Protective and destructive immune reactions in apical periodontitis.

Apical periodontitis is initiated primarily by the mixed microflora of infected root canals. Continuous flow of bacteria and their products through the apical foramen induces influx, activation and coordinated interaction of immune-inflammatory cells within the periapical area. Successful mobilization of host defense mechanisms prevents abundant extraradicular bacterial invasion. However, anti-infective effector mechanisms are not restricted to killing the invading microorganisms but also destroy normal tissue components and induce bone absorption, resulting ultimately in the loss of the affec

Allotransplantation of tooth: a case report.

Tooth transplantation has attracted great interests since ancient times. A successful case of tooth allotransplantation is presented. A mandibular first premolar from the donor was implanted into the socket of maxillary central incisor Follow up after 12 months indicated good periapical healing with no resorption. Clinically, the transplantation site was free of symptoms and there was no evidence of periodontal disease or tooth mobility. This article suggests tooth transplantation as an alternative to other restorative options.

Different treatment protocols for different pulpal and periapical diagnoses of 72 cracked teeth.

INTRODUCTION: The treatment plan for cracked teeth depends on the extent of the crack. A tooth with an extensive crack of long duration may be more likely to require root canal treatment. The purpose of this study was to analyze the characteristics of cracked teeth and to assess the outcome of different treatment protocols depending on the pulpal and periapical diagnoses. METHODS: Seventy-two of 476 crown-restored teeth were diagnosed as cracked teeth. The location of the cracked teeth, age and sex of the patients, restoration materials, a diagnosis of pulp and apex, and the periodontal probi

Mandibular disto-lingual root: a consideration in periodontal therapy.

BACKGROUND: Developmental and morphological abnormalities may contribute to the progression of localized periodontal disease. Although the presence of a disto-lingual root in the mandibular first molar is rare, its role in periodontal destruction has not been examined. The purpose of this study was to evaluate whether the presence of this root contributes to localized periodontal destruction. METHODS: The presence of the disto-lingual root was identified by examination of two periapical radiographs in each of 197 Taiwanese patients with 332 mandibular first molars. Regression analysis was use

Treatment opinions for dens invaginatus: A case series.

Dens invaginatus (DI) is a rare congenital dental malformation characterized by enamel or cementum folded into dentine. Such teeth are susceptible to caries, pulp infection or necrosis and periradicular lesion. The complex anatomy of this disease results in difficult treatment and a high rate of therapeutic failure. Therapeutic options, such as debriding and filling invagination, root canal treatment (RCT) and intentional replantation, vary according to the morphology and infection of the involved tooth. The present study reports five cases of DI with chronic apical periodontitis. The treatmen

Prosthetic treatment of oligodontia with a tooth-supported overdenture--a case report.

An unusual case of oligodontia is reported in a 20 year old Nigerian male. It is unusual because of the isolated occurrence of the oligodontia which is unassociated with a family history or any other clinical features suggestive of specific syndromes or severe systemic abnormality. The patient presented with conically or peg shaped anterior and malformed posterior teeth in both the maxilla and mandible. Full mouth periapical radiographs revealed stunted roots, widening of the periodontal space and an unerupted tooth in the right mandibular premolar region. Tooth-supported upper and lower remov

Extraoral root lengthening with titanium screws.

The aim of the present study was to experimentally evaluate marginal and periapical healing in extracted and subsequently replanted teeth following extraoral cementing of apically exposed titanium root screws in the root canal. In 5 out of 6 replanted teeth, the periodontal tissues had healed with a normal periodontal membrane or healed surface resorptions indicating that only minor trauma had been inflicted during the extraoral procedure. In 3 out of 6 teeth, total or partial osseointegration of the titanium screws was seen, while a connective tissue capsule had formed around the remaining 3

Opportunistic Assessment of Dental Pathologies in Cervical Computed Tomography Angiography: A Proof of Concept Study.

OBJECTIVE: Computed tomography angiography (CTA) of the head and neck routinely encompasses dentoalveolar structures within the field of view, yet dental findings are rarely reported. Opportunistic detection of dental pathologies could provide valuable information for patient management. This feasability study evaluated the diagnostic potential of CTA for identifying common dental pathologies compared with panoramic radiography (orthopantomogram, OPG). MATERIALS AND METHODS: Fifty-seven patients (39% female, mean age: 63 years), who underwent both OPG and head-and-neck CTA within 100 days wer

Intraoperative Identification of Vital Radicular Pulp in a Tooth Diagnosed with Pulp Necrosis: Four-Year Outcome of Tampon Pulpotomy.

Teeth diagnosed clinically with pulp necrosis and apical periodontitis may occasionally retain vital radicular pulp tissue despite negative sensibility test responses. Such diagnostic discrepancies highlight the limitations of conventional pulp testing and may create opportunities for biologically based treatment approaches. This case report describes the successful management of a mature permanent molar initially diagnosed as necrotic but found intraoperatively to contain vital radicular pulp tissue. A 36-year-old healthy patient presented with pain on chewing and tenderness to percussion in

Dental management of florid cemento-osseous dysplosia.

Cemento-osseous dysplasia encompasses several different clinical and radiographic presentations, including periapical, focal and florid cemento-osseous dysplasia (FCOD). FCOD is usually asymptomatic and discovered only fortuitously. No treatment is required unless the lesion is secondarily infected. Oral hygiene advice should be emphasized for patients with FCOD to prevent caries and periodontal diseases and to maintain natural teeth. Osseointegration of implants would likely not be successful in these patients because the bone is abnormal and not well-vascularized.

Microbiology of Root Canal Infections.

Inflammatory reaction in the periapical tissues is induced by microbial infection in the root canal system. The aim of root canal treatment is to preserve healthy periapical tissues or to provide healing of them in restorable teeth, which have sufficient periodontal support. The amount of microbial cells in the root canal system and their virulence as well as host responses influence on the degree of periapical inflammation and symptoms. Microbial biofilm formation is typically seen on root canal walls but some microbial species are able to invade the dentine tubules to varying depth. In prolo

Combination of platelet rich fibrin, hydroxyapatite and PRF membrane in the management of large inflammatory periapical lesion.

Periapical inflammatory lesion is the local response of bone around the apex of tooth that develops after the necrosis of the pulp tissue or extensive periodontal disease. The final outcome of the nature of wound healing after endodontic surgery can be repair or regeneration depending on the nature of the wound; the availability of progenitor cells; signaling molecules; and micro-environmental cues such as adhesion molecules, extracellular matrix, and associated non-collagenous protein molecules. The purpose of this case report is to add knowledge to the existing literature about the combined
