CE: A Review of Common Oral Treatments for Breast Cancer: Improving Patient Safety in Nononcology Settings.

: Breast cancer patients are living longer with the disease than ever before. According to the National Cancer Institute, more than 3 million women in the United States are currently living with a breast cancer diagnosis, and many seek care in nononcology settings, whether for treatment, acute symptoms and complaints related to their cancer diagnosis, or unrelated concerns. Yet many nononcology providers are unfamiliar with the various oral agents used to treat breast cancer, and their possible adverse effects and drug interactions. It is imperative that all providers be aware of these agents

Clinical validity and relevance of accidental puncture or laceration as a patient safety indicator for children.

PURPOSE: Accidental puncture or laceration (APL) has been endorsed as a patient safety indicator and is being used to compare hospital performance and for reimbursement. We sought to determine the positive predictive value (PPV) of APL as a quality metric in a pediatric population. METHODS: We retrospectively reviewed all cases that met APL administrative criteria over 5years in a quaternary pediatric hospital system. Events were categorized as false positive (FP) or true positive (TP). TP cases were further categorized as "potentially consequential" or "inconsequential". The PPV of APL was c…

Safety and efficacy of a patient-controlled bladder management system for treating urinary retention in men.

AIMS: The CymActive™ Bladder Management System (BMS) is a self-retaining, intraurethral catheter with a patient-controlled magnetic valve that allows cyclical bladder filling and emptying, without external appliances. We determined the safety and efficacy of the BMS in men with urinary retention who required catheterization for more than 7 days. METHODS: Men requiring continuous drainage, bladder capacity less than 300 ml, history of prostatic or urethral surgery, or urethral length outside of defined limits, were excluded. Data were collected from patient diaries and weekly visits during cat

Outpatient parenteral antimicrobial therapy with ceftriaxone for acute tonsillopharyngitis: efficacy, patient satisfaction, cost effectiveness, and safety.

BACKGROUND: Outpatient parenteral antimicrobial therapy (OPAT) is the administration of intravenous antimicrobial therapy to patients in an outpatient setting. It may be used for patients who have infections that require parenteral treatment but who are otherwise stable enough to not require admission as inpatients. OBJECTIVE: We aimed to review the treatment of patients with acute tonsillopharyngitis at the OPAT health care clinic in the Bahrain Defense Force Royal Medical Services (BDF-RMS), with regard to efficacy, patient satisfaction, cost effectiveness, and safety. METHODS: A retrospec

The role of emotion in patient safety: Are we brave enough to scratch beneath the surface?

Healthcare professionals work in emotionally charged settings; yet, little is known about the role of emotion in ensuring safe patient care. … responses to clinical situations. Evidence to date suggests that emotion plays an integral role in patient safety. Implications for training, practice and research are discussed in addition to strategies to facilitate health services to understand and respond to the influence of emotion in clinical practice.

Perampanel in the management of partial-onset seizures: a review of safety, efficacy, and patient acceptability.

Perampanel (PER) is a novel antiepileptic drug recently introduced for the adjunctive treatment in epilepsy patients aged 12 years or older with partial-onset seizures with or without secondary generalization in the US and Europe. Its antiepileptic action is based on noncompetitive inhibition of postsynaptic AMPA receptors, decreasing excitatory synaptic transmission. Evaluation of efficacy in three placebo-controlled randomized Phase III studies showed that add-on therapy of PER decreased seizure frequencies significantly compared to placebo at daily doses between 4 mg/day and 12 mg/day. PER'

The rate of obstetric anal sphincter injuries in Finnish obstetric units as a patient safety indicator.

OBJECTIVE: To study whether there are significant differences in the rate of obstetric anal sphincter injuries (OASIS) between the different sized delivery units in Finland. STUDY DESIGN: The study was performed as a population based registry study in Finland, including all births (294725) between 2006 and 2010. All the Finnish delivery units (34) were categorized by the number of annual deliveries and the OASIS rate was then compared between the different sized delivery units using a logistic regression analysis adjusting for maternal age and parity. The Robson ten group classification was u

Postpartum hemorrhage: the role of the Maternal-Fetal Medicine specialist in enhancing quality and patient safety.

Postpartum hemorrhage in excess of 1000 mL affects 2.9-4.3% of deliveries in North America and the prevalence is increasing (Calvert et al., 2012(1); Callaghan et al., 2010(2)). Given the unpredictable nature of most postpartum hemorrhages, all obstetric providers must understand the initial steps in the assessment and management of this emergency. In this monograph we will review the potential role of the Maternal-Fetal Medicine (MFM) specialist in managing this acute obstetric emergency. MFMs are uniquely positioned to develop hospital protocols, advocate for investment in resources to optim

Long-term safety, efficacy, and patient acceptability of teriparatide in the management of glucocorticoid-induced osteoporosis.

Glucocorticoids are commonly prescribed medications to treat multiple diseases across many medical specialties. One of the most common yet largely unappreciated side effect of glucocorticoid use is increased risk of fracture. Many different therapies are indicated to prevent and treat this condition; many guidelines exist that suggest appropriate use of both glucocorticoids and the medications approved to prevent this common side effect of glucocorticoid therapy. Nevertheless, 30%-50% of patients on long-term glucocorticoid therapy sustain a fracture. Teriparatide, recombinant human parathyroi

Triangulating case-finding tools for patient safety surveillance: a cross-sectional case study of puncture/laceration.

OBJECTIVE: To evaluate the need for triangulating case-finding tools in patient safety surveillance. This study applied four case-finding tools to error-associated patient safety events to identify and characterise the spectrum of events captured by these tools, using puncture or laceration as an example for in-depth analysis. DATA SOURCES/STUDY SETTING: Retrospective hospital discharge data were collected for calendar year 2005 (n=48,418) from a large, urban medical centre in the USA. STUDY DESIGN: The study design was cross-sectional and used data linkage to identify the cases captured by …

Review of the safety, efficacy and patient acceptability of the combined dienogest/estradiol valerate contraceptive pill.

The aim of this review is to define the role of the combined dienogest (DNG)/estradiol valerate (E2V) contraceptive pill, in terms of biochemistry, metabolic and pharmacological effects and clinical application as well. E2V is the esterified form of 17β-estradiol (E2), while dienogest is a fourth-generation progestin with a partial antiandrogenic effect. The cycle stability is achieved with 2 to 3 mg DNG, supporting contraceptive efficacy. In this new oral contraceptive, E2V is combined with DNG in a four-phasic dose regimen (the first two tablets contain 3 mg E2V; the next five tablets includ

Urinary diversion in the vesico-vaginal fistula patient: general considerations regarding feasibility, safety, and follow-up.

Some women with vesico-vaginal fistulas (VVF) have injuries so severe that they cannot be repaired in a way that would restore continence. The management of these women has been a source of controversy among the providers of VVF care. It would seem logical that urinary diversion surgery could relieve the suffering endured by women with unrelenting urinary incontinence. However, there is little objective evidence on which to base determinations on the safety and practicality of performing urinary diversion in the desperately poor areas where fistulas occur. As in all other areas of VVF care, mo

Safety of women in mixed-sex and single-sex medium secure units: staff and patient perceptions.

BACKGROUND: The development of single-sex medium secure units for women has been driven by concern about the vulnerability of women to sexual abuse and exploitation in mixed-sex secure settings. Less is known about how women patients and staff perceive gender segregation and their experiences in single-sex units. AIMS: To examine the impact of gender segregation on the safety of women patients detained in medium secure psychiatric facilities. METHOD: A qualitative study was conducted involving individual interviews with 58 male and female staff and 31 women patients in single-sex and mixed-s

Clinical Communication Response Latency as an Operational Efficiency and Patient Safety Indicator in Acute Care Hospitals.

OBJECTIVES: Timely clinical communication is a core safety function in acute care, yet response latency remains poorly measured. This study treats it as a stratified safety indicator and introduces a dual-phase model distinguishing verbal triage (T₀verbal) from physical intervention (T₀physical). METHODS: A mixed-methods observational design evaluated nurse call response latency across 3 anonymised hospitals: Hospital A (public, no-voice), Hospital B (private, no-voice), and Hospital C (private, voice-enabled). Voice-enabled systems provide 2-way audio between the patient and the nurse before

Design limitations and unintended consequences of the AHRQ patient safety indicator for endovascular thrombectomy stroke care.

BACKGROUND: To monitor endovascular and surgical care quality, the US Agency of Healthcare Research and Quality (AHRQ) has established a "failure-to-rescue" safety metric - Patient Safety Indicator (PSI) 04: Death Rate among Surgical Inpatients with Serious Treatable Complications. While a useful metric for elective procedures, PSI 04 is also applied to emergent endovascular thrombectomy (EVT) for acute ischemic stroke, which has a high rate of unavoidable mortality due to the presenting stroke. METHODS: PSI 04 rates after endovascular thrombectomy for stroke were identified by analysis of th…
