From experience in six cases the anabolic steroid hormones, especially long-acting testosterone and estrogen preparations, are the treatment of choice in Paget's disease, as in postmenopausal osteoporosis. Details of the management of three patients over a period of four years are presented. Roughly 4 per cent of the population, mostly persons over 40, show some evidence of Paget's disease. Only a small number of them, however, have severe manifestations requiring treatment, such as pain, howing or fracture of the bones, pressure on nerves or heart failure. In rare cases malignant changes occu

A 70-year-old man with a 20-year history of heart failure developed atrial fibrillation and underwent pacemaker implantation; however, his heart failure continued to worsen. Imaging with 99m Tc-PYP scintigraphy revealed significant uptake in the myocardium and the right pelvic bone. Similarly, 99m Tc-methylene diphosphonate scintigraphy demonstrated positive uptake in both regions. A myocardial biopsy confirmed cardiac amyloidosis, and mass spectrometry identified ApoA-I amyloidosis. In addition, a bone biopsy of the right iliac wing resulted in a pathologic diagnosis of Paget disease of bone.

BACKGROUND AND PURPOSE: Mutations in the gene encoding valosin-containing protein (VCP) are related to myriad medical conditions, including familial amyotrophic lateral sclerosis, inclusion body myopathy, and frontotemporal dementia. There are several reports of a link between these mutations and early onset Parkinson disease (PD).
CASE DESCRIPTION: We report a 53-year-old PD patient with VCP mutation who later developed motor complications, thus receiving subthalamic nucleus deep brain stimulation (DBS) at the age of 56 years. However, myopathy emerged 1.5 years after surgery.
CONCLUSIONS:

Paget's disease of bone (PDB) is the second commonest metabolic bone disorder in the UK after osteoporosis and is both underdiagnosed and undertreated. PDB is often found incidentally on plain X-rays. There is effective treatment so identification of affected individuals is important. The aim was to conduct an audit to determine what proportion of individuals with X-ray evidence of PDB were referred to secondary care. A retrospective audit of X-rays reports in men and women over 55 years of age was undertaken over 18 months searching for the key word "Paget's." The images of possible cases wer

Histological analysis of undecalcified bone biopsy specimens is a valuable clinical and research tool for studying the etiology, pathogenesis and treatment of metabolic bone diseases. In case of osteoporosis, bone biopsy is not usually required for the diagnosis ; however, bone histomorphometry may be useful in rare cases with unusual skeletal fragility. Bone histomorphometry also provides valuable information on the mechanism of action, safety and efficacy of new anti-osteoporosis drugs. Bone histomorphometry is useful for the diagnosis and the assessment of treatment response in rickets/oste

Paget disease of the bone is a chronic disease characterized by accelerated bone turnover with abnormal repair leading to expansion, pain and deformities. The disease is common in the West, but little if any information is available on its existence in the Arab world, including Saudi Arabia. We present four cases of Saudi patients with Paget disease with variable presentations. The first case, a 63-year-old woman with a history of papillary thyroid cancer, presented with bone, shoulder and chest wall pain and foci of uptake in the ribs and skull that were thought to be metastases, indicating t

PURPOSE: To describe the clinical findings in a patient who had developed painless supraorbital prominence.
METHODS: Clinical and histopathological findings of the case are reviewed and presented.
RESULTS: The patient had an isolated histologically confirmed Paget disease of the orbit. Local excision of the lesion was performed.
CONCLUSION: Paget disease can present not only as a diffuse process but also as a painless, solitary lesion of the orbit.

The imaging evaluation of hearing loss is usually focused on inflammatory entities, especially chronic otitis/ cholesteatoma and cerebellopontine angles lesions, particularly acoustic tumors. This discussion concerns several developmental and acquired entities. The developmental entities referred to as the otodystrophies include otosclerosis, Paget's disease, fibrous dysplasia, and osteogenisis imperfecta. Otosyphilis and postradiation temporal bone changes are also included because of the overlapping imaging appearances.

The first report of a patient with gross Paget's disease and progressive hearing loss who successfully underwent hearing rehabilitation with an osseo-integrated hearing aid is presented. The otological manifestations of Paget's disease and the principles of osseo-integration are discussed. The use of a bone-anchored hearing aid (BAHA) in selected patients with Paget's disease can provide useful amplification and hearing rehabilitation.

We conducted a medical record-based study of 169 patients with polyostotic involvement identified among 200 Paget's disease patients. Follow-up was 15 to 41 years in 31 cases. The pelvis was the only bone that was more likely than not to be involved bilaterally. All the other paired bones were more likely to be involved unilaterally and when both sides were involved the two lesions were very often frankly asymmetric. In a given patient, the duration of the various pagetic lesions, estimated from their size and from data provided by an earlier study on the rate of progression of pagetic lesions

Clinical and diagnostic findings of Paget's disease of the jaws are reported; up to date etiological and pathogenical hypotheses are presented. Laboratory data of increased P.A. as a marker of disease evolution is pointed out, particularly the relationship increasing hydroxyprolinuria, because the increase of a malignant transformation. Surgery, always modeling, is limited to cases of severe facial dysmorphosis associated or not with the local disturbance due to compression; in any case it is suggested to perform the surgery in the phases of inactivity of disease, to avoid local complications,

The coexistence of Paget's bone disease (PBD) and diffuse idiopathic skeletal hyperostosis (DISH) of the spine is controversial and could have implications for the pathogenic mechanisms involved in these disorders. In order to assess the prevalence of DISH in patients with Paget's disease, a prospective controlled study was carried out in which roentgenograms were obtained from a group of 50 consecutive ambulatory patients previously diagnosed as having PBD (25 males and 25 females; mean age: 66.2 +/- 8.9 years) and these were compared with 50 age- and sex-matched subjects selected randomly fr

An original case is reported: spinal cord compression due to extramedullary hematopoiesis of the epidural space, related to Paget's disease of bone, successfully treated by surgery. Extramedullary hematopoiesis, usually occurring in cases of hematological disease, can also complicate Paget's disease, probably by bone effraction mechanism. A review of literature has found 5 other cases of extramedullary hematopoiesis occurring in Paget's disease, without hematological disease. Medullar scintigraphy (Indium111, Technetium99 Sulfur colloid) can confirm the diagnosis after X-ray findings, without

Malignant change in Paget's disease of bone usually has the histological features of osteosarcoma, but follows a different clinical course from cases not associated with paget's disease of bone. Malignant fibrous histiocytoma is an uncommon neoplasm which may be associated with pagetic bone and its clinical course is not well defined. Seven cases of malignant fibrous histiocytoma in Paget's disease were reviewed from two Australian bone tumour registries. In stage IIB and III disease there was a three year survival rate of 57%, suggesting a better prognosis than for patients with other sarcoma

After analyzing the various types of anatomical and radiographic fractures in pagetic bone, the authors explain the reason for their preference for surgical treatment if the lesion is localized in the lower limbs. This choice allows for a reduction in the duration of immobilization and a lowered incidence of local (rapid and intense osteolysis of the pagetic bone) and general complications (hypercalciuria, at times, hypercalcemia). It is best to resort to internal osteosynthesis for diaphyseal fractures and prosthetic substitution for fractures of the femoral neck where treatment with screws a

The audiometric data of 41 patients with Paget's disease of the bone show that hearing loss exceeds the age-related presbycusis in patients with deformities of the temporal bone as demonstrated by X-ray (27%). The usual type of hearing loss is a sensorineural one. 80% of these patients showed hair cell damage, 32% of them a retrocochlear lesion, depending on the degree of deformity of the temporal bone. Conductive hearing loss of the otosclerosis type was found in only 20%. The Paget-related hearing loss can be regarded as a consequence of the deformation occurring both at the bone surrounding
