Inter-specialty collaboration in the formalization of a new foregut subspecialty.

Historical context and converging market conditions present a unique opportunity to study the factors contributing to the formalization of foregut, or upper gastrointestinal, surgery as a new subspecialty in United States healthcare. The aim of this study was to examine the justifications of those pursuing the formalization of a foregut subspecialty and to extract lessons for healthcare leaders on the evolving relationships between competing providers. This was a qualitative, cross-sectional study consisting of interviews, secondary survey data, and observation of society meetings between Octo

Being a university teacher in a landscape of practice-A qualitative study of critical incidents.

INTRODUCTION: Acceptance of teachers' need for pedagogical training to maintain quality in health professions education has grown. However, to enhance interest in becoming a professional teacher, a better understanding of experiences related to initial contact with teaching is needed. The framework of landscape of practice (LoP) offers a perspective highlighting identification processes as academics and clinicians participate in and navigate across familiar and new communities. The aim of this study was to explore how boundary crossing into teaching practices is expressed in educators' narrati

Becoming an academic: a trioethnography of radiographers transitioning from clinical practice to higher education.

INTRODUCTION: Diagnostic radiography relies on clinicians transitioning into academia to sustain teaching, research, and professional formation. Despite this reliance, the transition from clinical practice to higher education is often complex and poorly supported. While role shock, identity disruption, and managerial pressures are well documented across health professions, little empirical work has examined how radiographers experience becoming academics. This study explores how diagnostic radiographers navigate this transition and how professional identity is reshaped during early academic ca

Perceptions and experiences of missed nursing care among junior nurses: a qualitative study using reflexive thematic analysis.

BACKGROUND: Missed nursing care is an important indicator of healthcare quality and patient safety. Junior nurses undergoing the transition from student to professional practitioner are particularly vulnerable to missed nursing care due to limited clinical experience, insufficient professional competence, and challenges in adapting to complex clinical environments. However, the multidimensional factors contributing to missed nursing care among junior nurses remain insufficiently understood. METHODS: A qualitative descriptive study was conducted involving 12 junior nurses with ≤3 years of clin…

The Professional-Volunteer Paradox: Transforming Lived Experience into a Sense of Purpose in Dutch Oncology Patient Advocacy.

BACKGROUND AND OBJECTIVE: Transforming lived experience into a sense of purpose through patient advocacy in oncology research and policy development is increasingly demanded. However, the long-term sustainability of patient advocates - who formally operate as unpaid volunteers - remains under-researched. This study explores the motivational drivers and structural hurdles characterising the field of Dutch oncology patient advocacy to identify the prerequisites for a sustainable patient voice in oncology research and policy development. METHODS: We conducted a qualitative study among 16 Dutch o

Challenges for implementing culturally competent obstetric services in an Indigenous context in Mexico.

Obstetric healthcare for Indigenous women remains a severe problem in low-income countries with great cultural diversity and a colonial past. The work of health professionals to prevent complications leading to maternal deaths is paramount, yet in these contexts, they face significant challenges in implementing culturally competent services. This paper aims to present findings from an ethnographic study that attempted to document the experience of health professionals providing obstetric services in order to show the complex sociocultural contexts in which they perform their work.

What is the impact of the Rashomon approach in primary care education?: An educational case report of implementing dialogue and improvisation into medical education.

BACKGROUND: The excessive sub-divided or concrete pre-determined objectives found in the technological approach in contemporary medical education curricula may hinder the students' spontaneous learning about diverse needs and values in care. However, medical professionals must learn the diversity for care or a variety of social factors of the patients influencing decision making in daily practice. METHODS: We introduced a new method of curriculum development called the Rashomon approach. For testing the Rashomon approach, educational activities to teach the diversity in primary care were deve

Clinical Reasoning: A State of the Science Report.

Clinical reasoning is the cognitive process that nurses use to gather and incorporate information into a larger bank of personal knowledge. This incorporated information guides therapeutic actions, and helps determine client care. Since the process guides therapeutic actions regarding client care, failure to use the process effectively leads to poor clinical decision-making, inappropriate actions, or inaction. Because of the criticality of this process, this paper presents an analysis of the literature that reveals the current state of the science of clinical reasoning, identifies gaps in know

Molecular diagnosis in clinical parasitology: when and why?

Microscopic detection and morphological identification of parasites from clinical specimens are the gold standards for the laboratory diagnosis of parasitic infections. The limitations of such diagnostic assays include insufficient sensitivity and operator dependence. Immunoassays for parasitic antigens are not available for most parasitic infections and have not significantly improved the sensitivity of laboratory detection. Advances in molecular detection by nucleic acid amplification may improve the detection in asymptomatic infections with low parasitic burden. Rapidly accumulating genomic
