Shared molecular pathways in pediatric genetic epilepsies: Insights from a single-center cohort of 80 patients.

OBJECTIVE: Genetic epilepsies in childhood are highly heterogeneous, and approaches to identify shared biological mechanisms across distinct genetic etiologies remain limited. We aimed to investigate whether genes associated with pediatric genetic epilepsies in our heterogeneous cohort converge on common functional pathways. METHODS: We conducted a single-center retrospective study involving 80 children who met the International League Against Epilepsy (ILAE) criteria for epilepsy with a confirmed monogenetic etiology, with or without other neurological conditions. These children were part of

'It's everybody's business': a multi-method study to co-develop and evaluate a web-based interprofessional Sexual Function Intimacy Concerns after Cancer educational programme (SexFICC) to upskill cancer healthcare providers.

INTRODUCTION: With improved survival rates, there is a growing population of cancer survivors who experience sexual health concerns following cancer treatment. These are widely recognised sequelae of cancer and cancer treatment, negatively impacting intimate relationships and quality of life. At least 40% (up to 100%) of people affected by cancer report concerns in this area. However, despite being well documented long-term challenges, they remain largely unaddressed by healthcare teams leading to further deterioration in sexual health. Healthcare providers identify this as an important qualit

Dissemination in diagnostic accuracy studies is not associated with methodological quality: a systematic review and meta-epidemiological analysis.

OBJECTIVES: To evaluate associations between methodological characteristics and dissemination measures in diagnostic accuracy studies. The COVID-19 pandemic generated a large body of studies addressing a single diagnostic question, allowing assessment of how study characteristics relate to dissemination. STUDY DESIGN AND SETTING: Using a preregistered systematic review (PROSPERO CRD42023343656), we identified studies evaluating the diagnostic performance of SARS-CoV-2 serological tests through Medline, Embase, and the iSearch Portfolio. Risk of bias and applicability were assessed using QUADA

Impact of pre-surgical anxiety and fear on patient satisfaction following periodontal and implant surgical procedures.

BACKGROUND: Pre-surgical anxiety (PSA) and pre-surgical fear (PSF) may hinder patient compliance, while patient satisfaction (PS) is vital for adherence to treatment recommendations. This prospective study investigated the impact of PSA and PSF, and the influence of patient demographics and postoperative pain, on PS after periodontal and implant surgeries. METHODS: Seventy-four patients, ≥ 18 years, who underwent periodontal and implant surgeries were recruited. PSA and PSF were measured using the Modified Dental Anxiety Scale (MDAS) and Dental Fear Survey (DFS). PS was evaluated 2 weeks post

Physiological-behavioural discordance in paediatric perioperative anxiety assessment: a scoping review protocol.

INTRODUCTION: Preoperative anxiety in children is a common problem in paediatric surgical care. In current practice, anxiety is assessed mainly by behavioural rating scales, of which the modified Yale Preoperative Anxiety Scale (mYPAS) is the most widely used. However, behavioural observation may not capture children whose outward behaviour stays calm while their autonomic stress response is still high. This mismatch between visible behaviour and underlying physiology can lead to under-recognition of anxiety and may explain the modest correlations reported between behavioural scores and physio

Effectiveness and implementation of Self-Help Plus (SH+) and Doing What Matters in Times of Stress (DWMS): protocol of a systematic review and meta-analysis.

INTRODUCTION: Global increases in armed conflict, forced displacement, pandemics and economic instability have contributed to rising levels of psychological distress worldwide, placing relevant segments of the population at increased risk of developing mental health conditions. This burden is particularly pronounced in humanitarian and low-resource settings where access to specialist mental health services is limited. Scalable, low-intensity, evidence-based psychological interventions are therefore urgently needed. In response, the WHO has developed transdiagnostic programmes, including Self-H

Scoping Review on the Evidence Assessment of Interventions: A Protocol.

BACKGROUND AND OBJECTIVE: The assessment of evidence in health research for interventions has traditionally relied on hierarchical models that prioritize randomized controlled trials and meta-analyses. Over time, alternative models have evolved, reshaping and rebalancing sources of evidence and giving more weight to external validity. This scoping review will systematically identify and map existing models of evidence assessment, including hierarchical, circular, matrix-based, and integrative approaches. It will examine how these models conceptualize and balance internal and external validity.

Perampanel as add-on in high-grade glioma-related epilepsy: Seizure control and QoL in a prospective, multicenter, real-world 6-month follow-up study.

OBJECTIVE: High-grade astrocytomas, including glioblastomas, are aggressive brain tumors with poor prognosis and a 5-year survival below 7%. Seizures affect up to 75% of glioma patients, especially in low-grade tumors but also in high-grade cases. Elevated extracellular glutamate in peritumoral tissue-due to tumor release, impaired uptake, and metabolic changes like D-2-hydroxyglutarate in IDH1-mutated tumors-contributes to neuronal hyperexcitability and tumor progression via AMPA and NMDA receptors' activation. Perampanel (PER), a selective noncompetitive AMPA receptor antagonist, targets thi

Impact of cenobamate on cortical responses to transcranial magnetic stimulation in people with drug-resistant focal epilepsy.

OBJECTIVE: Cenobamate (CNB) is an effective antiseizure medication, though its mechanisms of efficacy remain incompletely understood. We assessed changes in cortical responses to transcranial magnetic stimulation (TMS) following CNB treatment. METHODS: We recruited people with drug-resistant focal epilepsy scheduled to start add-on CNB in a tertiary epilepsy center. We performed TMS coupled with electromyography (EMG) and electroencephalography (EEG) at baseline, at 100 mg/day and at the maximum tolerated or minimum effective dose. Participants were categorized as responders (≥50% seizure red

Salience network hub dysfunction is associated with cognitive impairment in self-limited epilepsy with centrotemporal spikes: An OPM-MEG study.

OBJECTIVE: Self-limited epilepsy with centrotemporal spikes (SeLECTS), although characterized by well-controlled seizures, is frequently associated with persistent cognitive deficits in affected children. This study aimed to investigate whether hub dysfunction of the salience network (SN) is associated with cognitive impairment in SeLECTS, using optically pumped magnetometer- magnetoencephalography (OPM-MEG) within the framework of the triple network model (TNM), which encompasses the salience, default mode, and central executive networks (CENs). METHODS: This cross-sectional case-control stu

Aging and periodontitis increase brain dissemination of oral bacteria.

BACKGROUND: The microbiome is a dynamic system that changes throughout life. Studies have revealed the relationship between periodontal disease and the oral microbiota; however, the impact of periodontal disease on the expression of senescence markers and on the inflammaging of the oral and systemic microbiome remains unclear. We hypothesized that aging increases the periodontitis-induced changes in the oral and systemic microbiome and is accompanied by an altered inflammatory response. METHODS: Experimental periodontitis was induced in 18-month-old (old) and 8-month-old (young) C57BL/6 mice

Frequency, cost, and time to re-treatment after active periodontal therapy.

BACKGROUND: Supportive periodontal therapy (SPT) reduces the probability of infection reoccurrence further disease progression. Despite that, several patient/tooth related variables have been linked to tooth loss during SPT. This longitudinal study examines the correlation between periodontal staging, grading, initial therapy, and frequency, cost, and time-to-re-treatment following active periodontal therapy (APT). METHODS: Patients who received scaling and root planing (SRP) or surgery (SUR) were included. Demographics, annual SPT visits, and medical history were collected. Radiographic bone

Surveying coating strategies for peri-implantitis management: Clinical implications and classificatory approaches.

BACKGROUND: Peri-implantitis, an inflammatory condition occurring in the supportive tissues, is triggered by a dysbiotic biofilm that grows on implant and/or abutment surfaces. Consequently, the entire surface becomes a notorious culprit, fostering bacterial adhesion that might lead to progressive loss of supporting bone. To combat peri-implantitis, research groups worldwide have diligently pursued the development of new antimicrobial coatings. However, for the successful development of coating materials, it is crucial to clarify their intended function. In this review, we propose a clear clas
