A 10-year, 11-month-old boy who had had chronic stridor since birth died suddenly following the development of slowly progressive symptoms of respiratory obstruction due to vocal cord paralysis. Autopsy showed extensive bilateral syringobulbia, with the greatest involvement in the nuclei ambigui. Other brain-stem tracts and nuclei were only partially affected. These findings support the theory of the early developmental origin of syringobulbia.

Sarcoidosis is a common disease and affects the respiratory system in > 90% of cases, most commonly the intrathoracic lymph nodes and the respiratory parenchyma. Less commonly, the airways are involved, and the disease is manifested as mucosal erythema, edema, granularity and cobblestoning, plaques, nodules, and bronchial stenosis, airway distortion, traction bronchiectasis, and bronchiolitis. Airway involvement may lead to airflow limitation. Involvement of oral, nasal, and pharyngeal mucosa may cause hoarseness, dysphagia, laryngeal paralysis, and upper airway obstruction. Airway symptoms ar

A man aged 66 years presented with pneumaturia as a major complaint. Cancer of the sigmoid colon with infiltration to the urinary bladder was diagnosed and the patient underwent colectomy of the sigmoid colon and partial cystectomy of the bladder in May 2015. Histopathologic examinations revealed pT4b, Si(bladder), pN(-), cM0, fStage II . Because intestinal sub-obstruction and lymphatic invasion were present, CapeOX was administered as an adjunctive chemotherapy for the high-risk Stage II cancer. Because Grade 2 peripheral neuropathy appeared as a side effect, the dose was decreased to 80% fro

Subjects with late-onset Pompe disease (LOPD) typically present as slowly progressive proximal muscle weakness. Respiratory muscle weakness and diaphragmatic paralysis are common features, and may be the initial manifestation of the disease. There is often a poor correlation between the severity of limb and respiratory muscle weakness. Early clinical observations about disproportionate hypercapnia to the respiratory muscular weakness in late-onset Pompe disease were recognized and will be discussed with special reference to blunted respiratory drive, and the connections between early clinical

We report the case of a 43-year-old African American man with a history of hypertension and chronic kidney disease presenting with hypertensive emergency and bulbar paralysis in a descending fashion, which ultimately led to acute respiratory failure. He ingested pufferfish liver during the preceding 4 hours prior to presentation, as well as canned foods and cocaine over the prior 3 days. He had a complicated hospital course requiring intubation and mechanical ventilation, as well as the development of acute respiratory distress syndrome and acute renal failure requiring haemodialysis. This cas

Acute immune-mediated polyneuropathies are a group of illnesses that cause paralysis following an infection. We describe a case of one of these illnesses being caused by the respiratory syncytial virus (RSV). An 81-year-old female was admitted for an RSV infection. She went into respiratory failure requiring intubation and mechanical ventilation and treatment with ribavirin. When she was extubated, she had bilateral lower extremity weakness and absent reflexes. Her disease progressed rapidly to involve her diaphragm, and eventually, the family agreed to let the natural death process occur. Our

Forestier's disease accompanies difficult airway management with the ossification of the anterior longitudinal ligament of the spine. The abnormalities include limited cervical mobility, constrained epiglottic elevation, exclusion of the part of aryepiglottic, recurrent nerve paralysis and thinning of respiratory tract mucosa. We planned endotracheal intubation using both AWS and bronchofiberscope in a patient with Forestier's disease. We could easily confirm his glottis by using AWS and evaluate his respiratory tract below the glottis by bronchofiberscope. We could intubate an enforced tube (

This paper describes a one-month-old girl presenting with respiratory and growth failure due to diaphragmatic paralysis associated with left brachial plexus palsy after forceps delivery. Despite continuous positive pressure ventilation and nasogastric feeding, the situation did not improve and a laparoscopic diaphragmatic plication had to be performed. When dealing with a child born with brachial plexus palsy, one must think of this possible association and if necessary proceed to the complementary radiological examinations. The treatment must avoid complications like feeding difficulties and

A 14 year-old boy became seriously ill, following an upper respiratory tract infection, with high fever, arthralgia and arthritis, and a complete paralysis of the left median nerve. There was inadequate response to NSAIDS, but improvement with prednisone. During reduction of prednisone the boy developed painful erythematous swelling of the feet, subcutaneous nodules and afterwards livedo reticularis. Skinbiopsy confirmed the diagnosis of cutaneous polyarteritis. Different aspects of the disease are discussed.

Respiratory distress and laryngeal paralysis were found to be caused by a Streptococcus equi abscess of cranial mediastinal lymph nodes, putting pressure on the trachea at the thoracic inlet. Surgical drainage was required to relieve the compression, and long-term antibiotic therapy was used to treat the bacterial infection. The trachea returned to normal diameter but left laryngeal hemiplegia persisted. Peritracheal abscesses should be considered in the differential diagnosis of inspiratory dyspnea of the horse.

A case of very unusual aetiology is reported. A severe traumatic injury of the cervical spine was associated with a suspended bilateral phrenic nerve paralysis, with limited sensory involvement, and weakness of the muscles of the right upper limb. The neurological status was otherwise normal, apart from slight weakness of the left upper limb. Artificial ventilation was necessary. Respiratory recovery was slow. The neural aetiology is discussed in relation to spinal cord and/or root involvement.

As the COVID-19 pandemic unfolds, emergency department (ED) personnel will face a higher caseload, including those with special medical needs such as persons living with spinal cord injuries and disorders (SCI/D). Individuals with SCI/D who develop COVID-19 are at higher risk for rapid decompensation and development of acute respiratory failure during respiratory infections due to the combination of chronic respiratory muscle paralysis and autonomic dysregulation causing neurogenic restrictive/obstructive lung disease and chronic immune dysfunction. Often, acute respiratory infections will lea

INTRODUCTION: Stridor is a respiratory sound caused by turbulent air flow through narrow airways. The most common cause for acute inspiratory stridor is a viral infection of the upper airways, causing laryngotracheitis (Croup). Other common causes include an allergic reaction, foreign body aspiration and trauma.
BACKGROUND: A nine week old infant with a history of meningomyelocele repair perinatally presented to the emergency room with alternating inspiratory stridor. The infant did not have a history of a recent febrile illness, and he did not suffer from cough or any other symptoms suggesti

A syndrome of alveolar hypoventilation has been identified in a group of patients with bilateral diaphragm paralysis. Eight patients were studied in whom diaphragm weakness had been suggested by paradoxical (inward) movement of the abdominal wall on inspiration, of whom seven had evidence of a generalized neuromuscular disorder. Diaphragm function was assessed quantitatively by measurement of the change in transdiaphragmatic pressure during a maximum inspiration (deltaPDi). In five patients, deltaPDi was zero and in the others ranged from 2-6 cm H2O (normal greater than 25 cm H2O) indicating p

Acute onset bulbar symptoms with respiratory failure and descending paralysis may occur in several neuromuscular disorders including variants of Guillain-Barre syndrome (GBS), diphtheria, botulism and toxins. We present a 51-year-old man who presented with complains of ptosis and dyspnea following pyrethroids spraying in an enclosed area for eradication of flea. Within 5-6 days of admission limb weakness, dysphagia, dysarthria, blurred vision, diplopia, tremor and respiratory distress added to previous symptoms. Temporal profile of events after exposure, development of similar symptoms in pati

OBJECTIVE: To determine the effectiveness of unilateral arytenoid lateralisation as a surgical treatment for laryngeal paralysis in dogs.
DESIGN: The case records of 100 dogs that received a unilateral arytenoid lateralisation for laryngeal paralysis between 1992 and 2000 were reviewed. The results of questionnaires on surgical outcome, formulated for the animal owner and the referring veterinarian, were analysed. Information obtained for dogs under 10 kg and dogs over 10 kg was analysed separately.
RESULTS: The Labrador Retriever was the most commonly affected breed. The male:female ratio w

A pregnant, non-Japanese-speaking Peruvian, and, thus, with communication difficulty, suffered hyperemesis gravidarum and had respiratory arrest, requiring cardiopulmonary resuscitation. The obese pregnant woman (prepregnancy weight: 107 kg) had vomited and lost 15 kg in bodyweight over appropriately 2 weeks prior to the arrest but had not complained due to communication difficulty, which, together with her obesity, prevented a Japanese obstetrician from noticing her severe condition. 1,000 mL of low potassium fluid plus thiamine was administered. She became unable to stand, suggesting lower-e

Bilateral vocal cord paralysis following endotracheal intubation is an uncommon cause of respiratory obstruction. We report one case, adding to ten previously identified. We dissected eight human cadaver larynges and noted the path of the anterior ramus of the recurrent laryngeal nerve. It appeared to be particularly vulnerable to compression injury as it passed between the arytenoid cartilage and the thyroid lamina. This point was confirmed by histologic sections of intubated larynges.

Bilateral adducted recurrent paralysis is a serious complication of thyroid surgery. The management of such conditions is a real challenge for ENT surgeons. Combined endoscopic and external cordopexy offers a way to alleviate laryngeal dyspnea and restore acceptable voice quality while preserving the anatomical integrity of laryngeal structures. From January 2012 to April 2023, we conducted a cross-sectional descriptive observational study with retrospective data collection, including 7 patients with bilateral post-thyroidectomy recurrent paralysis who had undergone combined cordopexy. Outcome
