Intussusception in the Neonate: A Case Study.

Intussusception is the most common cause of intestinal obstruction in infants and toddlers; however, it is a rare entity in neonates. During intussusception, a proximal portion of the bowel pushes inside the adjacent, distal segment of the bowel in a telescoping fashion leading to obstruction, strangulation of the mesentery, ischemia, and necrosis. The etiology of intussusception is often unknown and there are currently no preventive techniques. Intussusception often mimics other common disease processes affecting neonates; this can result in detrimental delays in diagnosis and treatment. Intu

Bounded professional courage and safe initiative in intensive care nursing.

BackgroundIntensive care nurses may need to take proportionate first action before a revised medical order is available. In such moments, patient advocacy, role boundaries, and accountability intersect with fear of blame, uncertainty, and emotional burden, linking safe initiative to both patient safety and nurse well-being.AimTo explore how intensive care nurses in Turkey describe initiative in the absence of explicit orders, how they distinguish bounded professional courage from hesitation and recklessness, and what these experiences imply for supportive work environments.Study DesignA qualit

Hand hygiene practices and resources in a teaching hospital in Ghana.

INTRODUCTION: Nosocomial infections have long been neglected in Sub-Saharan Africa, and hand hygiene (HH) is usually neglected in hospital settings. This study aimed to provide baseline data on HH compliance among health workers and HH resources in a large West African teaching hospital. METHODOLOGY: A cross-sectional, unobtrusive observational study assessed personal and care-related HH compliance among doctors and nurses and HH resources in 15 service provision centres of the Korle-Bu Teaching Hospital (KBTH), Ghana, in 2011. Data was collected with an infection prevention checklist and hea

Implementation of basic training in neonatal resuscitation in the outskirts of Conakry, Guinea: evaluation of neonatal mortality by 'before and after intervention' design.

High neonatal mortality remains a major health problem in Guinea (32 deaths /1,000 live births). This represents 15 000 deaths annually, without improvement over the past decade. We evaluated the impact of 2 days of neonatal resuscitation training of health professionals working in the disadvantaged outskirts of Conakry. Non-randomised interventional study with pre-and post-interventional analysis of the very early neonatal mortality with data collection over two 6-month periods, one before and one after intervention. Intervention: Theoretical and practical training given to health professiona

Implementing a Sustainable, Phased Approach to Reducing CLABSI Rates in a Level IV NICU.

BACKGROUND: Central line-associated bloodstream infections (CLABSIs) are preventable hospital acquired infections that lead to an increased mortality rate and extended hospital length of stay. Reducing CLABSIs in neonates in a Level IV neonatal intensive care unit (NICU) is challenging due to the complexities of patient diagnoses requiring extended line days and variability in nursing line care. PURPOSE: The purpose of this quality improvement project is to decrease the rate of CLABSIs from 2 infections per 1000-line days (baseline rate from 2019 to 2022) to less than 1 infection per 1000-lin

Equity, inclusion and cultural humility: contemporizing the neonatal intensive care unit family-centered care model.

Existing NICU family centered care models lack the key elements of equity, inclusion and cultural humility. These models were conceived to support families during the stressful life event of an infant's NICU admission. Their development, however, occurred prior to recognition of the medical field's systematic shortcomings in providing equitable care and their impact on outcome disparities for marginalized communities; thus, they do not include cultural or equitable healthcare considerations. Given the significant neonatal care inequities for marginalized groups, incorporating the experience of

What Every NICU Nurse Should Know About Perinatal Mood and Anxiety Disorders.

Perinatal mood and anxiety disorders (PMADs) are the most common complications of pregnancy and the postpartum period, affecting approximately one in five women in the United States. Perinatal mood and anxiety disorders encompass postpartum depression, perinatal anxiety, obsessive-compulsive disorder, posttraumatic stress disorder, bipolar spectrum disorder, and perinatal psychosis. Despite their prevalence, PMADs remain significantly underdiagnosed and undertreated, with up to 75% of affected women not receiving adequate care. Untreated PMADs carry substantial consequences for maternal health

The Normal and Abnormal Equine Neonatal Musculoskeletal System.

The first weeks of life are critical in many aspects, and the musculoskeletal system is no exception. Being able to stand and nurse within hours of life is necessary for survival. Laxity, flexural deformities, and skeletal immaturity can all make it difficult for neonates to ambulate. The increased vascularity to bones and cartilage mixed with the newly forming immune system also make neonates susceptible to infections that we rarely see in adult animals. This article concentrates on orthopedic conditions seen in the first 2 weeks of life.

Double surface phototherapy on a fluid bed.

Jaundice is a common neonatal problem with a higher incidence in premature infants. Phototherapy is an established treatment modality, effective and safer than exchange transfusion. Double surface phototherapy is more effective. The present methods have drawbacks, viz limited availability, high cost of equipment and difficulties in nursing infants during phototherapy. A modification to the technique which overcomes these difficulties made with equipment available in most neonatal units is described with three case reports of hyperbilirubinaemia successfully treated using this method.

Assessment and care of the newborn with Down syndrome.

Well-defined, distinctive phenotypic features and natural history characterize Down syndrome, the most frequent form of developmental disability caused by a microscopically demonstrable chromosomal aberration. Triplicate state or trisomy of all or a portion of chromosome 21 causes Down syndrome. There are 3 genetic mechanisms leading to Down syndrome, or trisomy 21: nondisjunction, translocation, and mosaic. With advances in prenatal testing techniques, the diagnosis can occur before birth. This article explores the embryology and pathogenesis of Down syndrome and its multisystemic effects on

Cerebellar hemorrhage in extremely low birth weight infants: incidence, risk factors, and impact on long-term outcomes.

Improvements in neuroimaging technology and techniques have contributed to the increased recognition of cerebellar hemorrhage (CBH) in the prererm infant. Studies have indicated that the extremely low birth weight (ELBW) infant (< 1,000 g) is at highest risk for this injury. Associated risk factors include a constellation of antenaral, intrapartum, and neonatal factors, with immaturity, fetal distress, and cardiorespiratory instability in the early neonatal course as significant contributors. The long-term impact of CBH for the ELBW infant is not fully understood, but recent reports suggest th
