Are all hamstring injuries equal? A retrospective analysis of time to return to full training following BAMIC type 'c' and T-junction injuries in professional men's rugby union.

We aimed to determine whether the anatomical location (intramuscular tendon or T-Junction) of hamstring muscle injuries in professional men's rugby union associates with a prolonged time to return to full training and a higher rate of re-injury/subsequent injury. We reviewed the medical records of an Irish professional rugby union club to identify hamstring muscle injuries incurred across five seasons. Clinicians and players were not blinded to MRI results at the time of rehabilitation. A blinded musculoskeletal radiologist re-classified all included injuries (n = 91) according to the British …

Oral injury and mouthguard usage by athletes in Japan.

The use of mouthguards in contact sports effectively prevents oral injury and preserves oral structures. We investigated oral injuries and awareness concerning mouthguards among Japanese high school soccer and rugby players. Athletes were asked a series of questions concerning their history of oral injury while participating in sports, and their pattern of mouthguard use was determined. The data were evaluated statistically using chi-square analysis. The incidence of oral injuries was 32.3% for soccer athletes and 56.5% for rugby athletes, with 0.8% and 24.1% of the respective groups having mo…

Cervical Spine Injuries: A Whole-Body Musculoskeletal Model for the Analysis of Spinal Loading.

Cervical spine trauma from sport or traffic collisions can have devastating consequences for individuals and a high societal cost. The precise mechanisms of such injuries are still unknown as investigation is hampered by the difficulty in experimentally replicating the conditions under which these injuries occur. We harness the benefits of computer simulation to report on the creation and validation of i) a generic musculoskeletal model (MASI) for the analyses of cervical spine loading in healthy subjects, and ii) a population-specific version of the model (Rugby Model), for investigating cerv…

Comparison of thoracic spine range of motion in different types of sports and relationship to low back pain.

BACKGROUND: Reduced hip joint mobility increases lumbar spine stress, leading to disorders. However, the impact of thoracic spine mobility on lumbar spine disorders remains unclear, as do sport-specific differences in thoracic mobility. METHODS: Participants were male athletes aged ≥18 years (26 baseball, 50 rugby, 20 handball, 33 gymnastics). Thoracic spine flexion, extension, and total ROM were measured using a Spinal Mouse, and rotation ROM using an inclinometer. Study 1 examined thoracic ROM differences among sports. Study 2 classified rugby players and gymnasts based on presence or absen…

Minimally Invasive Revision Surgery Combining Endoscopic Double-Row Reinsertion and Flexor Hallucis Longus Transfer for Insertional Achilles Tendon Re-Rupture in a Semi-Professional Rugby Player: A Case Report.

Background/Objectives: Acute insertional Achilles tendon rupture (A-IATR) is a rare, often season-ending injury in athletes. Revision after a failed repair is particularly challenging, and return to sport (RTS) after such surgery is rarely documented. We report one such case. Methods: A 32-year-old semi-professional rugby prop forward sustained an insertional re-rupture eighteen months after open transosseous reinsertion without suture anchors. MRI showed complete detachment with 15 mm of retraction and no residual tendon on the footprint. A single-stage procedure combining endoscopic double-r…

Tackle Risk Factors for Head Injury Assessments (HIAs) in Sub-Elite Rugby League and Recommendations for Prevention: Head Contacts from Upright Tackles Increase the HIA Risk to Both Ball Carrier and Tackler.

BACKGROUND: The rugby league tackle has been identified as the game event with the greatest propensity for a clinically diagnosed concussion. This study aims to replicate the work conducted in professional rugby league and rugby union by examining Head Injury Assessment (HIA) events to determine the associated tackle characteristics that increase concussion risk in sub-elite rugby league players. This comparison between competition levels is important due to the less developed physiological and tackle proficiency characteristics of sub-elite rugby league players and the fewer resources availab…

What Is the Impact of Physical Effort on the Diagnosis of Concussion?

OBJECTIVE: Sport-related concussion commonly occurs in contact sports such as rugby. To date, diagnosis is based on the realization of clinical tests conducted pitch-side. Yet, the potential effect of prior physical effort on the results of these tests remains poorly understood. The purpose of this study was to determine whether preceding physical effort can influence the outcome of concussion assessments. DESIGN: Prospective observational study. SETTING: University Medicine Center. PATIENTS: A cohort of 40 subjects (20 rugby players and 20 athletes from a range of sports). INTERVENTION: A…

Selective head-and-neck cooling as a treatment method for concussions in elite male rugby union players: the Clinical Observed Outcomes with Local HEad-and-neck cooling After Diagnosed concussions (COOLHEAD) study protocol.

Sports-related concussions (SRCs) typically occur when the brain is hyperthermic. Acute head-and-neck cooling should, therefore, reduce the brain's metabolic demands, with the potential to improve recovery following an SRC. Elite ice hockey players who underwent head-and-neck cooling after sustaining a concussion (SRC) showed reduced return-to-play times, although further investigation is warranted. This paper aims to describe the methods proposed for investigating the clinical effects and feasibility of acute head-and-neck cooling in elite male rugby union players. A quasi-experimental study …

'Do we know if we need to reduce head impact exposure?': A mixed-methods study highlighting the varied understanding of the long-term risk and consequence of head impact exposure across all stakeholders at the highest level of rugby union.

BACKGROUND: One strategy to prevent and manage concussion is to reduce head impacts, both those resulting in concussion and those that do not. Because objective data on the frequency and intensity of head impacts in rugby union (rugby) are sparse, stakeholders resort to individual perceptions to guide contact training. It is unknown whether there is a level of contact training that is protective in preparing elite players for contact during matches. OBJECTIVES: This study aimed to describe how contact training is managed in elite male rugby, and how staff and players perceive contact training…
