Primary Nocardia osteomyelitis as a presentation of AIDS.

Human infection with Nocardia species presents as a wide range of clinical syndromes. Nocardia is an important opportunistic pathogen in immunocom-promised patients. We report a case of primary Nocardia asteroides osteomyelitis as the initial clinical presentation of AIDS. The infection was successfully treated with a prolonged course of trimethoprim-sulfamethoxazole in conjunction with HAART. Nocardia osteomyelitis should be recognized as an unusual but important and treatable opportunistic infection in patients living with HIV infection.

Acute (haematogenous) osteomyelitis of the skull in childhood.

A 13-year old boy presented with a 2-week history of right frontotemporal headache, fever and progressive painful swelling in that region. There was no preceding trauma. Clinical and radiological evaluation confirmed osteomyelitis of the right frontal bone. Staphylococcus aureus was found to be the causative organism. The affected bone was debrided and appropriate antibiotics administered. A subdural abscess developed requiring drainage. Hospital stay was 8 weeks. Haematogenous (pyogenic) osteomyelitis of the skull is rare in childhood and diagnosis may be missed or delayed. A high index of su…

Primarily infected cephalhematoma and osteomyelitis in a newborn.

Cephalhematomas are subperiosteal blood collections occurring in newborns secondary to trauma at birth. They develop within a few days and are subsequently resorbed. Infection of a cephalhematoma is unusual and caused most often by colonization of the hematoma during bacteremia or by direct inoculation secondary to trauma. Less than 10 patients with primary infection of the hematoma, in the absence of a positive blood culture, complicated by osteomyelitis have been described. We report a newborn with a primarily infected cephalhematoma complicated by parietal bone osteomyelitis.

Osteomyelitis and osteonecrosis in inflammatory bowel disease.

Osteomyelitis and osteonecrosis are skeletal disorders seen in patients with inflammatory bowel disease (IBD). Osteomyelitis usually occurs in the pelvic bones, especially in complicated Crohn's disease, presumably by direct extension from a pelvic inflammatory mass, abscess or fistulous tract. Diagnosis of osteomyelitis may be difficult and can lead to spinal extension of the septic process with a resultant neurological deficit, including paraplegia. Osteonecrosis or avascular necrosis has been reported in patients with either ulcerative colitis or Crohn's disease, often, but not exclusively,…

Blastomycotic osteomyelitis of the pelvis: a case report.

Blastomycosis dermatitis is an unusual fungal infection that can become a diagnostic dilemma if a high index of suspicion is not maintained in patients who present with a chronic granulomatous infection of bone and soft tissue. We present a patient with a long history of pain, systemic manifestations of chronic infection, a draining sinus, and lytic changes on plain films to illustrate the difficulty in establishing the diagnosis. Appropriate staging studies and a bone biopsy led to the correct diagnosis in the patient.

Ultrasonic features of acute osteomyelitis in children.

The ultrasonic findings in 38 children with osteomyelitis of the limb bones were analysed in four time-related groups based on the interval between the onset of symptoms and the ultrasonic examination. Deep soft-tissue swelling was the earliest sign of acute osteomyelitis; in the next stage there was periosteal elevation and a thin layer of subperiosteal fluid, and in some cases this progressed to form a subperiosteal abscess. The later stages were characterised by cortical erosion, which was commonly present in those who had had symptoms for more than a week. Concurrent septic arthritis was r…

Successful treatment of coccidioidomycosis osteomyelitis in an infant.

A 10-month-old Hispanic male infant with expansile lesions of the third metacarpal and proximal phalanx positively diagnosed as Coccidioides immitis osteomyelitis is presented. Treatment consisted of combined surgical debridement and systemic antifungal therapy and resulted in complete resolution of the lesions. Treatment was guided by clinical response and complement fixation titers. Osteomyelitis is a relatively infrequent manifestation of disseminated coccidioidomycosis. Neonates and infants appear to be more susceptible to the development of dissemination, but less likely to develop toxici…

Coccidioidal osteomyelitis of a metatarsal. A case report.

Coccidioidal osteomyelitis is difficult to diagnose. Confirmation depends on culture and histopathological examination. This rare type of fungal osteomyelitis is to be considered in the differential diagnosis of a chronic infection in some areas of the world. Specific treatment with excision and amphotericin-B can be effective.

[Treatment of a refractory chronic osteomyelitis of the jaw].

Two cases of refractory chronic osteomyelitis of the jaw were presented, the first in a patient with pycnodysostosis, the second that had been receiving radiotherapy. Both cases were primary chronic osteomyelitis with a markedly protracted course of recovery, in spite of the antibiotic and local irrigation therapy. However the lesion was cured by the removal of sequesters and granulation tissues without radical treatment such as a decorticotomy. Radical surgery, decortication, is an effective procedure in the treatment of chronic osteomyelitis of the jaw, but treatment with the surgical proced…

Septic osteomyelitis and polyarthritis with ureaplasma in hypogammaglobulinemia.

We describe a hypogammaglobulinemic woman with a one-year history of destructive septic osteomyelitis and polyarthritis with positive cultures for Ureaplasma urealyticum from joint exudate and blood. The clinical course was complicated by subcutaneous abscesses from which both U. urealyticum and Mycoplasma hominis were grown. Multiple routine cultures had been negative, except for sporadic findings of Staphylococcus epidermidis before specific cultures for mycoplasmas were performed. Therapy with beta-lactam antibiotics, clindamycin, rifampicin, fusidic acid and aminoglycosides had been given …

[Pneumococcal infections in children: osteomyelitis and arthritis].

A pneumococci-osteoarthritis in 8 children is reported on. In almost all the cases an adequate therapy (antibiotics, immobilization, physical therapy) resulted in a healing process free of defects with a full maintenance of the articular function. Besides by staphylococci and haemophilus influenzae, pneumococci too use to play a role as pathogenic organisms for osteomyelitis and arthritis.

[Diffuse sclerosing osteomyelitis--a review and case].

The bone responds to stimulation with new bone formation, resorption or the combination of both. Exclussive bone formation i.e. sclerosation is defined as an increase in the bony mass which is a result of either decreased resorption or excessive bone production. The authors describe the following inflammatory sclerosing jaw lesions: sclerosing osteitis, sclerosing periostitis of Garre and diffuse sclerosing osteomyelitis. The etiopathogenesis, clinical features, histologic findings, diagnostic procedures, therapy and prognosis are presented for each pathologic entity. Further a case of rare lo…

Osteomyelitis: detection with US. Work in progress.

Ten patients with clinically suspected osteomyelitis were studied with ultrasound (US). Five patients had abnormalities detectable on sonograms. An abnormal fluid collection was demonstrated adjacent to the bone in three of these five. US-guided aspiration of the fluid in two of the three patients revealed purulent or infected fluid that was thought to represent an inflammatory exudate dissecting in a subperiosteal or extraperiosteal location. Findings from subsequent radiographic and scintigraphic studies confirmed the diagnosis of osteomyelitis in these three patients. The other two patients…

Pneumococcal osteomyelitis. An unusual cluster of cases.

Between 1979 and 1981, 22 children with osteomyelitis and/or septic arthritis were hospitalized at Soroka University Hospital. Streptococcus pneumoniae was cultured in five of the children, four of whom were under 2 years of age. In four other children under 2 years of age, Haemophilus influenzae was cultured. Staphylococcus aureus was identified less frequently in the younger age group. In deciding on initial antibiotic therapy, the possibility of such a bacteriological spectrum should be considered.

Vertebral osteomyelitis due to Nocardia in a dog.

Vertebral osteomyelitis in a Kelpie crossbred bitch was diagnosed on clinical and radiological grounds. Nocardia spp was isolated from a sample of the infected bone obtained during surgery. Decompressive surgery was performed in an attempt to relieve the pressure of the infected vertebrae on the spinal cord. The contraindication of the use of corticosteroids is emphasised when dealing with back pain of unknown aetiology.
