BACKGROUND: Central retinal artery occlusions (CRAOs) are an important cause of vision loss that lacks standardized emergent treatment. While intravenous thrombolysis is effective in acute ischemic stroke, its safety and efficacy in CRAOs remain unclear. This study compares visual outcomes of patients with CRAOs treated with intravenous thrombolysis versus medical management (MM).
METHODS: A retrospective cohort study was done of patients with acute CRAOs presenting with count fingers or worse. Patients receiving thrombolysis tenecteplase (TNK) or tissue plasminogen activator (tPA)] were matc…

This report presents a rare case of a central retinal artery occlusion in an eight-year-old girl attributed to an undiagnosed patent ductus arteriosus (PDA). Despite intensive treatment, the patient's eyesight failed to improve. Cases of central retinal artery occlusion may occur in patients with undiagnosed, small PDA, with only symptomatic treatment being available.

INTRODUCTION: Central retinal artery occlusion (CRAO) is a serious ophthalmic disease predominantly affecting older individuals with cardiovascular risk factors. While most cases are attributed to thrombus formation from atheromatous plaques or cardioembolic events, trauma-related CRAO, though rare, presents unique diagnostic and therapeutic challenges.
CASE PRESENTATION: A 47-year-old woman with multiple controlled comorbidities presented to the emergency department with right eye pain and erythema following a traumatic injury involving a knitting needle. Initial examinations revealed hand m…

BACKGROUND: Carotid stenosis is thought to be the primary risk factor for central retinal artery occlusion (CRAO); however, it is not known whether atrial fibrillation (AF)-a cardiac arrhythmia that underlies over 25% of cerebral ischemic strokes-predisposes patients to CRAO.
METHODS: A retrospective, observational, cohort study was performed using data from the State Inpatient Databases and State Emergency Department Databases from New York (2006-2015), California (2003-2011), and Florida (2005-2015) to determine the association between AF and CRAO. The primary exposure was hospital-document…

OBJECTIVES: The etiology of central retinal artery occlusion (CRAO) is unclear in approximately 50% of patients, suggesting pathomechanical heterogeneity; moreover, little is known about outcomes according to etiology. This study investigated whether the presence of an embolic source affects outcome in CRAO.
METHODS: CRAO patients within 7 days of symptom onset were retrospectively enrolled. Clinical parameters, including initial and 1-month visual acuity, CRAO subtype, and brain images, were reviewed. CRAO etiology was categorized as CRAO with or without an embolic source (CRAO-E+ and CRAO-E…

Central retinal artery occlusion (CRAO) is a form of acute ischemic stroke that results in painless vision loss attributable to retinal infarction. A keen understanding of clinical presentation and underlying pathophysiological features is key to timely intervention and development of new treatment modalities. In CRAO, the time between initial insult to presentation is significant because, analogous to ischemic stroke, the duration of ischemia is inversely related to viable retinal tissue. A major challenge in CRAO is delayed presentation, which reduces the amount of salvageable retina. In add…

Central retinal artery occlusion (CRAO) is an ophthalmologic emergency that can lead to irreversible loss of vision. Intravenous thrombolysis (IVT) has been used experimentally for its treatment. Our study aimed to evaluate the effect of emergency IVT on CRAO and its impact on visual acuity outcomes. We conducted a retrospective observational study of patients with CRAO. A total of 46 patients with CRAO were analysed; 16 patients received IVT treatment (IVT group) while 30 did not (no-IVT group). Seven patients from the IVT group received IVT early, within 4.5 hours (h) after the onset of symp…

The optic nerve is the second cranial nerve. It is supplied by the central retinal artery. It is said that this artery starts getting incorporated within the optic nerve at around the sixth week of gestation, and by the ninth week it is entirely inside the optic nerve. However, the point of incorporation lies in the intraorbital course of the optic nerve. The present study aimed at identifying the location of the central retinal artery inside the optic nerve, i.e. the peripheral or central position. Human fetuses of second and third trimester were dissected to expose the optic nerve. Morphomet…

Ophthalmic vascular occlusion is an infrequent but devastating complication following cosmetic facial filler injection. We report a case of developing retinal artery occlusion after poly-L-lactic acid (PLLA) injection. A 49-year-old woman with multiple chronic diseases experienced sudden central visual loss and severe ocular pain in the right eye immediately after PLLA injection in the temporal region. Her best-corrected visual acuity in the right eye dropped from 20/20 to 20/200. Fundus photography showed marked optic disc edema and localized retinal whitening in the territory of the blocked …

OBJECTIVE: The purpose of this study is to determine the application of thrombolytic treatment in central retinal artery occlusion (CRAO), its real benefit, safety and possible indications for treatment.
METHODS: We searched the PubMed, Cochrane and Google Scholar databases delving first into the effectiveness of the traditional treatment for CRAO, and then comparing them with new treatment strategies with intra venous or intra arterial fibrinolysis.
RESULTS: Whereas small retrospective and open-label observational trials support the use of thrombolytic therapy, multicenter randomized trials…
