Anatomy and relations of the infraspinatus and the teres minor muscles: a fresh cadaver dissection study.

PURPOSE: Despite their functional importance, the infraspinatus (ISP) and teres minor (TM) muscles have been little investigated. This study aimed to describe the macroscopic morphology, innervation, and inter-relations of the ISP and TM muscles. METHODS: Forty fresh cadaver dissections and histologic analysis were performed. Three groups of specimens were distinguished according to the rotator cuff tendon status: (1) intact rotator cuff; (2) supraspinatus tendon tears with intact ISP tendon; and (3) both supraspinatus and ISP tendons torn. Muscle fiber organization and muscle and tendon leng

The Burden of Craft in Arthroscopic Rotator Cuff Repair: Where Have We Been and Where We Are Going.

The rather turbulent history of arthroscopic rotator cuff repair went through stages of innovation, conflict, disruption, assimilation, and transformation that might be anticipated when a new and advanced technology (arthroscopic cuff repair) displaces an entrenched but outdated discipline (open cuff repair). The transition from open to arthroscopic rotator cuff repair has been a major paradigm shift that has greatly benefited patients. However, this technical evolution/revolution has also imposed a higher "burden of craft" on the practitioners of arthroscopic rotator cuff repair. Technologica

Animal model for chronic massive rotator cuff tear: behavioural and histologic analysis.

PURPOSE: Massive rotator cuff tears (MRCT) are usually chronic lesions that present associated degenerative changes of the myotendinous unit that have been implicated in limitations for surgical repair. In order to develop effective therapies, it is important to establish animal models that mimic the hallmarks of the injury itself. Therefore, in the present work, we aimed to (1) optimize a rodent animal model of MRCT that closely reproduces the fatty infiltration of the cuff muscles seen in humans and (2) describe the effects of unilateral or bilateral lesion in terms of histology and behaviou

Surgical treatment of symptomatic superior labrum anterior-posterior tears in patients older than 40 years: a systematic review.

BACKGROUND: Successful arthroscopic repair of symptomatic superior labral tears in young athletes has been well documented. Superior labral repair in patients older than 40 years is controversial, with concerns for residual postoperative pain, stiffness, and higher rates of revision surgery. PURPOSE: To analyze the published data on the surgical treatment of superior labral injuries in patients aged ≥40 years, including those with concomitant injuries to the rotator cuff. STUDY DESIGN: Systematic review. METHODS: A systematic review of the literature was performed using the Preferred Report

Mechanical properties of the long-head of the biceps tendon are altered in the presence of rotator cuff tears in a rat model.

Rotator cuff tears are disabling conditions that result in changes in joint loading and functional deficiencies. Clinically, damage to the long-head of the biceps tendon has been found in conjunction with rotator cuff tears, and this damage is thought to increase with increasing tear size. Despite its importance, controversy exists regarding the optimal treatment for the biceps. An animal model of this condition would allow for controlled studies to investigate the etiology of this problem and potential treatment strategies. We created rotator cuff tears in the rat model by detaching single (s

Imaging of the rotator cuff following repair: human and animal models.

Imaging of the rotator cuff following repair may be challenging due to the type of fixation, surgical manipulation of the tissue and the presence of residual defects that may exist in the presence of good functional outcome. Both ultrasound and magnetic resonance (MR) imaging present unique advantages in evaluation of the postoperative tissue. MR imaging has superior soft tissue contrast and provides a more global shoulder assessment, including the degree of arthrosis, while ultrasound enables a more dynamic testing of the repaired tissue. Power Doppler ultrasound and new contrast agents provi

Repair of tears of the subscapularis. Surgical technique.

BACKGROUND: Rotator cuff tears involving the subscapularis are less common than those involving the superior aspect of the rotator cuff. The purpose of the present study was to report the results of repair of isolated tears of the subscapularis. METHODS: The records on eighty-four shoulders that had undergone open repair of the subscapularis tendon were reviewed. The mean age of the patients at the time of surgery was 53.2 years. The mean interval from the onset of symptoms to the time of surgery was 12.5 months. Fifty-seven tears were traumatic, and twenty-seven were degenerative. Twenty-thr

Tendon-to-bone pressure distributions at a repaired rotator cuff footprint using transosseous suture and suture anchor fixation techniques.

BACKGROUND: Interface contact pressure between the tendon and bone has been shown to influence healing. This study evaluates the interface pressure of the rotator cuff tendon to the greater tuberosity for different rotator cuff repair techniques. HYPOTHESIS: The transosseous tunnel rotator cuff repair technique provides larger pressure distributions over a defined insertion footprint than do suture anchor techniques. STUDY DESIGN: Controlled laboratory study. METHODS: Simulated rotator cuff tears over a 1 x 2-cm infraspinatus insertion footprint were created in 25 bovine shoulders. A transo

Quantitative morphology of full thickness rotator cuff tears.

The occurrence of full thickness rotator cuff tears (RCTs) at time of death, the size and distribution of those tears among the tendons of the rotator cuff, and the anterior to posterior and medial to lateral dimensions of the RCTs were determined. A subset of 57 shoulders from a group of 414 were found to have full thickness rotator cuff tears. The three-dimensional coordinates of specific bony landmarks and points defining the circumference of the RCT were digitized using a Flock of Birds DC electromagnetic tracking device. Bony landmarks were used to determine the separation between adjacen

Gene expression of procollagen alpha 1 (I) and alpha 1 (III) in partial-thickness tears of the deep pectoral tendon in chickens.

The purpose of this study was to evaluate the healing process of partial-thickness tears of the deep pectoral tendon in chickens which is considered to be equivalent to the human rotator cuff. The superior side (bursal-side) and the inferior side (joint-side) layers of the deep pectoral tendon of 80 adult chickens were lacerated in the same manner. The mRNA expression levels of alpha 1 (I) and alpha 1 (III) procollagen was at a level 2 weeks after laceration. The mRNA expression level of procollagen alpha 1 (I) was significantly higher than that of procollagen alpha 1 (III) in all experimental

Added effect of graded motor imagery to rehabilitation protocol for post-operative rotator cuff repair on pain, kinesiophobia, functions, and mobility-a randomized controlled pilot study.

BACKGROUND: Severe grade rotator cuff injuries require surgery, but post-surgical pain and kinesiophobia can hinder rehabilitation. Graded Motor Imagery (GMI) aids to reduce these challenges through its three phases. However, there is less supporting evidence on the effect of GMI in post-surgical shoulder rehabilitation cases. OBJECTIVE: To explore the added effect of GMI combined with conventional rehabilitation on pain, kinesiophobia, functions, and joint mobility following rotator cuff repair. METHODS: 16 participants (aged 45-60) were randomly assigned to experimental (GMI + conventional…

Suprascapular neuropathy: diagnosis and management.

Although historically considered a diagnosis of exclusion, suprascapular neuropathy may be more common than once believed, as more recent reports are describing the condition as a cause of substantial pain and weakness in patients with and without concomitant shoulder pathology. The etiology is traction or compression of the suprascapular nerve. This can result from a space-occupying lesion, such as a ganglion cyst, or a traction injury as a result of repetitive overhead activities. More recent studies have cited cases of traction injuries occurring with retraction of a large rotator cuff tear

The impact of an intact rotator cuff on the outcomes of reverse shoulder arthroplasty: a meta-analysis of 20,924 patients.

BACKGROUND: While reverse shoulder arthroplasty (rTSA) is commonly utilized for rotator cuff tear arthropathy, indications have expanded to include, primary glenohumeral osteoarthritis (GHOA) with intact cuff. The presence of an intact cuff may influence outcomes after rTSA because preserved cuff musculature can contribute to shoulder stability and force which could potentially improve postoperative function and reduce complication rates. However, studies have reported contradictory results on whether or not an intact cuff would provide better outcomes in patients receiving an rTSA. METHODS:

[Echography of the supraspinatus tendon: forced passive adduction maneuver].

INTRODUCTION: Imaging methods are essential for a correct diagnostic approach and treatment planning in patients with a painful shoulder. More than any other, the supraspinatus tendon is often involved in impingement syndrome and sonography (US) is now the procedure of choice for the first assessment of rotator cuff tears. We suggest a supraspinatus tendon passive stress maneuver drawing the tendon out from underneath the acromion and showing a longer portion of the tendon that would otherwise be masked by the acromion. MATERIAL AND METHODS: Twenty normal subjects (10 men and 10 women, aged 2

The role of subacromial decompression in patients undergoing arthroscopic repair of full-thickness tears of the rotator cuff: a systematic review and meta-analysis.

PURPOSE: The purpose of this study was to determine the efficacy of arthroscopic repair of full-thickness rotator cuff tears with and without subacromial decompression. METHODS: We searched the Cochrane Central Register of Controlled Trials (third quarter of 2011), Medline (1948 to week 1 of September 2011), and Embase (1980 to week 37 of 2011) for eligible randomized controlled trials. Two reviewers selected studies for inclusion, assessed methodologic quality, and extracted data. Pooled analyses were performed by use of a random effects and relative risk model with computation of 95% confid

Instability and rotator cuff tear.

Prevalence of rotator cuff tears after traumatic dislocation increases with advancing age, as a consequence of the age-associated deterioration of structures and mechanical properties of the tendons of the rotator cuff. These act as effective stabilizers of the joint, compressing the humeral head in the three-dimensional concavity of the glenoid. It is impossible to establish whether a lesion of the capsulo-labrum complex or of the rotator cuff causes or follows a dislocation, regardless of whether it is anterior or posterior. A peripheral nerve or a brachial plexus injury can be associated wi

Tears of the rotator cuff. Causes--diagnosis--treatment.

Rotator cuff ruptures are the most common degenerative tendon injury and occur mainly in older patients as multifactorial disorders manifesting the main symptoms of pain and restricted range of motion. Thorough clinical examination of the shoulder includes testing the function of the rotator cuff and leads to a tentative clinical diagnosis that is the prerequisite for diagnostic imaging procedures. Sonography of the shoulder gives rapid access to a very good sensitive overview of the rotator cuff. Conventional radiological imaging permits differential diagnosis since a reduced acromiohumeral i
