Little is known about mechanisms of systemic hypotension frequently reported during plasma exchange (PE). Type of substitution fluids may interfere with hemodynamic tolerance. In a prospective study, right heart catheterization was performed during 18 PE by filtration with isovolumic substitution. Blood volume was measured with 51Cr tagged erythrocytes and plasma volume (PV) calculated from hematocrit. Substitution fluids were either albumin (A; n = 9) or A + gelatin (A + G; n = 9). Hydroxy-ethylated starch (HAES-Fresenius) has been evaluated clinically at the Department of Anesthesiology and Intensive Care, Medical Academy in Warsaw. This plasma substitute is used in several countries as plasma substitute similar to dextran but of better properties. Results of our studies have shown, that HAES is worth its opinion. It is quite good plasma substitute of favourable effect on tissue perfusion. Haemodilution includes intravenous administration of plasma-replacing solutions and/or blood letting for changing the rheological properties of blood, and for improvement of microcirculation. To achieve normovolemic hemodilution two methods have established themselves: the acute preoperative hemodilution and the therapy with different blood constituents. The ever increasing consumption of blood and blood products shows the absolute necessity of using all the available constituents, especially the erythrocytes. A proved and standardized transfusion program is shown, which --allows blood substitution with specific hemodilution without additional expenses of time and organization or technical difficulties, --shows means of substituting 80% of the blood consumption with packed red cell
