Acute idiopathic polyradiculoneuritis with secondary arterial hypertension in a 5-year-old male Siberian Husky.

Acute canine idiopathic polyradiculoneuritis (ACIP) is one of the most common generalised neuromuscular diseases affecting dogs. In this report, we describe a 5-year-old, 25-kg, male, intact, Siberian Husky dog with ACIP with secondary induced arterial hypertension {systolic blood pressure [mean (m) ± standard deviation (sd)], 214 ± 19 mmHg; mean blood pressure (m ± sd), 164 ± 6.36 mmHg; and diastolic blood pressure (m ± sd), 137 ± 0.7 mmHg} and sinus tachycardia. Heart rate variability analysis indicated decreased vagal activity (low root-mean-square values of successive RR interval differenc

Evaluation of a patient with suspected chronic demyelinating polyneuropathy.

Demyelinating neuropathies are typically characterized by physiological slowing of conduction velocity and pathologically by segmental loss of myelin and in some instances, evidence of remyelination. Clinically, patients with demyelinating neuropathy can be seen with inherited disorders (Charcot-Marie-Tooth disease) or acquired disorders, typically immune-mediated or inflammatory. The acquired disorders can be either acute or subacute as seen in the acute inflammatory demyelinating polyneuropathy (AIDP) form of Guillain-Barré syndrome or chronic progressive or relapsing disorders such as chron

Chronic inflammatory demyelinating polyneuropathy and other immune-mediated demyelinating neuropathies.

The identification of specific disorders caused by immune-mediated attack on Schwann cells and myelin have both improved and confused our understanding and classification of chronic inflammatory and immune-mediated neuropathies. Some investigators consider all neuropathies that respond to immunosuppressive treatment as chronic inflammatory demyelinating polyneuropathy (CIDP), regardless of whether there is evidence for demyelination. Other investigators rely on a very strict definition of CIDP and do not include variations. The goal of this article is to clarify the spectrum of these disorders

Electromyography of the diaphragm in neuromuscular disease.

We compared the diaphragmatic electromyographic (EMG) recordings from 32 patients with known neuromuscular disease and respiratory symptoms (23 neuropathies, 9 myopathies) to recordings from 23 normal subjects. Turns analysis of 219-ms sections, or epochs, of the EMG demonstrated a significant overlap between diagnostic groups, although some epochs from neuromuscular patients were significantly different from normal. Empirical rules were derived to infer neuropathic and myopathic involvement of the diaphragmatic EMG.

Guillain-Barré syndrome. Clinical manifestations and directions for treatment.

Guillain-Barré syndrome (GBS) is the commonest cause of acute neuromuscular paralysis in the developed world today. Patients present most commonly with a rapidly ascending paralysis together with sensory symptoms and variable autonomic involvement. The diagnosis is clinical, but lumbar puncture and nerve conduction studies are helpful in excluding other conditions. The improvement in prognosis in recent years is largely due to advances in respiratory intensive care management. Careful monitoring of cardiorespiratory function and in particular regular measurements of the vital capacity will hel

[Guillain-Barré syndrome following Campylobacter jejuni enteritis].

A 7-year-old boy nine days before onset of a Guillain-Barré-syndrome had had enteritis caused by Campylobacter jejuni. The clinical signs were restricted to minor motor and sensory deficits in the limbs; 5 weeks after the onset of the syndrome, recovery was complete. So far, 16 cases of Guillain-Barré syndrome after a Campylobacter enteritis have been reported in the literature.

[Vasculitic neuropathy in the Garin-Bujadoux-Bannwarth syndrome. A contribution to the understanding of the pathology and pathogenesis of the neurological complications in Lyme borreliosis].

Clinical examinations and nerve biopsies were performed on four patients with meningoradiculoneuritis and positive serology for Borrelia (Garin-Bujadoux-Bannwarth syndrome). Three patients had a painful multiplex mononeuropathy, while one presented with a picture resembling a Guillain-Barré syndrome. Nerve biopsy in two patients revealed marked perivasculitis, in part with thrombosis of the epineural vasa nervorum. In the other two patients there were small pericapillary infiltrates in the endoneurium with strikingly many plasma cells. These findings speak, on the one hand, for angiopathic-isc

Two Frontlines, One Diagnosis: Family Medicine-Neurology Collaboration Enables Early Recognition and Treatment of Acute Inflammatory Demyelinating Polyneuropathy in a Diabetic Patient.

Guillain-Barré syndrome (GBS) is an acute, immune-mediated polyradiculoneuropathy that often presents with rapidly progressive, symmetrical weakness and areflexia, typically following an infectious illness. Its recognition may be delayed in patients with underlying diabetic neuropathy due to overlapping sensory symptoms. We present the case of a 36-year-old woman with type 2 diabetes mellitus and chronic distal neuropathic pain who developed acute bilateral lower-extremity paralysis after a self-limited viral illness. Cerebrospinal fluid (CSF) revealed albuminocytologic dissociation, and magne…

Neurological Spectrum of Epstein-Barr Virus Infection: A Rare Clinical Case.

The Epstein-Barr virus (EBV) is a DNA virus that has been infecting humans since ancient times, capable of causing a wide range of pathologi … hy. Repeat EBV serology, showing strongly positive IgG and negative IgM, confirmed the diagnosis of Polyradiculoneuropathy secondary to EBV. This case underscores the rare neurological complications of EBV and the importance of considering viral infections in such presentations.

Early Guillain-Barré syndrome in coronavirus disease 2019 (COVID-19): a case report from an Italian COVID-hospital.

Guillain-Barré syndrome (GBS) is an acute polyradiculoneuropathy associated with dysimmune processes, often related to a previous infectious exposure. During Italian severe acute respiratory syndrome coronavirus-2 outbreak, a woman presented with a rapidly progressive flaccid paralysis with unilateral facial neuropathy after a few days of mild respiratory symptoms. Coronavirus was detected by nasopharyngeal swab, but there was no evidence of its presence in her cerebrospinal fluid, which confirmed the typical albumin-cytological dissociation of GBS, along with consistent neurophysiological dat…
