A male patient with a dental malocclusion presented with Epidermolysis Bullosa, a group of genetically determined diseases characterised by abnormal fragility of the skin and mucosa. Described is the management of the patient utilising Begg Light Wire fixed-appliance orthodontic treatment.

There has been much debate with respect to the ideal time to initiate orthodontic treatment. Recent clinical trials have tested the effects of early orthodontic treatment. The purpose of this study was to understand orthodontists' perspectives on the best time to initiate treatment, factors that preclude early treatment, and experiences with compliance or adherence problems among their younger patients. Questionnaires were distributed to 335 practicing orthodontists throughout the United States. Respondents were first asked to indicate the best stage to initiate orthodontic treatment for 41 di

The prevalence of dental anomalies including agenesis, crown shape, tooth position, root shape, and invagination were examined in 111 orthodontic patients; 74.77 per cent of the patients exhibited at least one dental anomaly. Invagination was found to be the most prevalent anomaly, whereas supernumerary teeth and root dilaceration were the least frequent anomalies. Dental invagination and short or blunt roots were significantly more prevalent in females than in males. Implications for orthodontic treatment planning are discussed.

This report reviews the orthodontic treatment of a patient suffering from a metabolic disease controlled by medication. The patient also had periodontal problems on both hard and soft tissue support. The results of involvement of general medicine, periodontal surgery, and proper biomechanics is shown and the problems of the multidisciplinary treatment discussed.

Many problems have been associated with the eruption of maxillary canines and careful supervision and early diagnosis of disturbances in eruption are portentous. A review of the literature concerning the impaction and treatment of the maxillary cuspids, followed by a case report, is presented. Incidence of impactions; most frequent location of impactions; common treatment for impactions; and etiology are discussed. Risks involved are enumerated. A case report of impacted maxillary cuspids utilizing a two-phase treatment (Sagittal II, Wilson 3D quad action, and straight wire appliance) is prese

Orthodontists are often concerned about the prognosis of root-filled teeth, particularly when extractions are required for orthodontic treatment. This review provides guidance on assessing the quality of root fillings, as well as the factors which affect the prognosis of root-filled teeth. The implications of previous traumatic injuries and the likelihood of root resorption during orthodontic tooth movement are discussed.

The wide range of interrelations between orthodontics and periodontology shows that besides the preventive significance in the mixed dentition, another focal point is the comprehensive rehabilitation of periodontally affected patients. With increasing experience in the field of adult orthodontics, setting therapeutic limits becomes increasingly difficult. It is impossible in principle to define in exact metric terms the probing depth or attachment loss up to which orthodontic tooth movement is feasible. The fact is rather that the orthodontic-periodontal treatment goal can be defined only afte…

More than 50% of pediatric malignancies are leukemias or lymphomas. Oral changes associated with these conditions include: gingival oozing, petechiae, hematomas, ulcerations, gingival pain, gingival hypertrophy, mucosal pallor, pharyngitis, and lymphadenopathy. Current medical management for patients with hematologic malignancies includes chemotherapy, radiation, surgery, bone marrow transplantation or a combination of these modalities. Nearly 60% of patients diagnosed today with malignancy will be long-term survivors. Patients undergoing orthodontic treatment when a diagnosis of malignancy is

Guided tissue regeneration techniques, which are used in the treatment of certain advanced Class II and Class II furca-involved teeth, have progressed from promising to predictable. However, furca-modification techniques remain important aspects of treatment. Tooth eruption, combined with standard furca-modification techniques, improves the prognosis for both the treated and the adjacent teeth. Continued eruption of periodontally involved molars improves the crown-to-root ratio and maintains the periodontium of the adjacent dentition. These same concepts can be used to treat restoratively comp

The cytotoxic and antibacterial effects of orthodontic appliances were assessed. Metallic devices used in orthodontics, such as molar bands, brackets, and archwires were tested by the agar overlay cytotoxicity test with mouse fibroblast cells. The same devices were tested for antibacterial effect with Streptococcus mutans and S. sanguis. The multicomponent devices, which are bonded with silver- and copper-based brazing alloys, were more cytotoxic than the single-component devices, probably because copper is more cytotoxic than nickel. The devices had a definite, but low, antibacterial effect, …

Computer-aided diagnosis of orthodontic anomalies is of particular interest in the screening of potential patients. General practitioners and public dental health workers would find considerable benefit in a reliable automated system capable of reflecting the clinical assessments of orthodontists. This study investigates the application of an expert system (Xpert Rule) to the assessment of treatment need using thirteen traits of malocclusion to construct a knowledge base. Decision trees were generated using a pruning option available with the expert system. The results of the study are encoura

Extraction of second molars as an orthodontic measure is a simple procedure which can lead to good clinical results. Indications as well as timing and long term effects on buccal occlusion are discussed. It is concluded that acceptable results may only be expected when the extraction is strictly indicated and carried out in cases when the third molars have reached their full crown stage but before radiographic evidence of root formation. Upper third molars with an angulation of less than 30 degrees and lower third molars with an angulation of 30-60 degrees to the occlusal plane are likely to g

A study was conducted to identify selection criteria for ordering pretreatment orthodontic radiographs. Thirty-nine orthodontists evaluated six test cases. They provided information on the rationale for ordering each specific radiograph and the impact of the radiograph on the diagnosis and treatment plan. Skeletal relationship of the jaws was the most common indication for a radiograph request, followed by root formation/length and molar position or development. Of the radiographs, 16% produced a change in diagnosis, and 20% produced a change in treatment plans. The criteria specified for the

Many anorexic and bulimic patients induce vomiting by inserting their fingers in the oral cavity. We hypothesized that finger pressure could lead to tooth movement, eventually resulting in the development of orthodontic abnormalities, particularly open-bite. Twenty-four females with eating disorders and 24 matched controls underwent orthodontic examination and completed a dental questionnaire. Orthodontic abnormalities were more commonly found in the eating disorder group and open-bite was the most common abnormality seen. The presence of open-bite or other orthodontic abnormality was not asso

A 14-year-old patient suffering from congenital absence of almost her entire permanent dentition was referred by her GDP. Management of the case required an integrated orthodontic and restorative approach. The authors describe how the imaginative use of a combination of relatively simple techniques achieved a greatly improved dental appearance for the patient.

Because of the increasing frequency of orthodontic treatments of patients with periodontal problems, the authors evaluate effects deriving from orthodontic treatment on periodontal tissues and provide practical indications to manage such patients. Experimental and clinical studies on effects of trauma from occlusion on periodontal tissues in different clinical conditions are examined. The authors have also considered studies dealing with periodontal response to different orthodontic therapy items, with particular regard to gingival inflammation degree and likelihood of root resorption. The Authors examine the possibility of resolving periodontal problems through orthodontic treatment and particularly: intrusion, extrusion, molars uprighting and tooth movement into infrabony defect, in order to reduce or eliminate the lesions. Finally they discuss methods and duration of the contention in these patients.
