Joint Diabetic Macular Edema Segmentation and Characterization in OCT Images.

The automatic identification and segmentation of edemas associated with diabetic macular edema (DME) constitutes a crucial ophthalmological issue as they provide useful information for the evaluation of the disease severity. According to clinical knowledge, the DME disorder can be categorized into three main pathological types: serous retinal detachment (SRD), cystoid macular edema (CME), and diffuse retinal thickening (DRT). The implementation of computational systems for their automatic extraction and characterization may help the clinicians in their daily clinical practice, adjusting the di

Effectiveness of Practices To Increase Timeliness of Providing Targeted Therapy for Inpatients with Bloodstream Infections: a Laboratory Medicine Best Practices Systematic Review and Meta-analysis.

BACKGROUND: Bloodstream infection (BSI) is a major cause of morbidity and mortality throughout the world. Rapid identification of bloodstream pathogens is a laboratory practice that supports strategies for rapid transition to direct targeted therapy by providing for timely and effective patient care. In fact, the more rapidly that appropriate antimicrobials are prescribed, the lower the mortality for patients with sepsis. Rapid identification methods may have multiple positive impacts on patient outcomes, including reductions in mortality, morbidity, hospital lengths of stay, and antibiotic us

An Expert-Derived Data-Driven Approach to Identifying Medicaid Children at Risk of Out-of-Home Placement: Development, Application, and Data-Sharing Considerations.

BACKGROUND: Out-of-home placement (OOHP) through child welfare, residential substance treatment, or extended inpatient care has life-altering impacts on child health and generates an outsized proportion of pediatric Medicaid costs. Early risk identification may prevent OOHP but is challenged by the difficulty of navigating state and federal data sharing regulations and multiple disconnected data systems. Identifying ways to streamline and simplify this process are critical to OOHP prevention. OBJECTIVES: To describe the development and implementation of an expert-derived data-driven algorithm

Creation of an Interactive Dashboard to Facilitate Early Detection of Cardiac Amyloidosis in African American Veterans.

BACKGROUND: Cardiac amyloidosis (CA) is an underdiagnosed cause of heart failure (HF) that disproportionately impacts men of African descent. Without a standardized method of screening and scattered patient health information, clinicians must integrate data that spans multiple disease systems and is stored across the electronic health record. OBJECTIVES: The aim of this project was to create a dashboard to facilitate identification of high-risk African American (AA) veterans who would benefit from CA screening tests. This paper described the development of the dashboard and identified barrier

A patient-centred web-based adverse drug reaction reporting system identifies not yet labelled potential safety issues.

PURPOSE: Reporting of adverse drug reactions (ADRs) by patients is essential for a comprehensive risk-benefit evaluation of drugs after marketing, but only few data are available regarding patient-centred web-based ADR reporting systems. Hence, we aimed to analyze ADRs reported by patients with a particular emphasis on novel drugs and serious ADRs not yet labelled in the respective summary of product characteristics (SPC). METHODS: All ADR reports received by a web-based, patient-centred platform ( www.nebenwirkungen.de ) between April 1, 2019, and September 1, 2020, were descriptively analyz

aiDIVA - hybrid AI for rare disease diagnostics using evidence-based, machine learning and language models.

Genome sequencing enables accurate detection of genetic variants and is transforming rare disease diagnostics. While data generation is scalable, prioritization and clinical interpretation remain challenging, often requiring expert manual classification. AI-driven decision support systems are therefore needed to assist in causal variant identification or to fully automate large-scale re-analysis of unsolved cases. Existing tools often estimate variant impact on protein function, but few integrate genomic, phenotypic, and clinical annotation data for diagnosis. We present aiDIVA, an ensemble-AI

From Brain Organoids to Translational Neurology: Exploring Neuroprotective Targets and Molecular Approaches in Perinatal Brain Injury.

Perinatal brain injury (PBI) is a leading cause of long-term neurological deficits in newborns, yet effective therapies are limited. At the cellular level, PBI involves hypoxic-ischemic stress, neuroinflammation, oxidative damage, excitotoxicity, and disrupted neurovascular and glial development. Traditional animal models and 2D cultures cannot fully capture the spatiotemporal complexity of the developing human brain, highlighting the need for more physiologically relevant systems. Human brain organoids have emerged as advanced three-dimensional models that recapitulate region-specific cytoarc

Interpreting the Black Box: Interpretable Machine Learning and Systems Pharmacology in Small-Molecule Therapeutics.

Small-molecule drug development faces high attrition rates driven by complex pharmacokinetics and unforeseen toxicities. While deep learning offers high predictive accuracy, its opaque "black-box" nature hinders mechanistic transparency, clinical trust, and regulatory approval. This review synthesizes how Interpretable Machine Learning, synergized with systems pharmacology, advances this paradigm by enhancing mechanistic transparency in drug development. By providing insights into algorithmic decisions, Interpretable Machine Learning helps researchers identify molecular features that are stati

Metabolic dysfunction-associated steatotic liver disease as a systemic disorder: extrahepatic manifestations and the need for data-driven phenotyping.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is increasingly recognized as a systemic disorder that extends beyond hepatic steatosis to involve multiple extrahepatic organs, including the cardiovascular system, kidneys, brain, pancreas, and other organ systems. This multisystem involvement is accompanied by marked heterogeneity in clinical presentation and disease progression, supporting the concept that MASLD comprises distinct phenotypes rather than a uniform entity. Conventional classifications based solely on hepatic features are insufficient to capture this complexity,

High-altitude pulmonary hypertension: a comprehensive review of mechanisms and management.

High-altitude pulmonary hypertension (HAPH) is characterized by an increase in pulmonary artery pressure due to prolonged exposure to hypoxic environment at high altitudes. The development of HAPH involves various factors such as pressure changes, inflammation, oxidative stress, gene regulation, and signal transduction. The pathophysiological mechanisms of this condition operate at molecular, cellular, and genetic levels. Diagnosis of HAPH often relies on echocardiography, cardiac catheterization, and other methods to assess pulmonary artery pressure and its impact on cardiac function. Treatme

Bridging the Gap in Chronic Disease Management: A Nursing Perspective on the Use of Predictive Tools and Telemedicine in the Hospital-Community Transition.

Chronic diseases represent one of the most complex, costly, and significant challenges for healthcare systems. The increase in chronic conditions and multimorbidity, together with the growing demand for continuity of care makes the vulnerability of the hospital-to-community transition increasingly evident. This phase is often characterized by delays, fragmented services, and insufficient support for patients and caregivers, leading to higher rates of early readmission and substantial clinical, social, and economic impacts. This paper was developed through a narrative synthesis of international

Patient-Derived Induced Pluripotent Stem Cell Models for Phenotypic Screening in the Neuronal Ceroid Lipofuscinoses.

Batten disease or neuronal ceroid lipofuscinosis (NCL) is a group of rare, fatal, inherited neurodegenerative lysosomal storage disorders. Numerous genes (CLN1-CLN8, CLN10-CLN14) were identified in which mutations can lead to NCL; however, the underlying pathophysiology remains elusive. Despite this, the NCLs share some of the same features and symptoms but vary in respect to severity and onset of symptoms by age. Some common symptoms include the progressive loss of vision, mental and motor deterioration, epileptic seizures, premature death, and in the rare adult-onset, dementia. Currently, al

Protein-Losing Enteropathy Following Fontan Palliation.

Protein-losing enteropathy (PLE) is a chronic condition involving multiple organ systems that may develop any time following Fontan completion. The pathogenesis of PLE is complex and multifactorial. Chronic venous hypertension, low cardiac output, and abnormal lymphatics may all play a role in the pathogenesis of PLE. Common signs and symptoms include chronic diarrhea, abdominal pain, and ascites. Diagnosis is based on the presence of signs and symptoms in addition to hypoalbuminemia and elevated stool alpha 1 antitrypsin. Early identification and a comprehensive approach to evaluation and tre

Integrated Management of Skin NTDs-Lessons Learned from Existing Practice and Field Research.

Integration of neglected tropical diseases (NTDs) into the public health agenda has been a priority in global health for the last decade. Because a number of these diseases share not only the geographical distribution, but also a common feature which is skin involvement, bringing together a sub-group of 'skin NTDs' is one way forward to promote further integration among NTDs. With these diseases, which include leprosy, Buruli ulcer, yaws, mycetoma, lymphatic filariasis, and leishmaniasis, patients may be left with life-long deformities and disabilities when diagnosis and treatment are delayed.
