Interaction of human liver cytochromes P450 in vitro with LY307640, a gastric proton pump inhibitor.

The interactions in vitro of LY307640 with the cytochromes P450 (P450s) were studied using human liver microsomes, specific inhibitors of the P450s, and cDNA expressed enzymes. The kinetics of formation of the two major oxidative metabolites, desmethyl LY307640 and LY307640 sulfone, were determined using two human liver microsomal samples. The kinetic data indicated that high and low affinity sites were present for the production of both metabolites of LY307640. The Km(apparent) and Vmax(apparent) for desmethyl LY307640 formation by microsomes from human liver E (HL-E) for the high affinity si

Progress with proton pump inhibitors in acid peptic disease: treatment of duodenal and gastric ulcer.

To date, the primary therapeutic approach to peptic ulcer disease, either gastric or duodenal ulcer, remains the inhibition of gastric acid secretion. The major therapeutic goals in peptic ulcer disease are symptom relief, acceleration of crater healing, and the prevention of relapse or recurrence. Since the introduction of H2-receptor antagonists and proton pump inhibitors, the full control of acid secretion can be achieved. In the treatment of gastric ulcer, analysis of all studies that have compared lansoprazole, a newly developed proton pump inhibitor, and H2-receptor antagonists has shown

[Magnifying endoscopic observation on the effect of a proton pump inhibitor on the healing process of gastric ulcer].

The effect of a proton pump inhibitor on the healing process of gastric ulcer was investigated through magnifying endoscopic observation of the regenerated mucosa. The findings were the same obtained as those by stereoscopic microscopic observation of the effect of conventional drugs. Namely, a structureless red area consisting of one layer of regenerated epithelium was the first to procede forward the ulcer base, and a radially arranged roughly striated pattern of regenerated mucosa, in which gastric glands were formed. The regenerated mucosa changed from a striated to a granular pattern. In

Proton Pump Inhibitor-Associated Hypomagnesemia: A Case Report Highlighting a Rare Adverse Effect.

Proton pump inhibitors (PPIs) are widely prescribed for the management of gastric acid-related diseases, such as gastroesophageal reflux disease (GERD), and are generally well tolerated, with few or mild adverse effects. We present a case of an elderly female patient who developed hypomagnesemia, a rare but serious complication associated with chronic PPI therapy. The patient had been receiving esomeprazole 40 mg daily for approximately 12 months. She developed secondary electrolyte disorders, with hypocalcemia being the most evident. Her clinical manifestations initially included muscle cramp

A case report and literature review of proton pump inhibitors inducing insulin autoimmune syndrome.

BACKGROUND: Insulin autoimmune syndrome (IAS) is an autoimmune endocrine disorder characterized by hyperinsulinemia, positive insulin autoantibodies (IAA), and hypoglycemia. IAS was previously thought to be induced by drugs containing thiol groups. OBJECTIVE: We describe a case highlighting that IAS may be temporally associated with a proton pump inhibitor (PPI) exposure. This article reports a patient who took beraprost sodium, clopidogrel, atorvastatin calcium, and the PPI pantoprazole and was subsequently diagnosed with IAS, presenting with a high IAA level, refractory hypoglycemia, and ma

Altered Pathogen Spectrum of Spontaneous Bacterial Peritonitis in Patients Treated With Proton Pump Inhibitors.

BACKGROUND: Proton pump inhibitor (PPI) treatment leads to significant changes in the gut microbiota composition. We hypothesized that these alterations may transfer into distinct changes in the causative agents of spontaneous bacterial peritonitis (SBP) in patients with cirrhosis. AIMS: To investigate the impact of PPI treatment on the pathogen spectrum of SBP. METHODS: A total of 227 cirrhosis patients with culture-positive SBP from two German tertiary care centers were included and the pathogen spectrum of SBP was analysed with respect to PPI treatment. RESULTS: In the PPI-group, gram-po

Beyond acid suppression: the multifaceted role of proton pump inhibitors in Helicobacter pylori eradication.

Gastric acid is essential for digestion, host defense, and maintenance of gastrointestinal homeostasis; however, its excessive or inappropriate secretion contributes to the development and progression of several acid-related disorders. Proton pump inhibitors (PPIs) have remained the cornerstone of acid-suppressive therapies for more than four decades owing to their potent blockade of gastric H+/K+-ATPases. This review provides a comprehensive overview of the pharmacological properties, clinical applications, and current limitations of the most widely used PPIs, including omeprazole, esomeprazo

RAF-like protein kinase signaling modules control plant proton pump activity.

Proton extrusion by plasma membrane autoinhibited H+ ATPases (AHAs) enables stomatal opening and plant cell expansion. Takase et al. demonstrated that heterocomplexes of C5 and C7 RAF-like kinases activate AHAs. The modular nature of C5-C7 RAF complexes allows cell-type-specific responses to stimuli, such as stomatal guard-cell CO2 levels.

Deprescribing proton pump inhibitors in long-term care: Outcomes of a pharmacist-led initiative.

BACKGROUND: Proton pump inhibitors (PPIs) are frequently continued without clear indication in long-term care (LTC) settings. Evidence-based deprescribing strategies may reduce unnecessary exposure, but real-world data from Middle Eastern LTC facilities remain limited. OBJECTIVE: To evaluate the feasibility, implementation, and short-term safety of a pharmacist-led PPI deprescribing initiative in a long-term care facility. METHODS: This retrospective cohort study included LTC residents receiving PPIs who underwent pharmacist-led medication review between September and November 2024. Resident

Overuse of Intravenous Proton-Pump Inhibitors in the Treatment of Low-Risk Upper Gastrointestinal Bleeding.

BACKGROUND: In patients with acute upper GI bleeding (UGIB), current best practice is to discontinue or transition (to oral) intravenous proton pump inhibitors (PPIs) when esophagogastroduodenoscopy (EGD) reveals low risk or no peptic ulcer disease (PUD). STUDY: This retrospective study from a large academic medical center evaluated the use of intravenous PPIs in patients admitted with acute UGIB due to low-risk PUD or non-PUD UGIB. Complete clinical data including endoscopic lesions and the duration of intravenous PPI therapy were recorded. RESULTS: Of 335 patients with acute UGIB, 253 had

Cytochrome P450 Enzymes in the Metabolism of Proton Pump Inhibitors: Implications for the Treatment of Peptic Ulcers.

INTRODUCTION: Peptic Ulcer Disease (PUD) remains a prevalent gastrointestinal condition primarily caused by Helicobacter pylori infection and chronic NSAID use. Proton Pump Inhibitors (PPIs) are the mainstay therapy, promoting mucosal healing through profound gastric acid suppression. However, variable clinical outcomes are frequently observed, largely influenced by individual differences in PPI metabolism mediated by cytochrome P450 (CYP450) enzymes, especially CYP2C19 and CYP3A4. METHODS: The review is a critical analysis of the evidence on PPI pharmacokinetics, pharmacogenomics, and drug-d

Proton pump inhibitors and peritoneal dialysis-associated peritonitis: a cohort study in Newcastle, Australia.

BACKGROUND: Peritoneal dialysis (PD)-associated peritonitis results in poor outcomes, including hospitalisation, PD discontinuation and death. Proton pump inhibitors (PPIs) have been linked to an increased risk of organism-specific and/or overall peritonitis in prior studies, yet they are commonly prescribed in the dialysis population. AIMS: To evaluate whether PPIs are associated with an increased risk of overall peritonitis in a local cohort. METHODS: We retrospectively identified patients with incident PD at a single Australian centre between 2012 and 2021. Baseline comorbidities, medicat

Pharmacodynamics, pharmacokinetics, interactions with other drugs, toxicity and clinical effectiveness of proton pump inhibitors.

The document comprehensively reviews proton pump inhibitors (PPIs), focusing on their pharmacodynamics, pharmacokinetics, drug interactions, toxicity, and clinical efficacy. PPIs irreversibly inhibit the H+/K+-ATPase enzyme in gastric parietal cells, effectively reducing gastric acid secretion. These drugs are widely prescribed for conditions like gastroesophageal reflux disease (GERD), peptic ulcer disease, eradication of Helicobacter pylori and as a prevention against bleeding from gastrointestinal tract. The review article highlights significant drug interactions associated with PPIs. Omepr
