Acute visual and auditory neurotoxicity in patients with end-stage renal disease receiving desferrioxamine.

We report 5 cases of acute visual and auditory neurotoxicity in patients with end-stage renal disease receiving desferrioxamine. Four patients complained of visual toxicity characterized by a decrease in visual acuity, loss of color vision and night blindness. In these patients there was a decrease in visual acuity, and the Farnsworth's test revealed a tritanopy (yellow-blue axis impairment); 1 patient also showed retinal pigmentary changes. A fifth patient complained of bilateral and severe neurosensorial deafness. Patients improved clinically upon drug-withdrawal. We recommend careful monito

Acute toxic blindness: unrecognized quinine poisoning.

Two cases of acute blindness due to quinine poisoning are presented. In both cases, the diagnosis was initially unsuspected. In addition, tinnitus, decreased hearing, vomiting, abdominal pain, and confusion were noted in one patient, and the other experienced decreased hearing, headache, confusion, tachycardia, later bradycardia, and first-degree atrioventricular block. The onset of blindness was delayed more than 12 hours after ingestion in both cases. Quinine levels of 13.6 micrograms/mL and 18.6 micrograms/mL were demonstrated (therapeutic = 1 to 3 micrograms/mL). One patient developed mark

Complex visual hallucinations and cyclosporine.

Insidious loss of visual acuity and simultaneous onset of complex visual hallucinations were observed in a patient with acute lymphoblastic leukemia who had recently received bone marrow transplantation and was being treated with prednisone and cyclosporine to suppress graft-v-host disease. Problems with visual acuity and visual hallucinations spontaneously ameliorated with reduction and termination of cyclosporine treatment. The complex visual hallucinations caused considerable psychological distress for the patient. These hallucinatory phenomena were placed within the framework of loss of co

Acute bilateral cataract with phacomorphic glaucoma in a girl with newly diagnosed type 1 diabetes mellitus.

The authors present a rare case of acute bilateral cataract with phacomorphic glaucoma in a girl with newly diagnosed type 1 diabetes mellitus without a known history of ocular problems. Within 3 months after the diagnosis of diabetes mellitus, she presented with high intraocular pressure. Her visual acuity was limited to hand motions. The patient required immediate surgical intervention. Postoperatively, the intraocular pressure normalized and bilateral visual acuity was 6/6.

Visual acuity in unilateral cataract.

BACKGROUND: Patching the fellow eye in infancy is a well recognised therapy to encourage visual development in the lensectomised eye in cases of unilateral congenital cataract. The possibility of iatrogenic deficits of the fellow eye was investigated by comparing the vision of these patients with untreated unilateral patients and binocularly normal controls. METHODS: Sweep visual evoked potentials (VEPs) offer a rapid and objective method for estimating grating acuity. Sweep VEPs were used to estimate acuity in 12 children aged between 4 and 16 years who had had a congenital cataract removed

Visual recovery after transsphenoidal removal of pituitary adenomas.

We reviewed the records of 100 consecutive patients with histologically verified pituitary adenomas who underwent transsphenoidal decompression of the optic nerves and chiasm. The patients' ages ranged from 18 to 80 years, with a median of 52 years. Preoperatively, all patients had objective signs of visual acuity or field defects. Postoperatively, visual acuity was normal or improved in 79% of the eyes and the visual fields were normal or improved in 74%. The visual outcome (for both acuity and fields) was better in younger patients and those with a shorter duration of symptoms. Patients with

Case Report: Implantable Collamer Lens for Keratoconus with Splinter Cataract.

BACKGROUND: Implantable collamer lens (ICL) could be an effective and safe treatment option for patient with keratoconus. The presence of cataract is considered a relative contraindication to the use of ICL. In this case report, we describe the outcomes of ICL implantation in an eye with keratoconus and splinter cortical cataract sparing the visual axis. CASE REPORT: Thirty-six-year-old female patient diagnosed with keratoconus was followed in the outpatient department and sought to be independent of glasses in the right eye. Upon examination she was found to have bilateral splinter cortical

[Visual Acuity and Visual Field in Optic Disc Drusen].

BACKGROUND: It has been assumed that visual field defects in optic disc drusen slowly increase with age or occur during adolescence and do not change substantially in later years. In our study, we aimed to validate these assumptions. MATERIAL AND METHODS: 255 consecutive cases with optic disc drusen were identified from the patient records of the University Eye Hospital Tübingen; the diagnosis was verified and visual fields were quantified as long as available and of sufficient quality. Additionally, visual acuity was evaluated. RESULTS: In 104 cases, quantifiable visual fields of sufficient

Visual prognosis better in eyes with less severe reduction of visual acuity one year after onset of Leber hereditary optic neuropathy caused by the 11,778 mutation.

BACKGROUND: Patients with Leber hereditary optic neuropathy (LHON) have a progressive decrease of their visual acuity which can deteriorate to <0.1. Some patients can have a partial recovery of their vision in one or both eyes. One prognostic factor associated with a recovery of vision is an early-age onset. The purpose of this study was to determine other clinical factors that are predictive of a good visual recovery. METHODS: Sixty-one Japanese LHON patients, with the 11,778 mutation and a mean age of 23.1 ± 12.1 years at the onset, were studied. All patients were initially examined at an a
