Biomechanical pathways of dentoalveolar fibrous joints in health and disease.

Spatial and temporal adaptations within periodontal tissues and their interfaces result from functional loads. Functional loads can be physiologic and/or pathologic in nature. The prolonged effect of these loads can alter the overall biomechanics of a dentoalveolar fibrous joint (dentoalveolar joint) by changing the form of the tooth root and its socket. This "sculpting" of the tooth root and alveolar bony socket is a consequence of several mechano-biological changes that occur within the periodontal complex of a load-bearing dentoalveolar joint. These include changes in biochemical expression

GSK3 inhibitor-loaded osteotropic Pluronic hydrogel effectively mitigates periodontal tissue damage associated with experimental periodontitis.

Periodontitis is a chronic inflammatory disease caused by complex interactions between the host immune system and pathogens that affect the integrity of periodontium. To prevent disease progression and thus preserve alveolar bone structure, simultaneous anti-inflammatory and osteogenic intervention are essential. Hence, a glycogen synthase kinase 3 beta inhibitor (BIO) was selected as a potent inflammation modulator and osteogenic agent to achieve this treatment objective. BIO's lack of osteotropicity, poor water solubility, and potential long-term systemic side effects, however, have hampered…

Orthodontic Management of a Migrated Maxillary Central Incisor With a Secondary Occlusal Trauma.

INTRODUCTION: Normal or excessive occlusal forces exerted on teeth with a reduced periodontal support might result in a secondary occlusal trauma. This type of injury is diagnosed based on histological changes in the periodontium. Multiple clinical and radiographic indicators are, therefore, required as surrogates to assist the presumptive diagnosis of a (secondary) occlusal trauma. CASE PRESENTATION: In this case report, the diagnosis, management, and the 1-year follow-up of a secondary occlusal trauma of a maxillary central incisor are described. The occlusal relationship was rehabilitated …

[Use of integral indices in the assessment of bucсal cytology in health and in the oral cavity pathology.].

Cytological analysis of buccal epithelium (BE) - the buccal cytogram allows to select about 20 parameters reflecting cytogenetic, inflammatory, proliferative and other disorders in epithelial cells used for biomonitoring and laboratory diagnostics. In order to simplify and increase the information content of this study, we conducted a comparative analysis of various integral indices when interpreting the buccal cytogram. 40 practically healthy volunteers, 35 patients with chronic periodontitis, 22 patients with chronic gingivitis and 20 patients with partial loss of teeth were examined. The ob

Occlusal trauma and excessive occlusal forces: Narrative review, case definitions, and diagnostic considerations.

OBJECTIVES: This narrative review determines the effects of occlusal trauma and excessive occlusal forces on the periodontium, including the initiation and progression of periodontitis, abfraction, and gingival recession. Case definitions, diagnostic considerations, and the effects of occlusal therapy are also reviewed and discussed. IMPORTANCE: The role of occlusal trauma in the initiation and progression of periodontitis remains a controversial subject in periodontology. Because occlusal trauma can only be confirmed histologically, its clinical diagnosis depends on clinical and radiographic…

Does common prescription medication affect the rate of orthodontic tooth movement? A systematic review.

BACKGROUND: As the taking of any medication may theoretically affect the complex pathways responsible for periodontal tissue homeostasis and the events leading to orthodontic tooth movement, it is considered important for the orthodontist to be able to identify prospective patients' history and patterns of pharmaceutical consumption. OBJECTIVE: To systematically investigate and appraise the quality of the available evidence regarding the effect of commonly prescribed medications on the rate of orthodontic tooth movement. SEARCH METHODS: Search without restrictions in eight databases and hand

The saddle connective tissue graft: a periodontal plastic surgery technique to obtain soft tissue coronal gain on immediate implants. A case report.

Based on recent studies regarding the advantages of flapless immediate implants on the maintenance of the soft tissue architecture (especially at papillae level) in those situations where it is necessary to extract an anterior tooth, this case report describes a clinical procedure designed to replace a hopeless central incisor (2.1) showing root resorption adjacent to an implant-supported crown (1.1), whose gingival margin is 2 mm coronal regarding the hopeless tooth to be replaced. After the extraction of the hopeless tooth (2.1), a flapless immediate implant was placed. The implant-bone gap

Autologous dental pulp stem cells in periodontal regeneration: a case report.

Anatomical characteristics, such as shallow vestibular depth, high frenum attachments, and thin labial bone tend to make treatment of mandibular gingival recession defects particularly challenging. Even though a number of recent systematic reviews have identified the connective tissue graft (CTG) in combination with coronally advanced flaps (CAF) as the most consistently effective treatment procedure for Miller Class I and II defects, they did not separate maxillary vs mandibular outcomes, reporting instead aggregate results. In this prospective consecutive case series, recombinant human plate

Periodontal tissue engineering strategies based on nonoral stem cells.

Periodontal disease is an inflammatory disease which constitutes an important health problem in humans due to its enormous prevalence and life threatening implications on systemic health. Routine standard periodontal treatments include gingival flaps, root planning, application of growth/differentiation factors or filler materials and guided tissue regeneration. However, these treatments have come short on achieving regeneration ad integrum of the periodontium, mainly due to the presence of tissues from different embryonic origins and their complex interactions along the regenerative process. …

Long-term clinical results on the use of platelet concentrate in the treatment of intrabony periodontal defects.

OBJECTIVE: The purpose of this clinical investigation was to evaluate long-term results obtained with the combination of platelet pellet (PP) plus bioabsorbable barrier membrane (BM) and to compare this outcome with the results obtained using bioactive glass (BG) graft material with a BM. MATERIALS AND METHODS: Using a split mouth design, 11 chronic periodontitis patients (power ≥ at least 80%) were randomly assigned to treatment with a combination of PP/GTR or BG/GTR in contra-lateral dentition areas. Clinical attachment level (CAL) as the primary outcome variable, calculated as the sum of p

The usefulness of an autologous blood clot combined with gelatin for regeneration of periodontal tissue.

Several growth factors have been used in tissue regeneration therapy. Here we describe the use of an autologous blood clot combined with gelatin for teeth affected by severe periodontitis and vertical root fracture treated using three oral surgical procedures: periodontal flap surgery, intentional tooth replantation (IR), and tooth autotransplantation. Treatment with a blood clot improved the condition of the periodontal tissue, including reduction of pocket depth. Radiographical images demonstrated no evidence of ankylosis, and revealed the presence of alveolar bone regeneration. Our successf

Serum levels of monocyte chemoattractant protein-1 in periodontal health and disease.

OBJECTIVES: Initial research has reported high levels of monocyte chemoattractant protein-1 (MCP-1) in gingival crevicular fluid (GCF) of periodontitis patients. Present study has been carried out to assess its concentration in serum and to find its association with periodontal health and disease. DESIGN: 60 subjects were divided into three groups (n=20) based on gingival index (GI), probing pocket depth (PPD) and clinical attachment loss (CAL): healthy (group I), gingivitis (group II) and chronic periodontitis (group III). A fourth group (group IV) consisted of 20 subjects from group III, 6-

[The morphological changes in the periodontal tissues of outbred white rats during ontogeny].

Morphologic studies, carried out by three methods, by van Gieson's, Schmorl's methods, and with the use of 0.1% solution of safranin T, have revealed heretofore unknown structures lying beyond the tooth rudiment of white noninbred rats and disappearing in the course of ontogenesis. The mechanism of the formation of autonomic circulation of the dental follicle, unrelated to the animal body, has been shown. Dental follicular tissues were found grown together with the gingival epithelium and they ruptured during tooth eruption.

Plasminogen activator in human periodontal health and disease.

Plasminogen activators, proteases associated with the fibrinolytic system, also play a major part in extravascular processes such as tissue remodelling, cell migration and activation of prohormones, growth factors and other proteases. It is likely that plasminogen activators participate in the pathophysiology of periodontal disease. Plasminogen activator has been identified in human gingival crevicular fluid in a concentration 100-fold greater than in plasma. The local activity of plasminogen activator in gingival tissues was examined and changes detected in its distribution in relation to the

Properties of periodontal mechanoreceptors supplying the cat's lower canine at short and long periods after reinnervation.

1. The properties of periodontal mechanoreceptors innervating the lower canine teeth of cats have been determined 6 weeks and 1 year after sectioning the inferior alveolar nerve. Recordings were made from single fibres dissected from the nerve central to the injury site, whilst forces were applied at right angles to the long axis of the tooth. 2. The range of directions over which each mechanosensitive unit responded (are of sensitivity) was established. Forces were then applied in the direction of maximum sensitivity and the maximum discharge frequency, dynamic index, adaptation rate, and the

The effect of the supra-alveolar soft tissue on the relapse of orthodontic treatment.

Orthodontic closure in cases of aplasia as also in patients with spaces of other genesis is often associated with problems of relapse. In this study ten patients were treated orthodontically in order to close unacceptable interdental spaces. Five patients had paired aplasia. The remaining five patients had interdental spaces of different genesis. Supra-crestal connective tissue fibres were surgically removed in all patients but only on one side of the jaw in the children with paired aplasia. The non-operated side of these children served as a control side. The stability of the orthodontic trea
