[Competence Network for Congenital Heart Defects: innovative research for a young patient group].

BACKGROUND: Congenital heart disease is the most frequent congenital malformation in humans and affects almost 1 % of all newborns. Thanks to advances in diagnostics and treatment, over 90 % of those affected reach adulthood today. Patient numbers are therefore growing constantly. The majority of those affected, however, remain chronically ill throughout life and require continuous specialized care. The spectrum of different clinical pictures is vast. Since this is a relatively young patient group, both research and care are lacking relevant knowledge and experience. OBJECTIVE: The Competence

Building and Sustaining Patient Engagement Infrastructure Focused on Overdose Prevention Within a US Veterans Affairs National Research Network.

INTRODUCTION: The U.S. Department of Veterans Affairs (VA) has funded intramural research for over 100 years. A growing number of research funders, including VA, have placed an increasing emphasis on engaging patients in the research process and have invested in engagement infrastructure; however, few accounts are available that detail that infrastructure. This article describes the process and outcomes to plan, build, operate, and evaluate an Overdose Prevention Veteran Engagement (VE) Panel. METHODS: We employed multiple methods to document panel creation and operation, including authors' e

Advancing a Data Justice Framework for Public Health Surveillance.

BACKGROUND: Bioethical debates about privacy, big data, and public health surveillance have not sufficiently engaged the perspectives of those being surveilled. The data justice framework suggests that big data applications have the potential to create disproportionate harm for socially marginalized groups. Using examples from our research on HIV surveillance for individuals incarcerated in jails, we analyze ethical issues in deploying big data in public health surveillance. METHODS: We conducted qualitative, semi-structured interviews with 24 people living with HIV who had been previously in

Ethical Governance of Open Data Across Biomedical Research, Healthcare, and Public Health: Privacy, Equity, Trust, and Controlled Access.

Open data has become central to biomedical research and public health, but health information is uniquely sensitive and difficult to share responsibly. In this narrative review, open data is considered as a spectrum of health-data sharing arrangements, ranging from public aggregate datasets to controlled-access repositories, federated analysis, and synthetic data. This narrative review synthesizes the scientific and societal rationale for greater openness with the ethical, legal, and governance constraints that shape what "open" can realistically mean in healthcare. We examine how data sharing

Syndromic surveillance using monthly aggregate health systems information data: methods with application to COVID-19 in Liberia.

BACKGROUND: Early detection of SARS-CoV-2 circulation is imperative to inform local public health response. However, it has been hindered by limited access to SARS-CoV-2 diagnostic tests and testing infrastructure. In regions with limited testing capacity, routinely collected health data might be leveraged to identify geographical locales experiencing higher than expected rates of COVID-19-associated symptoms for more specific testing activities. METHODS: We developed syndromic surveillance tools to analyse aggregated health facility data on COVID-19-related indicators in seven low- and middl

Anticipatory decision-making for cholera in Malawi.

Climate change raises an old disease to a new level of public health threat. The causative agent, Vibrio cholerae, native to aquatic ecosystems, is influenced by climate and weather processes. The risk of cholera is elevated in vulnerable populations lacking access to safe water and sanitation infrastructure. Predictive intelligence, employing mathematical algorithms that integrate earth observations and heuristics derived from microbiological, sociological, and weather data, can provide anticipatory decision-making capabilities to reduce the burden of cholera and save human lives. An example

Detection and Characterization of Salmonella enterica Serotypes by Simple PCR Technologies.

Polymerase chain reaction (PCR) is a molecular-based technology that has revolutionized diagnostics and characterization of pathogens, and thus affecting how we understand disease landscape. This technology has been found amenable to application on various strategies for management and control of infectious diseases. The main advantage with PCR technologies, when applied optimally, is the high sensitivity and short-turn-around time for results, thus rendering the strategy attractive to researchers in infectious diseases and public health. In this chapter, we describe PCR approaches that are in

Strengthening integration of clinical and public health systems to prevent maternal-child obesity in the First 1,000Days: A Collective Impact approach.

INTRODUCTION: Obesity interventions may be most effective if they begin in the earliest stages of life, support changes across family, clinical, and public health systems, and address socio-contextual factors. METHODS: The First 1000Days is a systematic program starting in early pregnancy lasting through the first 24months of infancy to prevent obesity among low-income mother-infant pairs in three community health centers in Massachusetts. The program uses a Collective Impact approach to create the infrastructure for sustained, system-wide changes for obesity prevention across early life clin

Effects of mosquito-proofing storm drains on adult and larvae mosquito abundance: Protocol of the IDAlErt storm drAin randomiSed controlled trial (IDEAS).

Aedes and Culex mosquitoes, known for spreading arboviruses like dengue and West Nile, thrive in cities, posing health risks to urban populations. Climate change can create suitable climatic conditions for these vectors to spread further in Europe. Cities contain numerous landscape and infrastructure elements, such as storm drains, that allow stagnant water build-up facilitating mosquito breeding. Modifying urban infrastructure to prevent water accumulation can reduce mosquito populations, but evidence is limited. The Public Health Agency of Barcelona, Spain, introduced a structural modificati

Developing capacity for implementation and evaluation of vaccine trials in Uganda: Perspective of the Makerere University Walter Reed Project.

INTRODUCTION: Infectious diseases and neglected tropical diseases continue to be a major challenge in resource limited settings, causing significant morbidity and mortality. Although vaccines are a key biomedical prevention tool, resource limited settings often lack the infrastructure, regulatory frameworks, and skilled human resource to conduct vaccine clinical trials. To address this gap, the Makerere University Walter Reed Project (MUWRP) was established and has contributed to vaccine research in Uganda and globally. METHODS: This was achieved through training a strong vaccine clinical tri

A heuristic indication and warning staging model for detection and assessment of biological events.

OBJECTIVE: This paper presents a model designed to enable rapid detection and assessment of biological threats that may require swift intervention by the international public health community. DESIGN: We utilized Strauss' grounded theory to develop an expanded model of social disruption due to biological events based on retrospective and prospective case studies. We then applied this model to the temporal domain and propose a heuristic staging model, the Wilson-Collmann Scale for assessing biological event evolution. MEASUREMENTS: We retrospectively and manually examined hard copy archival l

A Geriatric Approach to Percutaneous Coronary Interventions in Older Adults, Part II: A JACC: Advances Expert Panel.

About two in five children and adolescents will meet the criteria for a mental disorder by age 18, and more than half of youth who are accessing mental health services receive them in an educational setting. Yet there is limited evidence on the effectiveness of school-based interventions on children's mental health and education outcomes. We examine the effectiveness of two key United States' federally-funded interventions for expanding school-based capacities to improve children's health and education outcomes-School-Based Health Centers (SBHCs) and Advancing Wellness and Resiliency in Educat

Urban parks and public health: A multi-scale assessment of park and Covid-19 dynamics on public health in the US.

During the COVID-19 pandemic, urban parks emerged as vital public spaces, experiencing surges in visitation as people sought safe environments for physical and mental relief. While many other social infrastructures faced sharp declines due to gathering restrictions, parks played a crucial role in supporting public health during this time of heightened need. In such a context, where public health was already under immense strain, ensuring access to health-promoting environments without introducing further risks became especially critical. This study adopts a multiscale approach, analyzing data

Cholera in North Kivu: impacts of armed conflict on the resurgence of the epidemic, a narrative review.

Cholera remains a major public health threat in the Democratic Republic of Congo, particularly in North Kivu, a province marked by chronic armed conflict and humanitarian crises. This review explores how insecurity, mass displacement, and the collapse of health and sanitation systems have contributed to the resurgence of cholera epidemics. Recurrent armed clashes have not only weakened local health structures but also created living conditions that facilitate the spread of waterborne diseases. Efforts by health authorities and humanitarian organizations have included emergency case management,

Syndemic convergence of mpox and HIV crisis in the Philippines: implications for integrated and responsive public health action.

The Philippines stands at a critical juncture in its public health trajectory, confronting concurrent challenges of a rapidly growing HIV epidemic and the re-emergence of mpox, both of which contribute to a rising burden of disease among vulnerable populations. This article examines the emerging syndemic potential of HIV and mpox in the Philippine context, emphasizing how their convergence magnifies existing health disparities and exposes deep-rooted systemic issues. Recent epidemiological trends reveal critical shortcomings in surveillance, early detection, preventive outreach, and equitable

Comparing bicyclists who use smartphone apps to record rides with those who do not: implications for representativeness and selection bias.

Increasing population levels of cycling has the potential to improve public health by increasing physical activity. As cyclists have begun using smartphone apps to record trips, researchers have begun using data generated from these apps to monitor cycling levels and evaluate cycling-related interventions. The goal of this research is to assess the extent to which app-using cyclists represent the broader cycling population to inform whether use of app-generated data in bike-infrastructure intervention research may bias effect estimates. Using an intercept survey, we asked 95 cyclists throughou

Egypt's Ambitious Strategy to Eliminate Hepatitis C Virus: A Case Study.

INTRODUCTION: Chronic hepatitis C virus (HCV) infection is a major public health problem in many low- and middle-income countries. In 2015, Egypt's HCV infection prevalence of 7% among adults was among the highest in the world and accounted for 7.6% of the country's mortality. In 2014, Egypt embarked on an aggressive screening and treatment program that evolved into a national strategy to eliminate HCV as a public health threat by 2021. METHODS: In this qualitative case study, we analyzed Egypt's HCV control strategy using the Kingdon framework to understand how the problem, policy, and polit
