Review of the Physiology and Anesthetic Considerations for Pleuroscopy/Medical Thoracoscopy.

Pleuroscopy or medical thoracoscopy is the second most common utilized procedure after bronchoscopy in the promising field of interventional pulmonology. Its main application is for the diagnosis and management of benign or malignant pleural effusions. Entry into the hemithorax is associated with pain and patient discomfort, whereas concurrently, notable pathophysiologic alterations occur. Therefore, frequently procedural sedation and analgesia is needed, not only to alleviate the patient's emotional stress and discomfort by mitigating the anxiety and minimizing the pain but also for yielding

Recent advances in the understanding and management of chronic pancreatitis pain.

Abdominal pain is the most common symptom of chronic pancreatitis (CP) and is often debilitating for patients and very difficult to treat. To date, there exists no cure for the disease. Treatment strategies focus on symptom management and on mitigation of disease progression by reducing toxin exposure and avoiding recurrent inflammatory events. Traditional treatment protocols start with medical management followed by consideration of procedural or surgical intervention on selected patients with severe and persistent pain. The incorporation of adjuvant therapies to treat comorbidities including

Procedural Sedation and Analgesia in Trauma Patients in an Out-of-Hospital Emergency Setting: A Prospective Multicenter Observational Study.

BACKGROUND: The quality of procedural analgesia and sedation among trauma patients has not been studied much in the prehospital setting. The main objective of this study was to characterize the quality of procedural analgesia sedation practices in prehospital settings in trauma patients. METHODS: This was an open-label observational prospective multicenter study (January 01, 2012-December 31, 2013). We included all consecutive trauma victims undergoing a potentially painful procedure on the accident scene. The primary endpoint was the procedural pain intensity. RESULTS: Data for 210 patients…

Concordant pain provocation during transforaminal epidural steroid injection for lumbosacral radiculopathy: effect on pain outcome and predictive factors.

BACKGROUND: Anecdotal report suggests that provocation of pain during epidural steroid injection (ESI) that is concordant with typical radicular symptoms predicts pain outcome following injection. However, limited evidence exists that substantiates this theory. Additionally, there is a paucity of literature investigating factors associated with the provocation of pain during ESI. OBJECTIVES: The goal of this study was to determine whether provocation of concordant radicular pain during transforaminal ESI predicts pain relief immediately after injection and at short-term follow-up. Demographic

Injections of the Foot and Ankle.

Foot and ankle injections are commonly performed by family physicians for the management of musculoskeletal conditions such as plantar fasciitis, osteoarthritis, interdigital neuromas, tarsal tunnel syndrome, and pathology of the first metatarsophalangeal joint. If symptoms persist despite conservative measures, injections may improve function and reduce pain. Before an injection, the clinician should discuss procedural risks, including pain, swelling, redness, stiffness, infection, and damage to surrounding tissues, and obtain informed consent. Corticosteroid injections can provide short-term

Performance of Hospitals When Assessing Disease-Based Mortality Compared With Procedural Mortality for Patients With Acute Myocardial Infarction.

IMPORTANCE: Quality of percutaneous coronary intervention (PCI) is commonly assessed by risk-adjusted mortality. However, this metric may result in procedural risk aversion, especially for high-risk patients. OBJECTIVE: To determine correlation and reclassification between hospital-level disease-specific mortality and PCI procedural mortality among patients with acute myocardial infarction (AMI). DESIGN, SETTING, AND PARTICIPANTS: This hospital-level observational cross-sectional multicenter analysis included hospitals participating in the Chest Pain-MI Registry, which enrolled consecutive a

Effect of multisensory stimuli on neonatal pain during blood sample collection: a network meta-analysis of randomized controlled trials.

Multisensory stimulation (MSS) effectively alleviates procedural pain in neonates during blood sample collection. However, quantitative evidence on the comparative effectiveness of different sensory stimulation types remains limited. This network meta-analysis (NMA) systematically evaluated the efficacy of MSS interventions in reducing neonatal pain during blood sampling. The results aim to provide an evidence-based foundation for selecting optimal clinical pain-management protocols. A NMA of randomized controlled trials (RCTs) was conducted according to the PRISMA guidelines. Studies using MS…

Ultrasound-Guided Peripheral Nerve Blocks for Percutaneous Treatments of Common Tendinopathies.

OBJECTIVES: Ultrasound-guided percutaneous needle fenestration (UPNF) and percutaneous needle tenotomy (UPNT) are minimally invasive procedures commonly used to treat chronic tendinopathies. However, these techniques can be painful, potentially limiting patient tolerance, satisfaction, and procedural feasibility. Although local anesthetics (LAs) provide analgesia, their potential inhibitory effects on tenocyte activity and the efficacy of orthobiologics raise concerns regarding their impact on tendon healing. The objectives of this study is to propose a practical, anatomy-based approach for th

A Multicenter Randomized Pragmatic Trial Comparing Intra-Articular Injection, Genicular Nerve Block, and Radiofrequency Ablation for Knee Osteoarthritis Pain: SKOAP Phase 2 Protocol.

BACKGROUND: Knee osteoarthritis (KOA) is a leading cause of chronic pain and disability worldwide, yet clinical guidelines disagree on how to escalate care when conservative management fails. Procedural interventions, including intra-articular injections, genicular nerve blocks (GNB), and genicular nerve radiofrequency ablation (GN-RFA), are used widely, but head-to-head comparative evidence is limited and recent RFA trials have produced inconsistent results, in part because prior studies have used controls that do not adequately mimic the procedural experience of the active arm. Efficacy of oral sucrose for preterm infants undergoing point-of-care lung ultrasound: a randomized assessor-blinded controlled trial.

Although lung ultrasound (LUS) is considered non-invasive in nature, handling and probe pressure may provoke pain and transient physiological changes in preterm infants. The aim of this study was to determine whether oral sucrose combined with a pacifier reduces procedural pain and stress in preterm infants undergoing bedside LUS. A two-arm randomized controlled trial was conducted between November 2020 and January 2023. Preterm infants undergoing LUS were randomized to receive either oral sucrose-plus-pacifier or pacifier-alone, administered 2 min before the scan. Pain was evaluated using the…

A 360° virtual reality tool to reduce opioid consumption, pain, and anxiety during dressing changes after burn surgery - A randomized controlled trial.

BACKGROUND: Many patients experience severe pain during dressing change procedures for burn wounds, and require high doses of opioids. Given the risks associated with opioids, pharmacological and non-pharmacological adjuncts are commonly used to reduce pain while minimizing opioid use. One non-pharmacological adjunct is immersive virtual reality (VR). Emerging studies suggest that active VR, which allows users to interact with the virtual world, reduces pain during burn dressing changes. However, it remains unclear whether passive VR, where the viewer remains an observer, is similarly effectiv

Factors Associated with Pain During Transperineal Prostate Fusion Biopsy Under Local Anaesthesia: A Prospective Study.

Background/Objectives: Transperineal MRI-fusion prostate biopsy under local anaesthesia is increasingly performed in the outpatient setting, which places greater weight on procedural tolerability. Psychological and anatomical determinants of pain have rarely been compared within a single model, and MRI-derived perineal anatomical parameters have not been evaluated. We aimed to identify factors associated with procedural pain and to determine at which step pain peaks. Methods: In this prospective, pre-registered, single-centre cohort study (OSF: osf.io/pbacu), 182 consecutive men referred for t…
