A case of gastrointestinal histoplasmosis with esophageal involvement.

Histoplasmosis is a common infection endemic to the Ohio and Mississippi River Valleys caused by the inhalation of Histoplasma capsulatum spores from contaminated soil. Most infections are asymptomatic; however, patients with impaired cellular immunity (HIV infection, hematologic malignancy, solid organ transplant, hematopoietic stem cell transplant or TNF-⍺ inhibitor use) are at risk for disseminated disease. Disseminated histoplasmosis commonly affects the lungs, liver, spleen, bone marrow and gastrointestinal tract. Esophageal involvement is rare and usually due to extrinsic compression fro…

AIDS with disseminated histoplasmosis.

This report is a description of two Ohio cases of acquired immunodeficiency syndrome (AIDS) and disseminated histoplasmosis, with discussion of diagnosis and treatment of this combination. The patient in case 1 developed disseminated histoplasmosis as the first significant symptomatic medical condition of his life. The patient in case 2 presented with severe pharyngitis, but without signs or symptoms specific to the lungs. Amphotericin B alone does not eradicate histoplasmosis in an AIDS patient. The best therapy at present is a full course of amphotericin B followed by a lifetime regimen of k…

Use of horizontal gaze nystagmus as a part of roadside sobriety testing.

Alcohol is a central nervous system depressant that can affect eye movement control. Even at low blood alcohol concentrations (BAC's) smooth pursuit eye tracking can be lost and horizontal nystagmoid movements become apparent at relatively small angles of lateral gaze. Law enforcement officers across the nation are being taught evaluation techniques to use these eye movement signs as an indicator of alcohol intoxication. Information obtained at the Ohio State Highway Patrol (OSHP) course on the Advanced Detection, Apprehension, and Prosecution of subjects suspected of driving a vehicle while i…

Tick-borne relapsing fever in the Eastern United States.

Tick-borne relapsing fever is endemic in the western part of the United States, but it has not been reported east of the Mississippi River. Sporadic cases have been reported in the eastern part of the United States, but travel to the West during the incubation period appeared to provide the source of infection. In the fall of 1975, a case of relapsing fever was diagnosed in Cincinnati in a child who had not traveled outside of Ohio, indicating the presence of Borrelia in this area. Serial serological studies indicated that B turicatae was the species involved. The occurrence of this case sugge…

Acute presumed histoplasmosis of the optic nerve head.

A 38-year-old man from the Ohio Valley area presented to the Ophthalmology Cinic at a west coast hospital (USA) because of sudden loss of the right inferior temporal visual field after severe right frontal headache of several hours' duration. During the following months, diffuse peripapillary atrophy and peripheral punched-out lesions developed. When he had been seen initially, the fundus of the left eye had also revealed extensive, diffuse, peripapillary atrophy and scarring. All laboratory tests were normal except a histoplasmin intradermal skin test. The clinical appearance of both eyes 8 m…

Racial disparity in previable birth.

BACKGROUND: Extremely preterm birth of a live newborn before the limit of viability is rare but contributes uniformly to the infant mortality rate because essentially all cases result in neonatal death. OBJECTIVE: The objective of the study was to quantify racial differences in previable birth and their contribution to infant mortality and to estimate the relative influence of factors associated with live birth occurring before the threshold of viability. STUDY DESIGN: This was a population-based retrospective cohort of all live births in Ohio over a 7 year period, 2006-2012. Demographic, pr…

Carotid artery stenosis as a cause of stroke.

BACKGROUND: Population-based studies have estimated that about 15% of ischemic strokes are caused by large-vessel cerebrovascular disease. We determined the types of large-vessel atherosclerosis responsible for ischemic strokes in a population-based stroke study. METHODS: Patients with first-ever or recurrent ischemic stroke in the Greater Cincinnati area were identified during 2005 at all local hospitals. Study physicians assigned ischemic stroke subtypes. Overall event rates and incidence rates for first-ever events were calculated, and age-, race- and sex-adjusted to the 2000 US population

Laryngotracheoplasty for subglottic stenosis in Down Syndrome children: the Cincinnati experience.

Previous studies from our institution have noted difficulties in the surgical repair of subglottic stenosis (SGS) in children with Down Syndrome. The objectives of this paper were to update our 15 year experience in the Down Syndrome patient population, compare our results with our overall series of laryngotracheoplasty for SGS, and to report on the increased incidence of posterior glottic stenosis (PGS) within this group of patients. Medical records of all children with SGS and Down Syndrome evaluated between 1982 and 1997 were reviewed for history of prior intubation, tracheotomy, gastroesop

What is the incidence of varicocele in a fertile population?

One hundred men with proven fertility who presented for vasectomy consultation were examined for testicular size and presence of a varicocele, including examination with the Doppler stethoscope for the presence of subclinical varicocele. A diagnosis of varicocele was established in 61%. In 17 patients, the varicocele was evident clinically, whereas in 44 patients it was subclinical (8 bilateral). There was a discrepancy in testicular size in 22 patients, but this group included 8 men without varicoceles and 7 with subclinical varicoceles. It is apparent that varicoceles, especially subclinical

A Case of Orbital Histoplasmosis.

Histoplasma capsulatum var capsulatum is a dimorphic fungus endemic to the Ohio and Mississippi River Valleys of the United States. In this case report, a 33-year-old woman who presented with a right orbital mass causing progressive vision loss, diplopia, and facial swelling is described. Lateral orbitotomy with lateral orbital wall bone flap was performed for excisional biopsy of the lesion. The 1.5 × 1.8 × 2.3 cm cicatricial mass demonstrated a granulomatous lesion with necrosis and positive staining consistent with Histoplasma capsulatum var capsulatum infection. To the authors' knowledge, …
