Effect of pneumoperitoneum and Trendelenburg position on internal carotid artery blood flow measured by ultrasound during robotic prostatectomy.

INTRODUCTION: Robotic prostatectomy requires pneumoperitoneum and a steep Trendelenburg position; however, this condition may compromise cerebral blood flow. Here, we evaluated the effect of pneumoperitoneum and the steep Trendelenburg position on internal carotid artery (ICA) blood flow measured by Doppler ultrasound during robotic prostatectomy. METHODS: Patients who underwent robotic prostatectomy were prospectively recruited. The ICA blood flow was measured at the following five time-points: with the patient awake and in the supine position (Ta), 10 min after anaesthetic induction in the …

Self-Inflicted Abdominal Air Insufflation Leading to Diffuse Subcutaneous Emphysema, Pneumoperitoneum, Pneumomediastinum, Pneumopericardium, and Pneumothorax: A Case Report.

Subcutaneous emphysema is defined by air becoming confined in the soft tissues beneath the skin and it may occur following various surgical … tration of his umbilicus with a manual air compressor leading to widespread subcutaneous emphysema, pneumoperitoneum, pneumomediastinum, pneumopericardium, and pneumothorax. The pathway of possible air movement between body cavities has been theorized, but excluding case reports and anecdotal evidence, treatment of severe subcutaneous emphysema is less clear. This …

Warm and Humidified Versus Cold and Dry CO2 Pneumoperitoneum in Minimally Invasive Colon Resection: A Randomized Controlled Trial.

INTRODUCTION: Peritoneal insufflation with warm-humidified (WH) CO2 gas during minimally invasive surgical procedures is purported to prevent hypothermia and peritoneal desiccation and is associated with decreased postoperative IL-6 levels. This randomized study's purpose was to determine the clinical impact of WH versus cold-dry (CD) CO2 in minimally invasive colon resection (MICR), and to assess perioperative plasma levels of IL-6, TIMP-1, sVEGF-R1, and HSP-70 after MICR. METHODS: Operative and short-term clinical data plus perioperative blood samples were collected on MICR patients randomi

Failed pneumoperitoneum for laparoscopic surgery following autologous Deep Inferior Epigastric Perforator (DIEP) flap breast reconstruction: a case report.

BACKGROUND: Laparoscopic abdominal surgery may prove difficult in patients who have undergone previous abdominal procedures. No reports in the medical literature have presented an aborted laparoscopic procedure for failed pneumoperitoneum following autologous flap-based breast reconstruction. CASE PRESENTATION: A 55-year-old woman presented with recurrent invasive lobular carcinoma of the right breast as well as a history of ductal carcinoma in situ of the left breast. The patient desired to proceed with bilateral skin- and nipple-sparing mastectomies with right axillary lymph node biopsy, fo…

Combined low pressure pneumoperitoneum and intraperitoneal infusion of normal saline for reducing shoulder tip pain following laparoscopic cholecystectomy.

INTRODUCTION: Intraabdominal CO(2) gas after laparoscopic cholecystectomy causes postoperative shoulder-tip pain. Many methods of analgesia have been used to reduce this pain, including analgesic drugs, intraperitoneal local anesthetic, intraperitoneal saline, a gas drain, heated gas, low-pressure gas, and nitrous oxide pneumoperitoneum. The aim of this study was to evaluate the efficacy of combined low-pressure CO(2) pneumoperitoneum and intraperitoneal infusion of normal saline in reducing the incidence of postoperative shoulder-tip pain. METHODS: Altogether, 109 patients undergoing electiv…

Tension pneumoperitoneum combined with CO2 gas embolism during peroral endoscopic myotomy: a case report and review of literature.

BACKGROUND: Peroral endoscopic myotomy (POEM) is a safe, effective, and minimally invasive treatment for achalasia. While continuous carbon dioxide (CO2) insufflation during POEM may lead to various insufflation-related adverse events, the occurrence of gas embolism remains an exceedingly rare complication. This report describes a case of tension pneumoperitoneum complicated by carbon dioxide gas embolism during POEM. CASE: A 51-year-old man was diagnosed with achalasia and underwent POEM. Intraoperatively, the end-tidal carbon dioxide (PetCO2) continuously increased to 70 mmHg, accompanied b…

Hepatosplanchnic perfusion changes following isolated abdominal trauma and surgery: A scoping review of evidence dominated by pneumoperitoneum models.

The hepatosplanchnic vascular response to isolated abdominal organ injury remains poorly defined. The aim of this review was to synthesize t … arratively. Twenty-six studies were included: 14 human and 12 animal studies. Nineteen investigated pneumoperitoneum or elevated intra-abdominal pressure, four major abdominal surgery, and three other models. Perfusion reductions became more frequent with increasing pressure and were significantly more common at >15 than <10 mmHg (p = 0.031). Higher pressures also …

Negative surgical exploration for CT-proven pneumoperitoneum: a diagnostic pitfall in suspected gastrointestinal perforation-a six-case series.

BACKGROUND: Computed tomography (CT) plays a vital role in diagnosing suspected gastrointestinal perforation (GIP), particularly in the presence of pneumoperitoneum and intra-abdominal fluid. Nevertheless, these radiologic findings do not invariably correspond to bowel perforation, and negative surgical exploration may occur. We conducted this study to characterize the clinical features and outcomes of patients who underwent emergency surgery for suspected GIP but had no perforation. METHODS: We retrospectively reviewed consecutive non-trauma patients who underwent emergency surgical explorat…

Acute gastroesophageal intussusception in a patient with pneumomediastinum, pneumoperitoneum, and extensive subcutaneous emphysema resulting from iatrogenic colonic perforation.

Acute gastroesophageal intussusception is a rare condition in which the stomach invaginates into the adjacent esophagus. We report a case of acute gastroesophageal intussusception with massive pneumomediastinum, pneumoperitoneum, and subcutaneous emphysema, effectively treated with conservative management.

Isolated tension pneumoperitoneum following endobronchial ultrasound-guided transbronchial needle aspiration complicated by cardiac peri-arrest: A case report.

Transbronchial needle aspiration under endobronchial ultrasound guidance (EBUS-TBNA) is recommended for the diagnosis and staging of lung cancer. Major complications following EBUS-TBNA are uncommon. We report a case of isolated tension pneumoperitoneum following EBUS-TBNA under deep sedation in an 80-year-old male patient affected by a right hilar mass suspicious for lung cancer. At the end of the procedure, the patient suddenly manifested desaturation, severe bradycardia and hypotension, and abdominal distension. After resuscitation, in the suspect of intraperitoneal free air, needle decompr…

Accidental peritoneum-cutaneous fistula after insufflation of preoperative progressive pneumoperitoneum in a large incisional hernia with loss of domain.

Preoperative progressive pneumoperitoneum has represented an important advancement in achieving the reintroduction of large herniated volumes into the abdominal cavity. However, this technique is not free of complications. We present a case of a man in his 70s with an accidental peritoneal-cutaneous fistula, secondary to the excessive pressure of the pneumoperitoneum, during the preparation of a large incisional hernia with loss of domain intervention.

Adjunct botox to preoperative progressive pneumoperitoneum for incisional hernia with loss of domain: no additional effect but may improve outcomes.

INTRODUCTION: Incisional hernia with loss of domain (IHLD) remains a surgical challenge. Its management requires complex approaches including specific preoperative and intra-operative techniques. This study focuses on the interest of adding preoperative botulinum toxin A (BTA) injection to preoperative progressive pneumoperitoneum (PPP), compared to PPP alone. MATERIAL: Patients between January 2015 and March 2020 with IHLD who underwent pre-operative preparation were included. Their baseline characteristics were retrospectively analyzed, along with the characteristics of their incisional her…

Feasibility of Catheter Placement Under Ultrasound Guidance for Progressive Preoperative Pneumoperitoneum for Large Incisional Hernia with Loss of Domain.

INTRODUCTION: Large incisional hernias with loss of domain (LIHLD) of the abdominal wall remain a therapeutic challenge due to the difficulty of replacing the contents of the hernia sac into the peritoneal cavity. Preoperative progressive pneumoperitoneum (PPP) is a valuable option. The purpose of this study was to evaluate the feasibility of peritoneal catheter insertion under ultrasound guidance for PPP and to compare the morbidity and mortality of this new technique to previously used techniques in our department. METHODS: Medical records were reviewed retrospectively from February 1989 to…

[The pneumoperitoneum course forecasting and surgery tactic in the group of patients with acute and chronic cholecystitis and concomitant pathology of cardiovascular system].

The share of elderly and senile patients with acute cholecystitis concomitant cardiovascular pathology whom the laparoscopic cholecystectomy has been provided is increased. The heightened intraabdominal pressure has negative influence at the cardiovascular system, so the alternative ways for treatment of this group of patients are used in clinic. We propose the pneumoperitoneum model using the pneumatic belt which is fixed at the abdomen in preoperative period in patients with an acute and chronic cholecystitis. This model is useful to forecast cardiovascular disorders during future laparoscop…
