Isolated first rib fractures are uncommon. They are usually associated with severe blunt trauma, although other mechanisms have been suggested, these being (a) indirect trauma, (b) sudden contraction of the neck muscles, and (c) stress or fatigue fractures attributable to repeated pull of muscles. Two cases are reported of stress fracture of the first rib, who presented to the accident and emergency department.

We report on a previously unreported site of rib stress fracture in an elite athlete. A 27 year old sweep rower, a National champion and an Olympic finalist, presented with a 3-day history of the left-side thoracic pain. Technetium 99m bone scintigraphy showed increased focal uptake of the radioisotope, leading to the diagnosis of stress injury of the anterolateral part of the sixth rib. The authors report an exceptional case of spinal compression following an isolate rib hydatidosis. The CT scan has suspected the diagnosis. The authors recall the anatomoclinic features and specify the radiologic aspects of the osseous hydatidosis, especially the rib's localization which is very rare. MR imaging in addition of its diagnosis role showing a very evocative cyst images, is the exam of choice in order to appreciate the disease's extent and the degree of medular sufferance.

Osteochondrodysplasias are hereditary dysfunctions of the enchondral ossification with a decreased longitudinal bone-growth. We describe the case of a female child which suffered from a short rib-polydactyly-syndrome, a localised form of a osteo-chondrodysplasia, and died at the age of nine months as a result of the typical complications of this disease.

In a retrospect study we evaluated the clinical impact of traumatic rib fractures. Our series included 96 patients treated between 1986 and 1988. Haemato- and/or pneumothorax corresponded closely to the number of fractured ribs. There was a highly significant association (p less than 0.001) between the number of fractured ribs and polytrauma. The leading cause of injuries was a traffic accident.

According to our results, permanent epidural anaesthesia was significantly superior to systemic opioid treatment in patients with serial rib fractures. The main advantages were not only continuous pain relief despite the fact that the nonepidural control group required more than twice the dosage of morphine derivatives; also, the respiratory and pain-related recovery time was reduced. Two cases are reported of vascular complications in the cervical rib syndrome. In one case thrombosis developed in the distal part of the brachial artery, and in both cases compression of the subclavian artery could be demonstrated clinically and during arteriography. Attention is called to the necessity of surgical treatment of patients with evidence of vascular compression in view of the possibility of thrombotic complications. Coexistence of several causes of compression and the relationship of good therapeutic result to removal of all causes is stressed.

First and second rib fractures occurred in 11 (16%) of 69 patients undergoing median sternotomy. Rib fractures and flail chest could be fatal if gas exchange is impaired. Efficient pain relief with continuous thoracic epidural analgesia allows a good physiotherapy management without central sedation and impairment of cough reflex, this prevents pulmonary atelectasis and infection. Eighteen patients/19 were treated with success in spite of flail chest, chronic obstructive pulmonary disease and minor pulmonary contusion. Intermittent positive pressure ventilation must be reserved to severe pulmonary contusion and other crushing injuries of the chest as bronchial or great vessels ruptures.

A case of isolated fracture of a cervical rib in a young woman is presented. The mechanism of injury was direct blunt trauma. Histological examination showed changes compatible with a previous fracture of the cervical rib. Literature concerning the incidence, complications and removal of these ribs is reviewed. Closely associated with this anomaly is the thoracic outlet syndrome, a differential diagnosis of which is given.

A case of "short rib-polydactyly" (SRP) syndrome (type II-Saldino-Noonan) with a detailed study of the bones, muscles and peripheral nerves in the extremities is presented. Analysis of the literature (35 cases with different forms of SRP) and the comparison of clinical, genetic and morphological data lead us to consider that different types of SRP may be related to different alleles of the same gene. Genetic heterogeneity (mutations of different genes) is also possible.

Clinical, radiographic, and morphologic studies of non-Majewski short rib-polydactyly cases show a spectrum of abnormalities. While the findings of Yang et al reported in this journal suggest that a unique change in chondrocytes, namely PAS-positive inclusions, might distinguish one group of patients from another, experience with 4 other cases with radiographic findings similar to the case reported by Yang and colleagues does not confirm the finding.
