PURPOSE: To report an inter-eye difference of the ocular pulse amplitude (OPA) in a case of innominate steal syndrome, as measured by recording applanation tonometry.
METHODS: OPA was measured in a 49-year-old male before and after percutaneous transluminal angioplasty of the innominate artery.
RESULTS: Before dilation of the stenotic innominate artery, OPA was 2.00 (+/- 0.49) mm Hg in the right and 3.46 (+/- 0.53) mm Hg in the left eye. After dilation, OPA was 3.26 (+/- 0.51) mm Hg in the right eye and 3.25 (+/- 0.99) mm Hg in the left eye.

Patients with steroid-resistant nephrotic syndrome often have an unsatisfactory long-term outcome and are at risk of developing chronic renal failure. A new complex method for treating partial atrophies of the optic nerve unites surgical, physiotherapeutic, and drug effects. One electrode is attached directly to the anterior segment of the optic nerve by a collagen infusion system and the other is fixed at the back of the neck. The involved optic nerve is exposed to sinusoidal modulated currents in the rectified mode, permitting direct drug electrophoresis in parallel with electric stimulation of nerve fibers. The efficiency of the method is two times higher than of standard treatment.

An evaluation survey on Pulse-polio Immunisation (PPI) in January 2000 was carried out in rural areas of Maharashtra to assess the immunisation coverage, the knowledge regarding pulse-polio and the routine immunisation schedule. House-to-house survey was carried out and information was collected by interviewing the parents of 778 children. It was observed that PPI coverage was excellent i.e., 98%. Majority of informants were female. Knowledge about pulse-polio had a direct relationship with literacy. PURPOSE: To report the response of acute Behçet retinitis to high-dose corticosteroids.
METHOD: Case report. A 58-year-old man with Behçet disease and severe bilateral glaucoma experienced a sudden decrease of visual acuity to counting fingers in his (better-seeing) left eye. Examination disclosed hypopyon uveitis and an infiltrative retinitis threatening the fovea. He received intravenous methylprednisolone hemisuccinate, 1 gram per hour on each of 3 successive days, followed by oral prednisone and cyclosporine.

Toxicities related to low-dose weekly methotrexate are largely due to its antifolate properties. Preexisting folate deficiency is associated with methotrexate toxicity in some patients. At the onset of methotrexate therapy and throughout therapy, the physician should be vigilant regarding one or more nutrient deficiencies. A multivitamin and, where appropriate, specific daily folic acid supplements should be employed. The plethysmo time interval (PTI) is the time interval between the beginning of QRS complex on ECG and the upstroke of the pulse wave on the plethysmogram as measured by a pulse oximeter. We examined whether measuring the PTI has clinical value for evaluating neonatal peripheral circulation. We correlated PTI values and measured, from the hand and foot, height and body weight of 14 neonates. PTI was strongly correlated with height (R2 = 0.85) and body weight (R2 = 0.78). A previous study showed that albino Lewis (L) rats could be triggered into paradoxical sleep (PS) by dark pulse stimulation, i.e., turning off cage lights, whereas brown Norway (BN) rats showed no evidence of PS triggering by dark pulses (2). Infants and children undergoing upper intestinal endoscopy were monitored by both pulse oximetry and chest EKG. Fifty-seven patients between the ages of 6 weeks and 36 months underwent 60 flexible upper intestinal endoscopies. All patients received parenteral sedation only. We describe 2 adolescents with relapsing polychondritis who developed acute airway obstruction. Both were successfully treated with intravenous steroids for this complication following failure with oral steroids. Early respiratory tract involvement in younger patients seems predictive of a poor outcome and aggressive therapy with intravenous high dose steroids and/or immunosuppressive drugs appears to be indicated.
