Changes in Density of Peripapillary and Intrapapillary Capillaries on OCT Angiography in Acute Retinal Necrosis Treated by Pharmacotherapy Alone without Surgery.

PURPOSE: To analyze radial peripapillary capillaris (RPC) and intra-papillary capillaris (IPC) using optical coherence tomography angiography (OCTA) in acute retinal necrosis (ARN) with good outcome. METHODS: RPC and IPC were analyzed by OCTA in patients diagnosed with ARN and treated with pharmacotherapy alone without surgery at the Tokyo Medical University Hospital. RESULTS: A total of 13 patients were studied. Ophthalmoscopic examination showed no abnormality in the optic disc in 12 of the 13 patients. However, OCTA findings of the affected eye compared with the unaffected fellow eye reve

Traumatic proximal brachial artery injury selectively managed non-operatively: A case report and review of the literature.

A right hand dominant 18-year-old female with a body mass index greater than forty presented to the trauma bay after sustaining two gunshot wounds to her right upper extremity. On physical exam, she had doppler signals and she reported neuropathy in the right median nerve distribution. She had no active signs of bleeding and she was subsequently taken to computed tomography which revealed an abrupt proximal brachial artery opacification with distal reconstitution in addition to having air tracking into the axillary and subclavian arteries. She underwent further resuscitation with normalization

[A case of pure motor isolated finger palsy due to cerebral infarction].

A 73-year-old man, a right-handed, has been pointed out his atrial fibrillation and seen a doctor regularly for varicose veins of left leg and hypertension. He had complaint of a sudden paralysis of his left thumb and index finger while drinking beer. The next day, there was no improvement and he was admitted to our hospital. Neurological examination revealed mild weakness of the most muscles of both thumb and index finger which were innervated by radial nerve, ulner nerve, and median nerve. But he had no any other neurological deficits including sensory system. A brain MRI revealed the acute-

Incidence and outcomes of radial nerve palsy in distal third diaphyseal humerus fractures after surgical fixation.

BACKGROUND: Radial nerve palsy (RNP) is a common complication of diaphyseal humerus fractures, with primary RNP rates varying by fracture location. However, the relationship between fracture location and the risk of secondary RNP following surgical fixation remains unclear. This study aimed to evaluate the incidence of postoperative RNP based on fracture location within the middle and distal thirds of the humeral diaphysis and to describe the outcomes of RNP over time. METHODS: A retrospective cohort study was conducted of patients undergoing open reduction and internal fixation for middle an

Anterolateral Thigh Flap for Low-Voltage Fourth-Degree Electrical Burn Injury With Immediate Radial Nerve Palsy at the Elbow: A Case Report.

While high-voltage electrical injuries usually cause severe burn wounds and axonal polyneuropathy, low-voltage electrical injuries cause limited cutaneous wounds and demyelinating mononeuropathy, of which the median and ulnar nerves are the most commonly involved. We present the case of a 42-year-old man who suffered a 480-voltage electrical injury at his right elbow, resulting in a 24 × 10 cm fourth-degree burn wound and immediate radial nerve palsy. The burn wound was debrided with confirmation and preservation of radial nerve continuity. The wound was covered with a free anterolateral thigh

Plate osteosynthesis of humeral diaphyseal fractures associated with radial palsy: twenty cases.

BACKGROUND: Surgical management of humeral fractures associated with radial nerve palsy remains controversial. Some advocate surgical treatment in emergency while others recommend nonoperative treatment. PURPOSE: The aim of the study was to assess nerve recovery in a series of patients treated with open reduction and internal fixation (ORIF) with plate for humeral fracture associated with radial nerve palsy. We hypothesised that surgical treatment with ORIF in emergency allows good nerve recovery. METHODS: We conducted a retrospective study on 20 patients out of 225 humeral shaft fractures o

Acute and chronic humeral shaft fractures in adults.

Humeral shaft fractures account for up to 5% of all fractures. Many of these fractures are still being treated conservatively using functional (Sarmiento) bracing or a hanging arm cast. Union is achieved in 10 weeks in more than 94% of cases. Angulation of less than 30° varus or valgus and less than 20° flessum or recurvatum can be tolerated by the patient from a functional and esthetic point of view. The ideal candidate for this treatment is a patient with an isolated fracture. Plate and screw fixation of the fracture results in union in 11 to 19 weeks. Reported complications include non-unio

The "double wrist flexor" tendon transfer for radial nerve palsy.

In isolated high radial nerve palsy, it is traditionally taught that one should not use both wrist flexors for tendon transfers. Over the last 17 years, the author has encountered 4 unusual cases of high radial nerve palsy with concurrent direct injury to the pronator teres, flexor digitorum superficialis, and the palmaris longus in the proximal forearm. In these cases, the author used both wrist flexors, namely, the flexor carpi radialis to restore wrist extension and the flexor carpi ulnaris to restore finger/thumb extension as well as thumb radial abduction. Despite the major loss of wrist

Ulnar nerve palsy-like motor and sensory loss caused by a small cortical infarct.

A 56-year-old man with a small infarct in the left precentral knob area induced both motor and sensory impairments that were similar to right ulnar nerve palsy. The only difference from ulnar nerve palsy was that the patient showed sensory disturbance not only on the ulnar side but also on the radial side of the right ring finger.

Iatrogenic radial nerve palsy after operative management of humeral shaft fractures.

BACKGROUND: It is generally agreed that a period of observation is appropriate for primary radial nerve palsy associated with humeral shaft fractures. There is no consensus, however, with regard to secondary radial nerve palsy, particularly when it is iatrogenic. Most texts state that surgical exploration is indicated for nerve palsy that occurs after fracture manipulation, but our experience suggests that it is not necessary for radial nerve palsy developing after operative management of humeral shaft fractures. METHODS: A total of 707 humerus shaft fractures were treated operatively at our

Characterization of Major Complications of Bridge Plating of Distal Radius Fractures at a Level I Trauma Center.

BACKGROUND: Comminuted, markedly displaced distal radius fractures can cause instability requiring advanced stabilization with dorsal bridge plating. However, published complication rates of bridge plating widely vary. We hypothesize that complications of bridge plating of distal radius fractures are more prevalent than published rates. METHODS: A retrospective review was performed on all patients at an academic level I trauma center treated with a bridge plate for a distal radius fracture from 2014 to 2022. RESULTS: Sixty-five wrists were included in the final analysis: average age 53 years

A Pilot Study of a Novel Automated Somatosensory Evoked Potential (SSEP) Monitoring Device for Detection and Prevention of Intraoperative Peripheral Nerve Injury in Total Shoulder Arthroplasty Surgery.

INTRODUCTION: Peripheral nerve injury is a potentially devastating complication after total shoulder arthroplasty (TSA) surgery. This pilot study aimed to assess the feasibility of using an automated somatosensory evoked potential (SSEP) device to provide a timely alert/intervention to minimize intraoperative nerve insults during TSA surgery. METHODS: A prospective, single-arm, observational study was conducted in a single university hospital. The attending anesthesiologist monitored the study participants using the EPAD automated SSEP device and an intervention was made if there was an alert
