Treatment of advanced thymoma and thymic carcinoma.

Although thymic epithelial tumors are rare, they are relatively common among neoplasms of the anterior superior mediastinum. They usually exhibit indolent behavior, but do have the capacity to invade surrounding structures and metastasize to distant sites. Thymic carcinomas are rare, but are highly aggressive tumors that are associated with a poor prognosis. The mainstay of therapy is complete surgical resection. Locally advanced thymoma and thymic carcinoma require a multimodality treatment approach with a combination of surgery, chemotherapy, and radiation therapy to decrease the chances of

Two cases of secondary soft tissue sarcomas after radiotherapy and radiochemotherapy.

BACKGROUND: The development of secondary soft tissue sarcomas after chemo-radiotherapy is a rare and little known event, but its frequency is increasing. PATIENTS AND METHODS: We report two cases of secondary soft tissue sarcomas. The first is the case of a 51-year-old woman treated for Hodgkin's disease with chemotherapy and radiotherapy 15 years before she developed a high-grade malignant pleural sarcoma. The patient had no history of asbestos exposure. The second is the case of a 64-year-old woman with a giant cell malignant histiocytoma secondary to colorectal cancer treated with surgery

Solitary fibrous tumor occurring in the colon as submucosal mesenchymal lesion: report of two cases and review of the literature.

Solitary fibrous tumor (SFT) is a rare mesenchymal neoplasm most often arising from the pleura and rarely in extra-pleural locations, including the gastrointestinal tract. We describe two cases of a SFT presenting as submucosal colonic lesion and review the literature on this lesion. One submucosal lesion was localized in the cecum and was 10 mm in size. The second lesion presented as a 17 mm submucosal rectal lesion. Both lesions presented as well-circumscribed submucosal lesions arranged in short fascicles, blending with abundant collagenous stroma. In both cases, the spindle cells were posi

Askin's Tumour: A Report of a Rare Case.

Primitive neuroectodermal tumours (PNETs) are rare, malignant tumours arising from primitive nerve cells. PNET of the chest wall is rare and is observed in children and young adults. Askin defined Askin's tumour as a PNET of the thoracopulmonary area. It develops from the soft tissues of the chest wall, particularly in the paravertebral region. Here, we report a case of Askin's tumour, a rare neoplasm occurring in the thoracopulmonary region in a 13-year-old girl. She came with complaints of fever, cold, cough with mucoid expectoration, breathlessness for 15 days, and generalized weakness for

Malignant Solitary Fibrous Tumor of the Inguinal Region: A Case Report.

A solitary fibrous tumor (SFT) is a rare spindle cell neoplasm in adults, usually found in the pleural and thoracic cavities. We report an interesting case of a malignant solitary fibrous tumor in a 64-year-old male who presented with a history of swelling in his right inguinal region that gradually increased in size during the past three years. Computed tomography (CT) and magnetic resonance imaging (MRI) scans of the abdomen and pelvis showed a rounded solid mass originating from the right inguinal canal suggestive of sarcoma. Elective excision of the mass was done under general anesthesia w

Combined use of irinotecan and p53 activator enhances growth inhibition of mesothelioma cells.

Malignant mesothelioma (MM) is an aggressive malignant neoplasm which rapidly invades pleural tissues and has a poor prognosis. Here, we explore enhancement of the effect of irinotecan [camptothecin-11 (CPT-11)] by the p53-dependent induction of carboxylesterase 2 (CES2), a CPT-11-activating enzyme, in MM. The level of CES2 mRNA was greatly increased on treatment with nutlin-3a. A combination of CPT-11 and nutlin-3a inhibited the growth of MM cells more effectively than either drug alone. Knocking down CES2 in MM cells reduced the effect of the drug combination, and its forced expression in ME

Thymic carcinoma - a rare case report.

INTRODUCTION: Thymic carcinoma is a rare, invasive mediastinal neoplasm with a tendency to metastasize. They constitute a heterogeneous group of tumors that present differently in terms of both behavior and prognosis. CASE REPORT: We present a case of thymic carcinoma in a 55-year-old male patient known to suffer from Myasthenia gravis, whose chief complaints were fatigability, ptosis and coughing. All electromyography studies were done along with excision biopsy to reach the definitive diagnosis. RESULTS AND CONCLUSION: The histopathological diagnosis of Thymic carcinoma-Lymphoepithelial va

Concurrent thoracic mesothelioma and thyroid C-cell adenoma with amyloid deposition in an aged horse.

A 21-year-old American Saddlebred mare died with a history of weight loss and breathing difficulties of 1 month duration. Post-mortem examination revealed a copious pleural effusion with multifocal to coalescing numerous white to grey nodular masses on the serosal surface of the pericardium, lungs and thoracic cavity. In addition, the left thyroid gland was markedly enlarged. A thoracic mesothelioma and C-cell adenoma with amyloid deposits of the left thyroid gland were diagnosed by histopathology and confirmed by immunohistochemistry employing antibodies against cytokeratin (CK), vimentin and

Giant thymoma successfully resected via hemiclamshell thoracotomy: a case report.

Thymoma is an epithelial neoplasm of the thymus. It commonly lies in the anterior mediastinum and represents 20-30% of mediastinal tumours in adults. In this report we present a case of giant thymoma locating in the anterior-inferior mediastinum. A 46-year-old male came to our institute with slight pectoralgia and dyspnea. Chest CT shows a giant tumor measuring 19 cm × 16 cm × 15 cm in the left thoracic cavity. After careful examination, we performed surgery. At surgery, we found the tumor was adherent to left upper lobe of the lung, mediastinal pleura, and parietal pleural. The tumor was comp

Successful chemotherapy with Carboplatin and s-1 for thymic carcinoma: a case report.

Thymic carcinoma is a rare but aggressive neoplasm. Although there is no clearly optimal first- or second-line chemotherapy regimen for thymic carcinoma, platinum-based chemotherapy has repeatedly been shown to be of benefit to patients with advanced thymic carcinoma. Some case reports have described S-1 as a novel agent with good activity against advanced thymic carcinoma. A 74-year-old female was diagnosed with thymic carcinoma complicated by pleural dissemination and pericardial effusion of carcinomatosa. She was treated with carboplatin on day 1 plus S-1 on days 1-14 in cycles repeated eve

A rare solitary fibrous tumour of kidney.

A solitary fibrous tumour is an unusual spindle cell neoplasm. It frequently arises from the serosal surface of pleural cavity but has recently been described in diverse extrapleural sites. Urogenital localization is rare and only 36 cases of solitary fibrous tumours of the kidney have been described on published report. We report a case of a large solitary fibrous tumour clinically and radiologically thought to be renal cell carcinoma arising in the kidney of a 30 year old female. The radical nephrectomy was performed. The tumour was a well- circumscribed, solid mass attached to the renal pel

Extramedullary plasmacytoma presenting as a mediastinal mass.

Extramedullary plasmacytoma (EMP) is a plasma cell neoplasm without bone marrow involvement or other systemic characteristics of multiple myeloma. Few large-scale clinical studies have been conducted because of the rarity of EMP, especially when it arises from the mediastinum. Herein we report a rare case of solitary mediastinal plasmacytoma with reactive pleural effusion. A 58-year-old female presented with grade 4 dyspnea and dysphagia, with a mediastinal mass observed with on PET. CT-guided biopsy results were suggestive of IgG kappa-type EMP arising from the anterior mediastinum. The patie

[A case of enlarging rounded atelectasis difficult to distinguish from a pulmonary neoplasm].

A 55-year-old man with occupational exposure to asbestos presented with a chest roentgenographic abnormality that was found during a physical check-up. Computed tomography disclosed pleural thickening and a pulmonary mass in the left lower lobe. A fine-needle biopsy was done, but the presence of a malignant lesion was not confirmed. The patient was asymptomatic. He was observed closely, and 10 months later computed tomography showed that the mass had enlarged. A fine-needle biopsy was done again, but no evidence of malignancy was found. We suspected that it was rounded atelectasis, but could n

[Nonmalignant lymphoid disorders of the chest: a report of 4 cases with a review of literature].

The lung parenchyma, the mediastinal or hilar lymph nodes were involved by several nonmalignant lymphoid disorders which are not specific infections or malignant neoplasm. The etiology of these disorders remains unknown. They may be the outcome of viral-autoimmune causes or some chronic stimulation of the reticuloendothelial system by unknown antigens. Pulmonary pseudolymphoma is a localized pulmonary disease, generally not accompanied by lymph node enlargement or pleural effusion. However, lymph nodes of the mediastinum and hilum are involved in case of giant lymph node hyperplasia and angioi

Giant right atrium after mitral valve replacement: plain film findings in 15 patients.

We reviewed the radiographs of 166 patients who had undergone mitral valve replacement and found 15 (9%) with giant right atrium. It was confirmed in 13 cases by angiography and/or echocardiography. It was proved at autopsy in one case and confirmed by a Swan-Ganz catheter delineating the lateral right atrial wall in another case. Tricuspid insufficiency of varying severity was present in all patients. Progressive bulging of the right heart border on frontal radiographs and an anterior masslike density on lateral radiographs were the important findings. The bulging right heart border was seen

Outcomes of radiologically diagnosed solitary fibrous tumours of the pleura.

INTRODUCTION: Solitary fibrous tumours of the pleura (SFTP) are rare neoplasms for which surgical resection is considered the gold standard treatment. Anecdotally, SFTPs are sometimes conservatively managed, but there are little data on this in published literature. In this study, we aimed to collect data on radiological features in SFTP which might be associated with a benign clinical course to assist in risk stratification, and therefore selection for a more conservative management pathway. METHODS: A service evaluation of outcomes of radiologically diagnosed SFTP was completed in 12 UK cen

Transthoracic Ultrasound in Infectious Organizing Pneumonia: A Useful Guide for Percutaneous Needle Biopsy.

In patients presenting with classical features of CAP (i.e., new peripheral pulmonary consolidations and symptoms including fever, cough, and dyspnea), a clinical response to the appropriate therapy occurs in few days. When clinical improvement has not occurred and chest imaging findings are unchanged or worse, a more aggressive approach is needed in order to exclude other non-infective lesions (including neoplasms). International guidelines do not currently recommend the use of transthoracic ultrasound (TUS) as an alternative to chest X-ray (CXR) or chest computed tomography (CT) scan for the

Mesothelioma mortality surveillance and asbestos exposure tracking in Italy.

INTRODUCTION: Spatial distribution of mortality from pleural mesothelioma (which in the ICD-10 Revision has a specific code: C45.0) in Italy for the period 2003-2009 is described. Previous mortality studies at national level employed the topographic code "Malignant neoplasms of pleura", because of unavailability of a specific code in ICD-9 Revision for pleural mesothelioma. METHODS: Standardized mortality ratios were computed for all municipalities, using each regional population as reference; for municipalities in Regions with rate higher than the national rate, the latter has been used as r

The role of autologous bone marrow transplantation in primary effusion lymphoma: a case report and literature review.

Primary effusion lymphoma (PEL) is an aggressive and rare type of diffuse large B-cell lymphoma (DLBL) that commonly presents itself as pleural, pericardial or peritoneal effusion without lymph node or extranodal involvement in immunosuppressed patients, such as HIV-positive or transplanted receptors. On rare occasions, it may be found in solid sites without effusion, in an immunophenotypically and morphologically similar neoplasm well-known as extracavitary PEL (EC-PEL). Both PEL and EC-PEL are associated with extremely poor prognosis. Due to the rarity of these entities, ther e are no gold s
