesiologists. A rare face-to-face case of conjoined twins underwent surgical separation and presented peculiar differences compared with those already reported in the literature. Even in cases lacking large cerebrovascular sinus connections, the impending risk of large blood loss and hemorrhagic shock in the infant requires a high level of surveillance and the institution of invasive monitoring.

Frontoethmoidal mucocele with intracranial extension: a case report.

Mucoceles with intracranial extension are often reported by otolaryngologists and neurosurgeons. The authors present a case of a large frontoethmoidal mucocele with intracranial and intraorbital extension exerting a mass effect on the anterior cranial fossa. The patient experienced spontaneous drainage of the mucocele without evidence of recurrence of the lesion after a one-year period, suggesting that mucoceles, regardless of size, can be treated with conservative procedures such as functional endoscopic sinus surgery.

Management of choroid plexus tumors in children.

Choroid plexus tumors are most common in the first year of life and are quite large at presentation. Total resection is the treatment of choice, and many children require diversion of the cerebrospinal fluid despite removal of the tumor. Tumor size and associated hydrocephalus present formidable challenges to the pediatric neurosurgeon. The operative approaches as well as other treatment options are discussed.

Indocyanine Green-Assisted Endoscopic Transorbital Excision of Lateral Orbital Apex Cavernous Hemangioma.

Orbital apex lesions posed operative difficulties to neurosurgeons and ophthalmologists due to limited surgical corridor and close vicinity to cranial nerves and arteries. Lateral orbital apex lesions were traditionally operated via the transcranial route by neurosurgeons. Recently, only a handful of reports have described the use of endoscope alone for excision of lateral orbital apex lesion. Our group, with both endoscopic skull base neurosurgeons and oculoplastic surgeons, has adopted the endoscopic transorbital approach for orbital apex lesions. We also used an indocyanine green (ICG) endo…

Characteristics of Patients With Trigeminal Neuralgia Referred to the Indonesian National Brain Center Neurosurgery Clinic.

Trigeminal neuralgia (TN) is a debilitating neuropathic pain involving the fifth cranial nerve. There has been no study investigating the clinical and socioeconomical characteristics of patients with TN in Indonesia. A total of 100 patients were included in this study. Symptoms indicating a later stage of the illness, namely, involvement of all the trigeminal nerve branches, numbness, and concomitant persistent pain, were the common presentations found in our cohort. Only one TN diagnosis was made by a general practitioner (GP). None were immediately referred to a neurosurgeon following their

A transnasal foreign body penetrating the spinal cord from the nasopharynx.

OBJECTIVE AND IMPORTANCE: This is the first article regarding a transnasal Foreign bodies (FB) penetrating the spinal cord. We tried to remove it safely with a bilateral approach by performing a nasal endoscopy and partial laminectomy. CLINICAL PRESENTATION: During logging work, a tree hit the occipital region of a 47-year-old man, producing unconsciousness with left paresis. Although he did not remember his injuries due to traumatic amnesia, a computed tomography (CT) scan showed a metal rod lodged from the left side wall of the nasopharynx to the spinal column at the cranial-cervical transi

BAER suppression during posterior fossa dural opening.

BACKGROUND: Intraoperative monitoring with brainstem auditory evoked responses (BAER) provides an early warning signal of potential neurological injury and may avert tissue damage to the auditory pathway or brainstem. Unexplained loss of the BAER signal in the operating room may present a dilemma to the neurosurgeon. METHODS: This paper documents two patients who displayed a unique mechanism of suppression of the BAER apparent within minutes following dural opening for resection of a posterior fossa meningioma. RESULTS: In two patients with anterior cerebellopontine angle and clival meningio
