Revision rhinoplasty of Asian noses: analysis and treatment.

OBJECTIVE: To analyze postrhinoplastic deformities in Asian noses and the differences between noses of white and Asian patients. For a successful revision rhinoplasty, one of the most difficult and unpredictable procedures in plastic surgery, an accurate diagnosis and analysis of error are needed. DESIGN: A retrospective study of 84 revision rhinoplasties performed by the senior author (C.H.P.) from January 2001 through December 2006. SETTING: Tertiary care rhinology clinic. PATIENTS: The study population comprised 84 patients (28 men and 56 women), with a mean age of 36 years (range, 20-58…

Immunofluorescent study of polyps in nose and paranasal cavities.

OBJECTIVES: Aim of this research was an immunofluorescent study and to measure immunoglobulins status of the polyps in the nose and paranasal cavities. BACKGROUND: Polyps of the nose and the paranasal cavities are very often guised, and their frequency is growing with increasing chemicals and allergens. These factors interfere important research on polyps. Developments in allergology and immunology help in the detection of etiopathogenetical mechanisms of polyposis development, like the disturbance in mucus immunity. METHODS: Clinical material was collected from 100 hospitalized patients at …

A rare case of congenital unilateral hypoplasia of the nose.

The present paper introduces a rare case of congenital unilateral hypoplasia of the nose. A 29-year-old woman consulted the authors' institute complaining of asymmetric appearance of the nose. Besides the external nose, the nasal cavity and paranasal sinuses were also involved in the deformity; computerised tomography scanning revealed complete absence of the right nasal cavity and right maxillary sinus. The nasal septum, left nasal cavity, and left maxillary sinus were not impaired. The asymmetry of the nose was corrected using local flaps. Since the patient did not have major functional dist…

Percutaneous root osteotomy for correction of the deviated nose.

BACKGROUND: This study describes the use of percutaneous root osteotomy near the intercanthal level for correction of deviated noses. This procedure provides complete mobilization of the midline bony septum. The study analyzed outcomes in patients undergoing this procedure. METHODS: The study retrospectively reviewed 50 patients who underwent corrective rhinoplasty involving a percutaneous root osteotomy. Two independent rhinoplastic surgeons assessed treatment outcomes by comparing pre- and postoperative photographs. Complications associated with the procedure were assessed also. RESULTS: P…

Use of an electronic nose to diagnose bacterial sinusitis.

BACKGROUND: Having previously established that an electronic nose (enose) can distinguish among bacteria samples, between cerebrospinal fluid leak and serum, and can identify patients with ventilator-associated pneumonia, we hypothesized that bacterial sinusitis could be diagnosed by sampling exhaled gas with an enose. METHODS: Using a nasal continuous positive airway pressure mask, we sampled gas exhaled through the nose of patients with sinusitis and compared them with controls. Data were first projected onto the principal components and then classified by support vector machine (SVM), a ma…

Rhabdomyosarcoma of nose and paranasal sinuses--a therapeutic challenge.

Rhabdomyosarcoma is a highly malignant tumour of striated muscle and it is the most common soft tissue sarcoma in the paediatric age group. It is very uncommon in the nose and paranasal sinuses. Pinkish nasal mass and recurrent epistaxis are the most common presenting symptoms. Histologically embryonal rhabdomyosarcoma is the most common variant. The present series deals with seven such cases of different histopathological types including the embryonal, botryoid, and alveolar varieties in the paediatric age group. Multimodal treatment including limited surgery followed by radiotherapy and chem…

Reconstructive surgery of the leprosy nose: a new approach.

There has still been no reduction in the detection rate worldwide for leprosy, despite supervised multi-drug therapy. In time, leprosy can result in a severe saddle-nose deformity leading to functional problems, disfiguration and stigmatization. In severe cases, only the nasal skin tissue and the lower lateral cartilages are preserved. In such cases, the ideal would be to restore the cartilaginous skeleton but, by contrast with other causes of saddle-nose deformities, this is complicated by the quantity and the poor quality of the remaining nasal mucosa. Leprosy-related saddle-nose deformities…

Ehrlichia under our noses and no one notices.

Ehrlichia chaffeensis, an obligately intracellular bacterium, resides within a cytoplasmic vacuole in macrophages, establishes persistent infection in natural hosts such as white-tailed deer and canids, and is transmitted transstadially and during feeding by ticks, particularly Amblyomma americanum. Ehrlichial cell walls contain glycoproteins and a family of divergent 28 kDa proteins, but no peptidoglycan or lipopolysaccharide. The dense-cored ultrastructural form preferentially expresses certain glycoproteins, including a multiple repeat unit-containing adhesin. Ehrlichiae attach to L-selecti

Endoscopic excision for postrhinoplasty mucous cyst of the nose.

Mucous cysts formation following a rhinoplasty is a rare complication. Only 16 cases have been reported. The cysts seem to arise from mucosal remnants left in the subcutaneous layer and must be distinguished from other complications causing postoperative swelling of the nose, such as foreign body, paraffinoma and bony or cartilagineous fragments. We describe two cases of dorsal mucous cysts following rhinoplasty, provide a review of the concerning literature and discuss the removal of the lesions by nasal endoscopy. Nasal endoscopy allows excellent exposure of the operative field, without the …

Saddle nose deformity in a patient with Crohn's disease.

Crohn's disease is a chronic idiopathic inflammatory disease of the bowel, and in most cases it involves the small bowel and colon. Extraintestinal manifestations occur frequently and multiple organ systems may be affected. In contrast, nasal manifestations are extremely rare and only a few cases have been reported to date. Saddle nose deformity in a patient with Crohn's disease has not been reported in the English literature. We report a case of such a deformity and the current literature on nasal manifestations in Crohn's is reviewed. The normal diagnostic and therapeutic features are also d…

[Clinical and morphological parallels in combined deformations of the nose].

Analysis of clinical characteristics of 56 patients with combined deformations of the nose and morphological (histological and histochemical) changes in the nasal septal cartilage in 20 patients showed that common nasal deformations are complicated with age, impeding nasal respiration. The intensity of dystrophic changes in chondrocytes and intercellular cartilaginous matrix depends on the duration of posttraumatic or congenital deformation but not on its clinical severity. This necessitates earlier interventions, before the development of pronounced deformation of the whole nose and deteriora…

Histopathological findings of the nose of Down syndrome abortuses.

Recent reports of absent nasal bone in fetuses with Down syndrome have sparked much interest in the use of this finding for the screening of Down syndrome. We describe the histopathological findings of nasal bones of two fetuses with Down syndrome, one with absence and the other with normal ossification of the nasal bone. We propose that histopathological examination of the nasal bone could improve the accuracy of diagnosis of nasal hypoplasia among Down syndrome abortuses.

[The case of Wegener's granulomatosis of the nose, larynx and ear].

We have described a rare case of Wegener's granulomatosis in our 41-year old patient, located in the nose, larynx and ear. The disease usually presents as a triad of respiratory, vascular and kidney location. It may occur as a rapid or indolent progress. We have shown in our paper diagnostic difficulties connected with this disease. The biopsies of involved organs and blood tests are the best steps of establish proper diagnosis. Antineutrophil cytoplasmic antibodies (ANCA) are typical markers of Wegeners's granulomatosis. The conservative therapy including immunosuppression and surgical treatm…

Aneurysmal bone cyst of the nose with 17p13 involvement.

We report on a 6-year-old girl with a polypoid mass, filling up the entire right nasal cavity as shown on a magnetic resonance imaging scan. Histologically, the tumor had the characteristics of an aneurysmal bone cyst, which is extremely rare in this location. Cytogenetic analysis disclosed a single (6:17)(p21;p13) translocation, confirming a specific genetic involvement in the development of aneurysmal bone cysts. Fluorescent in situ hybridization analysis mapped the putative gene between the p53 (17p13.1) and the Mieller-Dieker gene (17p13.3) loci.

A case of reconstruction of saddle nose deformity in leprosy.

A case of reconstructive surgery for saddle nose deformity is presented in this paper. A 22 year-old Bengali female who had completed WHO/MB regimen for 27 months underwent reconstructive surgery for saddle nose deformity. Since a saddle nose is one of the symbols of leprosy, it often causes serious psychological and social troubles to patients. This happens more often when the patient is a young unmarried woman. In this case the saddle nose seemed to be very serious. After being discharged from hospital she got married and had a baby. This operation gave the patient great relief to live in th…
