Advances in plastic surgery.

Recent progress in plastic surgery has been rapid and many new techniques have been developed. Reconstructive procedures have been advanced by a better understanding of the anatomy of the blood supply to skin and muscle, with the subsequent development of the use of axial flaps, musculocutaneous flaps and neurosensory flaps. Burn treatment has advanced greatly, making it possible to successfully treat larger and more complicated burns. The development of microsurgery has made possible free-flap transfer and replantation of amputated parts. Advances in surgical procedures on the hands include a

Reconstruction Technique for Defects of the Cutaneous and Mucosal Lip: V-to-flying-Y Closure.

Repair of a surgical defect following Mohs micrographic surgery can be particularly challenging when it involves free margins of the face. Lip reconstruction falls under this category, requiring careful planning of the repair to avoid not only aesthetic but also functional complications of the oral aperture. Our technique, termed V-to-flying-Y closure, allows for an aesthetically pleasing and functional outcome for repairs of surgical defects that involve both the cutaneous and mucosal lip.

Z-plasty for correction of standing cutaneous deformity.

Cutaneous head and neck reconstruction following Mohs micrographic surgery frequently presents the surgical dilemma of dog-ear formation during wound closure. Z-plasty corrects a dog-ear deformity without skin excision by recruiting tissue from the axis of the standing cone and redistributing it along another. We describe dog-ear correction using the Z-plasty technique.

[Expert consensus on navigation-guided unilateral orbital fractures and orbital floor reconstruction techniques].

Orbital fractures and orbital floor reconstruction surgery is characterized by limited surgical exposure and high risks. Before the advent of digital technology, the design and implementation of the surgical scheme of orbital floor reconstruction surgery mainly depended on the intuitive understanding of imaging and the clinical experience of the surgeons, with strong unpredictability and experience dependence.Surgical navigation systems with real-time positioning and imaging functions, when used in orbital reconstruction surgery, can detect the real-time intraoperative position, avoid risks, a

Orbital Fractures.

Anatomic, rather than volumetric, reconstruction leads to improved outcomes in orbital reconstruction. Endoscopic visualization improves lighting and magnification of the surgical site and allows the entire operative team to understand and participate in the procedure. Mirror-image overlay display with navigation-guided surgery allows in situ fine adjustment of the implant contours to match the contralateral uninjured orbit. Precise exophthalmometry is important before, during, and after surgery to provide optimal surgical results.

Head and Neck Reconstruction: Does Surgical Specialty Affect Complication Rates?

BACKGROUND: Reconstruction of head and neck defects resulting from resection of head and neck masses is performed by both plastic surgeons and otolaryngologists. The American College of Surgeons National Surgical Quality Improvement (NSQIP) database allows one to directly compare the outcomes for a given procedure based upon specialty. The purpose of this study is to compare outcomes and resource utilization of microvascular head and neck reconstruction between plastic surgery and otolaryngology. METHODS: Institutional review board approval was obtained and NSQIP was queried from 2005 to 2015

Development of reconstructive surgery services in the Gaza Strip, Palestine.

Plastic surgery involves the restoration, alteration or reconstruction of the human body. It includes reconstructive and aesthetic surgery. This paper discusses the authors' experience introducing new reconstructive, microsurgical and aesthetic surgeries in the Gaza Strip. We analyzed 192 cases of reconstruction carried out in the Al-Alamy Burns Center in Al-Shifa Hospital from August 2017 to July 2018 and at the European Gaza Hospital during a one-month delegation, as well as 38 aesthetic surgeries performed in the author's private clinic. We highlight challenges hindering the development of

Hereditary coagulopathies: practical diagnosis and management for the plastic surgeon.

BACKGROUND: Venous thromboembolism is a devastating complication representing one of the major causes of postoperative death in plastic surgery. Within the scope of plastic surgery, body-contouring procedures are often considered to carry a higher risk of venous thromboembolism. Hereditary thrombophilias comprise a group of conditions defined by a genetic predisposition to thrombosis development. Collectively, hereditary thrombophilias are present in at least 15 percent of Western populations and underlie approximately half of thromboembolic events. Although the topic of venous thromboembolism

The Use of Optical Coherence Tomography as an Intraoperative Adjunct of Oculoplastic Surgery.

The advent of integrated intraoperative optical coherence tomography (i2OCT) has opened the door for safer and more complex surgeries in the retina and cornea. However, to limit its use to just two subspecialties within ophthalmology is an opportunity lost for many other subspecialties. Here, we describe the use of i2OCT in oculoplastic surgery. It can be used in cases of severe symblepharon where the cornea is heavily involved, so that the surgeon is aware of the depth of the pannus when trying to remove it. It can also be used when the corneal planes are difficult to identify. In oculoplasti

Comparing Plastic Surgeon Versus Orthopedic Surgeon Outcomes Following Distal Upper Extremity Amputations: A Study of the National Surgical Quality Improvement Program (NSQIP) Database.

BACKGROUND: Both plastic and orthopedic surgeons manage care for urgent/emergent hand conditions. It is unclear if surgeon specialty affects patient outcomes of these cases. The purpose of this study was to evaluate differences in 30-day perioperative outcomes between plastic and orthopedic surgeons following distal upper extremity amputations. METHODS: Patients who underwent distal upper extremity amputations between 2005 and 2016 were identified within the National Surgical Quality Improvement Program (NSQIP) database using Current Procedural Terminology (CPT) codes. Optimizing neurorrhaphy to improve sensation in phalloplasty: a systematic review.

Phalloplasty aims to construct a functional and aesthetic phallus in transgender and gender diverse and cisgender patients with absence or loss of penile tissue. Sensation is a critical outcome in postoperative quality of life, impacting placement of erectile devices, sexual function, and overall well-being. Despite this, there is currently a lack of consensus on strategies to optimize sensation in phalloplasty. A comprehensive search of Medline and Embase databases was conducted with keywords related to phallic sensation in penile reconstruction. 237 abstracts were identified, and 28 articles

The L brachioplasty: an innovative approach to correct excess tissue of the upper arm, axilla, and lateral chest.

BACKGROUND: Brachioplasty is aesthetic reshaping of the upper arm after removal of excess medial skin and fat. Massive weight loss patients evolve a severe arm deformity that extends through the axilla and onto the chest. Prevalent operations are incomplete and leave conspicuous scars along the bicipital groove that end as Ts or Zs in the axilla. The L brachioplasty starts with a long ellipse centered over the lower half of the inner arm that sweeps up to the deltopectoral groove. A shorter ellipse is connected at right angles through the axilla onto the chest. The V flap formed between the el

Flaps and Grafts in Robotic Reconstructive Surgery.

PURPOSE OF REVIEW: The robotic approach is increasingly popular in reconstructive urology. Reconstructive surgeons have commonly used flaps and grafts for obliterating dead space including tissue interposition or as an alternative to mesh in addressing lower urinary tract dysfunction. Advantages of the robotic approach are less incisional pain, excellent visualization in the deep pelvis, and improved surgeon ergonomics. In this literature review, we describe flaps and grafts used in lower urinary tract robotic reconstructive urology, serving as an almanac for these techniques. RECENT FINDINGS

Current Limitations of Surgical Robotics in Reconstructive Plastic Microsurgery.

Surgical robots have the potential to provide surgeons with increased capabilities, such as removing physiologic tremor, scaling motion and increasing manual dexterity. Several surgical specialties have subsequently integrated robotic surgery into common clinical practice. Plastic and reconstructive microsurgical procedures have not yet  benefitted significantly from technical developments observed over the last two decades. Several studies have successfully demonstrated the feasibility of utilising surgical robots in plastic surgery procedures, yet limited work has been done to identify and a…
