The Significance of Scalp Involvement in Pemphigus: A Literature Review.

Scalp is a unique location for pemphigus because of the abundance of desmogleins localized in hair follicles. Scalp involvement is observed in up to 60% of patients in the course of pemphigus. The lesions may occasionally lead to alopecia. Unforced removal of anagen hairs in a pull test is a sign of high disease activity. Direct immunofluorescence of plucked hair bulbs is considered a reliable diagnostic method in patients with pemphigus. Follicular acantholysis is a characteristic histopathological feature of pemphigus lesions localized on the scalp. Trichoscopy may serve as a supplementary m…

Ewing Sarcoma Presenting as a Congenital Scalp Mass.

Ewing sarcoma is a locally aggressive, highly malignant tumor most commonly seen in the skeletal system. The "Ewing family of tumors" also includes other tissue types that are not common, such as soft tissue origin classified as extraosseous Ewing sarcoma (EES) or primitive neuroendocrine origin. Age of onset most often occurs within the first 2 decades of life. Congenital presentation of EES is exceedingly rare. We report the first described case to our knowledge of congenital EES originating from the scalp.

Lipoedematous scalp: is there an association with fatty infiltration of the parotid?

Lipoedematous scalp (LS) is an extremely rare condition characterised by a soft and boggy consistency in the scalp due to an increased layer of subcutaneous tissue. In this report, we present a case of LS in a 64-year-old Indian woman. Clinical examination revealed only vague boggy lumpiness involving the whole of occipital scalp extending to parietal scalp. MRI scalp showed diffuse fatty infiltration of the scalp, particularly at the posterior parietal and occipital convexity extending to both lateral aspects of the cranium, with homogeneous signal in keeping with fat. Incidentally MRI also fo…

Mercedes Flap With Releasing Incisions for Scalp Defects.

INTRODUCTION: The reconstruction of scalp defects is surgically challenging due to the limited distensibility in the area. OBJECTIVES: To describe a new variant of the Mercedes flap with releasing incisions for the reconstruction of medium-sized scalp defects. MATERIALS AND METHODS: Pigs' feet were used as the experimental model for the design of this new variant. Different designs were created, and the best result was achieved by measuring the tension of the edges using tensiometers. Later, this design was applied to 3 patients with medium-sized scalp defects. RESULTS: The 3 patients achie…

Dissecting Cellulitis of the Scalp with Excellent Response to Isotretinoin.

An 18-year-old man presented for evaluation of a 1-year history of painful nodules on the scalp with associated hair loss. Physical examination revealed multiple confluent, fluctuant, boggy nodules on the scalp with overlying alopecic patches. Based on these findings, a diagnosis of dissecting cellulitis of the scalp was made and the patient was successfully treated with oral isotretinoin therapy.

[Development of Researches on Scalp Acupuncture for Ischemic Stroke].

Ischemic stroke is one of the commonly met diseases in clinical practice nowadays. Acupuncture therapy is widly used in the treatment of sequela of ischemic stroke in China and its mechanisms have been extensively studied in recen years. In the present paper, the authors focus on the development of studies on the mechanism of scalp acupuncture therapy in the treatment of ischemic stroke. Results indicate that scalp acupuncture intervention can 1) improve cerebral blood circulation to promote regional energy metabolism, 2) up-regulate expression of glial cell-line derived neurotrophic factor (G…

Relation between the Peripherofacial Psoriasis and Scalp Psoriasis.

BACKGROUND: Facial involvement of psoriasis is known to be one of the clinical manifestations that indicate the severity of the psoriasis and thought to be more closely associated with certain distribution. Centrofacial (CF) psoriasis has been suggested to be related with severity of systemic disease while peripherofacial (PF) psoriasis has been thought to have connection with scalp psoriasis. OBJECTIVE: To analyze the epidemiologic characteristics, clinical features and subjective feelings of patients with facial psoriasis and to find out relationship between scalp psoriasis and facial invol…

Definitive Radiation Therapy for Angiosarcoma of the Face and Scalp.

AIM: To examine the role of radiation therapy (RT) and factors' influence on tumor control for angiosarcoma of the face and scalp (AS-FS). PATIENTS AND METHODS: We reviewed the records of 14 patients with histopathology proven AS-FS (median age=77 years) who underwent RT between 2000 and 2015. A total dose of 60 to 100 Gy was administered in 24 to 50 fractions. The median calculated biological effective dose (BED) was 93 Gy (range=63.25-120) based on α/β=10. The median follow-up was 16 months (range=5-62). RESULTS: Of the 14 patients, 8 patients (57%) experienced disease recurrence. The medi…

Incidental Syringomas of the Scalp in a Patient with Scarring Alopecia.

Syringomas are benign adnexal neoplasms of eccrine lineage, which occur most commonly in the periorbital region in middle-aged females. These cutaneous lesions rarely occur on the scalp, are typically asymptomatic and are predominantly of cosmetic significance. Involvement of the scalp may be indistinguishable from that of scarring alopecia. We present an unusual case of clinically inapparent syringomas occurring on the scalp of a 56-year-old female with alopecia who was subsequently diagnosed with lichen planopilaris after repeated scalp biopsy. In patients with unexplained hair loss, or in c…

Metastatic synovial sarcoma of the scalp: Case report.

BACKGROUND: Synovial sarcoma is a malignant tumor of soft tissue that is rarely found in the head and neck. Even less common are metastasis within the head and neck. METHODS: We describe a case of a delayed metastatic synovial sarcoma to the scalp. A man who had been diagnosed and treated 16 years previously for monophasic synovial sarcoma of the groin, presented with a new scalp lesion confirmed to be metastatic monophasic synovial sarcoma. Wide local excision and sentinel lymph node biopsy (SLNB) were performed and adjuvant radiation therapy was deferred. RESULTS: A positron emission tomog…

Multiple Trichilemmal Cysts of the Scalp in a Young Male.

Trichilemmal cyst (TC) is a benign cyst and is lined by stratified squamous epithelium that is derived from the outer root sheath. Middle-aged females are more commonly affected. It has close resemblance to epidermoid cyst and hence should be differentiated. We report a case of painful multiple TC on the scalp in a young male where a skin punch has been used as a therapeutic method.

Zosteriform configuration of multiple eccrine spiradenomas of the scalp.

BACKGROUND: Eccrine spiradenoma (ES) is a benign adnexal tumor predominantly located in the head and neck regions. Multiple neoplasms located on the scalp have been reported but never with a zosteriform configuration on the first trigeminal area. CASE REPORT: We describe an original case report of a 75-year-old Caucasian man presenting multiple subcutaneous blue and purple nodules disseminated on the first left trigeminal dermatome. All the nodules appeared gradually on a one-year period. Biopsy revealed a nodular adnexal tumor in the dermis without malignant eccrine spiradenoma (MES) transfo…

[Tumors of the scalp: special aspects of selected examples].

BACKGROUND: Tumors of the scalp display some clinical, histological and prognostic characteristics. Early recognition of tumors is hampered by dense hair growth which can result in delayed diagnosis. MATERIAL AND METHODS: Taking current literature into consideration atypical fibroxanthoma, cutaneous angiosarcoma, melanoma of the scalp, some adnexal tumors including the proliferating trichilemmal tumor as well as cutaneous metastases from visceral malignancies will be discussed. RESULTS: Based on the fact that early scalp tumors are clinically difficult to recognize, they are often diagnosed …
