Retromolar ridge in edentulous patients: clinical considerations.

PURPOSE: A removable denture base should cover the mandibular retromolar regions to provide proper basal seal and denture function in edentulous patients. The bony residual ridge form, attached muscles, and covering mucosa provide support, stability, and retention of the planned prosthesis. There is insufficient information regarding bone anatomy, mucosal tissues, and muscles in the retromolar region after tooth loss. The purpose of this study was to examine the tissue morphology in the mandibular retromolar area of edentulous subjects and report on the clinical inferences in prosthetic and im

The use of TIPS in chronic liver disease.

The development of cirrhosis and portal hypertension in the natural history of chronic liver disease is associated with many complications. A transjugular intrahepatic portosystemic stent shunt (TIPS) is a metal prosthesis that has been shown to be very effective in lowering sinusoidal portal pressure, and therefore is effective in the management of complications of cirrhosis, especially those related to portal hypertensive bleeding and sodium and water retention. In patients with acute variceal bleeding not responding to pharmacologic and endoscopic treatments, a reduction of the hepatic veno

Changes in stability and marginal bone level around implants with hydrophilic and hydrophobic surfaces for posterior tooth replacement: A 1-year randomized clinical trial.

AIM: This two-group parallel, double-blind, randomized clinical trial compared the 1-year changes in longitudinal stability and marginal bone level (MBL) between implants with hydrophilic (HPL) and hydrophobic (HPB) surfaces. MATERIALS AND METHODS: Patients with single posterior edentulous spaces were randomly assigned to two groups (HPL and HPB), and implants were inserted using a guided protocol. Screw-retained provisional and metal-ceramic crowns were provided per the manufacturer's loading protocol. The implant stability quotient (ISQ) was measured immediately and after 15, 30, 45 (HPL lo

Effect of implant connection and restoration design (screwed vs. cemented) in reliability and failure modes of anterior crowns.

The mechanical performance of cemented or screw-retained implant-supported crowns with an internal or external configuration is yet to be understood. This in vitro study evaluated the effect of screw-retained and cement-retained prostheses on internal and external implant-abutment connections. Thereby, the reliability and failure modes of crowns were investigated. Eighty-four implants (Emfils; Colosso Evolution system) were divided into four groups (n=21 each): screw-retained and internal connection (Si), screw-retained and external connection (Se), cement-retained and internal connection (Ci)

Immediate, non-submerged, root-analogue zirconia implants placed into single-rooted extraction sockets: 2-year follow-up of a clinical study.

This study evaluated non-submerged, root-analogue zirconia implants with two different surfaces for immediate single-rooted tooth replacement in 18 patients. After tooth extraction the root was laser scanned and one-piece root analogue zirconia dental implants with one of two different surfaces were manufactured. In group A (n=6) the implant surface was roughened by sandblasting only, in group B (n=12) additional macroretentions limited to the interdental space, to avoid fracture of the thin buccal cortex, were designed prior to laser scanning. Implants were placed in the socket 1-8 days after

Three-dimensional finite element analysis of the effect of different bone quality on stress distribution in an implant-supported crown.

STATEMENT OF PROBLEM: Primary implant stability and bone density are variables that are considered essential to achieve predictable osseointegration and long-term clinical survival of implants. Information about the influence of bone quality on stress distribution in an implant-supported crown is limited. PURPOSE: The purpose of this study was to investigate the effect of 4 different bone qualities on stress distribution in an implant-supported mandibular crown, using 3-dimensional (3-D) finite element (FE) analysis. MATERIAL AND METHODS: A 3-D FE model of a mandibular section of bone with a

Predicting Failure in Early Acute Prosthetic Joint Infection Treated With Debridement, Antibiotics, and Implant Retention: External Validation of the KLIC Score.

BACKGROUND: Debridement, antibiotics, and implant retention (DAIR) is a widely used treatment modality for early acute prosthetic joint infection (PJI). A preoperative risk score was previously designed for predicting DAIR failure, consisting of chronic renal failure (K), liver cirrhosis (L), index surgery (I), cemented prosthesis (C), and C-reactive protein >115 mg/L (KLIC). The aim of this study was to validate the KLIC score in an external cohort. METHODS: We retrospectively evaluated patients with early acute PJI treated with DAIR between 2006 and 2016 in 3 Dutch hospitals. Early acute PJ

Periprosthetic joint infection (PJI) due to candida tropicalis following total knee arthroplasty (TKA): A case report.

Fungal periprosthetic joint infections (PJIs) are uncommon and diagnostically challenging, frequently associated with delayed recognition, complex antimicrobial management, and poor outcomes. Candida tropicalis PJIs are particularly difficult to manage due to variable antifungal susceptibility patterns. In the present study, a 72-year-old male developed a polymicrobial PJI following total knee arthroplasty (TKA) despite the absence of classic predisposing risk factors. Early postoperative symptoms were nonspecific, leading to empirical antibacterial therapy and debridement, antibiotics, and im

Root analog zirconia implants: true anatomical design for molar replacement--a case report.

Replacement of lost teeth using oral implants is an accepted treatment modality with well-documented high long-term success rates. Conventional screw- or threaded cylinder-type implants have been used almost exclusively. Their incongruence with the extraction socket necessitates the use of a barrier membrane or bone augmentation to prevent down-growth of connective tissue or epithelium between the implant and socket. Although some minor changes in implant design have been made, the neck and abutment connection areas have not changed much in the past 30 years. Custom-made root analog implants h

Assessment of prosthetic restorations on bone-lock implants in patients after oral tumor resection.

The feasibility of implant treatment in patients after oral ablative tumor surgery has not yet been investigated with consideration of the requisite high periodontal standards. A report on this topic has to deal not only with implant survival but also with implant health, bone response, soft tissue health, failure pattern, time of failure, and ease of restoration. For the assessment of an implant system, an overview must be accomplished that takes into account the different restorations used and their interaction with the implant system that was used. This study presents the Bone-Lock implant

Preventive Prosthodontics: Integrating Preventive Prosthodontic Principles Into the Indian Oral Health Policy Framework.

BACKGROUND: Prosthodontics is considered to be a rehabilitative discipline that aims at improving the functional, esthetic, and quality aspects of an individual's mouth after the loss of their teeth. Recent research indicates that prosthodontics can also play a preventive role by preserving oral structures, timely intervention, maintenance and prevention of biological and mechanical problems related to prosthodontic rehabilitation. Despite its clinical relevance, the concept of preventive prosthodontics remains underrepresented within Indian oral health policy frameworks. OBJECTIVE: The purpo

Problems associated with the atrophic mandible.

A range of treatment options has been presented regarding the use of dental implants to rehabilitate patients who otherwise have compromised function using conventional tissue-borne prostheses. This patient population offers significant challenges to both the surgeon and the prosthodontist. Generally speaking, IODs retained on two or more endosteal osseointegrated implants; transosteal, subperiosteal, ramus frame implant-supported and implant-retained prostheses, and a totally implant-supported design offer options to treat the severely atrophic mandible. When bone support or volume is so lack
