Adequacy of prescribing nasal continuous positive airway pressure therapy for the sleep apnoea/hypopnoea syndrome on the basis of night time respiratory recording variables.

BACKGROUND: The increased demand of full polysomnographic studies, not only for diagnostic purposes but also for continuous positive airway pressure (CPAP) titration, has produced long waiting lists. Simpler methods are therefore needed to avoid having to refer all patients for full polysomnography. The hypothesis that CPAP therapy for the sleep apnoea/hypopnoea syndrome (SAHS) can be performed exclusively on the basis of recording night time respiratory variables was tested. METHODS: The level of CPAP in a group of 41 patients (three women) of mean (SD) age 52 (10) years, body mass index 31.…

A machine learning-based test for adult sleep apnoea screening at home using oximetry and airflow.

The most appropriate physiological signals to develop simplified as well as accurate screening tests for obstructive sleep apnoea (OSA) remain unknown. This study aimed at assessing whether joint analysis of at-home oximetry and airflow recordings by means of machine-learning algorithms leads to a significant diagnostic performance increase compared to single-channel approaches. Consecutive patients showing moderate-to-high clinical suspicion of OSA were involved. The apnoea-hypopnoea index (AHI) from unsupervised polysomnography was the gold standard. Oximetry and airflow from at-home polysom…

Low leptin concentration may identify heart failure patients with central sleep apnea.

Low leptin concentration has been shown to be associated with central sleep apnea in heart failure patients. We hypothesized that low leptin concentration predicts central sleep apnea. Consecutive ambulatory New York Heart Association (NYHA) classes I-IV heart failure patients were studied prospectively, including measurement of serum leptin, echocardiography and polysomnography. Sleep apnea was defined by type (central/mixed/obstructive) and by apnea-hypopnea index ≥5 by polysomnography. Subjects were divided into four groups by polysomnography: (1) central sleep apnea, (2) mixed apnea, (3) n…

Nasal patency measured by acoustic rhinometry in East Asian patients with sleep-disordered breathing.

BACKGROUND: Nasal obstruction is thought to be a risk factor in obstructive sleep apnea (OSA). However, the relationship between nasal patency and sleep-disordered breathing remains controversial. The aim of this study was to examine the association between acoustic rhinometry findings and results of overnight polysomnography. METHODS: From February to October 2003, patients who underwent overnight polysomnography assessment were enrolled in the study. We excluded patients who were under 20 years old, had severe deviated nasal septum, had previously received nasal or palatal surgery, or could…

Forestier syndrome and obstructive sleep apnea: Surgical treatment.

INTRODUCTION: We report a case of obstructive sleep apnea that occurred as a result of Forestier disease and describe the surgical treatment that was performed. SUMMARY: The patient is a 56-year-old man who presented dysphagia for solids and liquids, snoring (score 10) and excessive daytime sleepiness for 5 years. On fiber optic laryngoscopy examination, there was interarytenoid edema and protrusion of the posterior wall of the larynx. The cervical X-Ray showed protrusion of intervertebral disc between C3-C5 (skeletal hyperostosis) and the polysomnography revealed apnea-hypopnea index (AHI) o…

[Coronary heart disease in patients with obstructive sleep apnea].

OBJECTIVE: As patients with severe obstructive sleep apnoea (OSA) have increased cardiovascular morbidity and mortality rates, the prevalence of coronary heart disease (CHD) was investigated in patients with OSA proven by polysomnography (apnoea/hypopnea index > or = 10/h). PATIENTS AND METHODS: In 440 patients (400 men, 40 women; mean age 55.3 [16-80] years) with OSA proven by polysomnography, the possible diagnosis of CHD was assessed by history, physical examination, ECG at rest and on exercise, one- and two-dimensional echocardiography and, where indicated, myocardial scintigraphy and cor…

The value of one hour daytime sleep recording in the diagnosis of the sleep apnoea syndrome.

In this study we investigated whether the diagnosis of sleep apnoea syndrome (SAS), based on night-time polysomnography (NPSG), can be predicted or excluded by a one-hour daytime polysomnography (DPSG). The results of 306 NPSGs were compared with DPSGs, which were performed the day before. Treated patients were excluded. In the 89 patients with SAS (Apnoea index (AI)>/=5) 59 showed apnoeas during the DPSG and 30 did not. In the 217 without SAS 25 showed apnoeas daring DPSG and 192 did not. Sensitivity for detecting SAS was 66%, the specificity was 88%, the positive predictive value (PPV) 70% a…

NT-proBNP is not elevated in patients with obstructive sleep apnoea.

BACKGROUND: N-terminal pro-brain natriuretic peptide (NT-ProBNP) has emerged as an important marker of cardiac stress and may reflect the severity of underlying cardiac dysfunction, which is thought to be associated with obstructive sleep apnoea syndrome (OSAS). METHODS: This study evaluated the plasma concentration of NT-ProBNP in 60 consecutive patients (median age 55.7 years, median body mass index (BMI) 31.8) who were referred to a sleep laboratory with a suspicion of OSAS. Each subject underwent measurement of morning NT-ProBNP plasma levels, polysomnography and echocardiography. Patient…

A comparison of sleep profiles in patients with dementia with lewy bodies and Alzheimer's disease.

INTRODUCTION: Sleep disturbances are common in healthy old age and in dementia syndromes. Polysomnography has demonstrated typical changes in both Alzheimer's disease (AD) and dementia with Lewy bodies (DLB) with AD being characterised by sundowning and sleep apnoea and DLB patients showing more disturbances of movement control during sleep. The technical difficulties associated with EEG sleep recordings mean that polysomnography is not possible out of specialist centres. OBJECTIVES: To use questionnaires to assess the frequency of sleep disturbances in patients with Alzheimer's disease and d…

Simplified diagnostic procedure for obstructive sleep apnoea syndrome: lower subsequent compliance with CPAP.

The aim of this study was to investigate whether a simplified diagnostic procedure based on ambulatory monitoring with MESAM IV altered subsequent compliance with continuous positive airway pressure (CPAP) in obstructive sleep apnoea (OSA) patients. During a period of 16 months, 60 patients with symptoms evocative of OSA and positive MESAM recording were prescribed CPAP after titration with standard polysomnography. Compliance was followed during 2 yrs based on built-in time counters and was compared with the compliance in two comparison groups: an equal number of equally severely affected pat…

Screening for obstructive sleep apnoea in an early psychosis cohort: a pilot study.

OBJECTIVE: Rates of obstructive sleep apnoea (OSA) appear to be lower in the youth population (< 4%) compared to the general population (6%-17%); rates in people with psychotic illness are estimated at 13.5%-57.1%. We hypothesised that this comorbidity extends to early psychosis (EP) populations, and used previously validated OSA questionnaires to screen for OSA in an EP cohort. METHOD: Fifty-three patients were screened using the OSA50 and STOP-Bang questionnaires with collection of anthropometric measures. Patients who screened positively were referred for polysomnography. RESULTS: Fifteen…
