Proximal and distal oblique radiography of the wrist: value in occult injuries.

Fractures of the scaphoid and capitate bones are frequently nondisplaced at the initial examination. In addition, obliquity of the fracture line may result in the fracture not being profiled on routine views. Angling the radiographic tube 30 degrees off the vertical toward the elbow results in elongation of the scaphoid and capitate bones and improves visualization of the scaphoid over routine posteroanterior radiographs. Reversing the angle of the tube to 30 degrees from the vertical toward the fingertips further elongates the capitate. This is useful for delineating occult fractures of the c

Scaphoid enchondroma.

Enchondromas are rarely found in carpal bones. This paper reports a case with scaphoid enchondroma and reviews three cases from previous literature. Three of the patients with scaphoid enchondromas presented with pathologic fractures after traumatic events. Each enchondroma had characteristic histopathologic findings. It is important to realize that although enchondroma is usually a painless lesion, it weakens the cortex of the involved bone, facilitating fracture. Thus, when cystic scaphoid fractures are encountered, enchondroma must be part of the differential diagnosis.

Tomosynthesis: A new radiologic technique for rapid diagnosis of scaphoid fractures.

Scaphoid fractures constitute 71% of all carpal bone fractures.1 Early diagnosis and treatment has significant bearing on fracture union rates and better clinical outcomes. While displaced fractures can be readily seen on plain radiograph, undisplaced fractures can require advanced imaging modalities to confirm that diagnosis. Advanced imaging such as Magnetic Resonance Imaging (MRI), Computerised tomography (CT) and bone scintigraphy are routinely used for the diagnosis of scaphoid fractures but require significant radiation exposure, increased cost and can be difficult to access.2 Tomosynthe

Dorsal Plating of Unstable Scaphoid Fractures and Nonunions.

Achieving stable fixation of displaced acute and chronic nonunited scaphoid fractures continues to be a challenge for the treating surgeon. The threaded compression screw has been the mainstay of treatment of these fractures for the last 3 decades; however, persistent nonunion after screw fixation has prompted development of new techniques. Recent results of volar buttress plating have been promising. We describe a novel technique of dorsal scaphoid plating. In contrast to volar plating, the dorsal plate is biomechanically more favorable as it utilizes the tension side of the scaphoid bone for…

Anatomic Locking Plate Fixation for Scaphoid Nonunion.

Nonunion can occur relatively frequently after scaphoid fracture and appears to be associated with severity of injury. There have been a number of techniques described for bone grafting with or without screw fixation to facilitate fracture healing. However, even with operative fixation of scaphoid fractures with bone grafting nonunion or malunion rates of 5 to 10 percent are still reported. This is the first report of an anatomic locking plate for scaphoid fracture repair in a 25-year-old right hand dominant healthy male.

Measuring orthographic transparency and morphological-syllabic complexity in alphabetic orthographies: a narrative review.

PURPOSE: Bone graft is often recommended as an adjuvant for treatment of scaphoid nonunions. However, recent literature has suggested that fibrous nonunion may be suited to treatment with rigid fixation without bone grafting. This work reported on outcomes of compression screw fixation for established scaphoid fibrous nonunions without bone graft. METHODS: Fourteen patients underwent surgical compression screw fixation without bone grafting of scaphoid fibrous nonunion between January 1, 2000, and December 31, 2012, with minimum follow-up until the time of clinical and radiographic healing. What is the role of ultrasonography in the early diagnosis of scaphoid fractures?

Scaphoid fractures are the most prevalent type of carpal bone fractures. High-spatial-resolution sonography detects direct signs of scaphoid fractures such as scaphoid cortical disruption; nevertheless, indirect signs such as radiocarpal effusion and scapho-trapezium-trapezoid effusion can also be visible. The diagnosis is performed when both direct and indirect signs of scaphoid fracture are presented. The presence of indirect signs alone is not enough to complete the diagnosis, for which more advanced imaging modalities are usually required. Here, we review the anatomy of the scaphoid, the c

Detecting fractures of the scaphoid: the value of comparative X-rays of the uninjured wrist.

Pain or tenderness in the anatomical snuff-box following trauma suggests an injury to the scaphoid or surrounding soft tissues. It is often difficult to make a definite diagnosis since normal bone markings across the scaphoid can be misinterpreted as a fracture. To clarify whether a fracture is present or not, an X-ray of the uninjured scaphoid is taken in a comparable position to the most suspicious view. If bone markings are similar on both views, then a fracture can be excluded. Conversely, if the bone markings differ, then suspicion of a fracture may be confirmed. In combination with the c

Central versus eccentric internal fixation of acute scaphoid fractures.

PURPOSE: To accurately calculate the cross-sectional area of typical scaphoid fracture patterns and compare the amount of fracture surface area available for healing when a screw was positioned in the center of the scaphoid (central) versus perpendicular (eccentric) to the fracture plane. METHODS: We employed a laser scanning technique to create 3-dimensional models of the scaphoid and permit the precise calculation of area along any cross-sectional cut of the bone. We computed approximate bone apposition areas for typical acute Herbert and Fisher fractures for 10 dry bone specimens. Next, we

Vascularized pisiform graft for the treatment of scaphoid nonunion: An anatomical study.

Scaphoid fracture can evolve into scaphoid nonunion leading to wrist arthritis. Vascularized bone flaps used to treat scaphoid nonunion are supplied by delicate, small, or short arteries that are not always reliable. The pisiform bone has never been considered as a possible treatment of scaphoid nonunion since the traditionally harvested pedicle is too short. This study aimed to characterize the vascularization of the pisiform with the goal of developing a method of harvesting it with a longer pedicle that can be used as a graft to treat scaphoid nonunion. A cadaver study on 30 upper limbs was

Failure of treatment of ununited fractures of the carpal scaphoid. The role of non-compliance.

We studied the results in thirty patients who had grafting of an ununited fracture of the carpal scaphoid bone. Psychiatric evaluations revealed that the thirteen patients who had a non-union of the initial scaphoid bone graft had significant psychiatric problems, whereas the seventeen patients in whom the non-union had healed after initial bone-grafting of the scaphoid did not have psychiatric abnormalities. Technically successful surgical revisions in the first group of patients did not result in significant clinical improvement. We concluded that most failures of the bone grafts were relate…

[Habitual scapholunar dissociation].

The authors discuss a rare form of wrist injury, involving scapholunar dissociation, which is manifested by subluxation of the scaphoid bone. The radiologic, clinical, and therapeutic aspects are described. The authors also represent a case of their own where the subluxation of the scaphoid bone became habitual.

Evaluation of the Effect of Ununion Percentage of Scaphoid on Stress and Displacement of Fragment Part of Scaphoid in Various Degrees of Wrist Extension with and without External Force.

BACKGROUND: Scaphoid fractures are almost two-thirds of different kinds of carpal fractures. Surgeons usually have challenges in deciding if … ing motion of wrist from neutral to 40 degrees of extension. RESULTS: Although Von Mises stress of scaphoid bone increased following use of external force, the difference was significant for conditions 3 and 6 (75% of union in middle and distal parts). The stress applied on scaphoid increased depends on conditions (ununion percentage of bone). Moreover, its st …

Three-dimensional computed tomographic analysis of 11 scaphoid waist nonunions.

PURPOSE: To virtually assess nonunions of the scaphoid waist using 3-dimensional computed tomography (CT) reconstruction for the amount of displacement of the distal fragment and the postfracture reduction position using the intact opposite scaphoid for reference. METHODS: We generated 3-dimensional reconstructions for 11 nonunions of the scaphoid waist and the contralateral intact scaphoids based on CT. The mean age of the patients was 25 years and the time from injury to the CT scan was 2.4 years. We used the mirrored 3-dimensional model of the healthy scaphoid to guide virtual reduction of

Overstuffing of Unstable Scaphoid Nonunions: A Radiographic Analysis of Carpal Parameters.

PURPOSE: The purpose of this study was to radiographically evaluate scaphoid length and carpal parameters before and after reconstruction of nonunions with interposition vascularized medial femoral condyle (MFC) bone graft to determine if the scaphoid was overstuffed or if normal anatomy was restored and to determine the effect on ulnar carpal translocation when the volar radiocarpal ligaments were left unrepaired. METHODS: Thirty-nine patients with established scaphoid nonunions with carpal collapse were reconstructed by interposition vascularized MFC bone grafts without repair of the volar

Lessons learned from volar plate fixation of scaphoid fracture nonunions.

UNLABELLED: Treating scaphoid nonunions presents difficulties particularly when there is bone loss, significant humpback deformity or avascular necrosis. We describe a new type of fixation with a volar scaphoid plate that adds to the methods of internal fixation that are available for the treatment of recalcitrant scaphoid nonunions. We will also discuss 'lessons learned' from a cases series. The case series includes 20 consecutive patients treated with volar buttress plating and a pedicled vascularized bone graft from the ipsilateral volar distal radius. There was clinical and radiographic ev

Operative management of nonunion scaphoid fracture in children: a case report and literature review.

Scaphoid fractures are uncommon in children, but if maltreated, they can result in nonunion. The authors report a case of left scaphoid nonunion in an 11-year-old boy. The operative management of this pseudarthrosis was performed (K-wire fixation and bone grafting of scaphoid). After a 10-month follow-up period, the left wrist regained a full range of motion with no impairment. The roentgenograms showed union of the scaphoid. Surgical management of scaphoid nonunion fractures in children offers successful fusion, with very low nonunion rate and patient's satisfaction.

Segmental fracture of the scaphoid: report of a rare fracture pattern.

INTRODUCTION: To the best of our knowledge, there are no reports in the orthopaedic and trauma literature of true segmental fracture of the scaphoid bone. We present such a case with a brief discussion of the morphology and mechanisms of injury of scaphoid fractures and the problems they present, particularly in diagnosis. CASE HISTORY: A 43-year-old male with polytrauma sustained in a motorcycle road traffic accident was treated at our hospital. His injuries included a fracture initially thought to involve the waist of the scaphoid. Because he had bilateral upper limb injuries, we elected to…

[Pseudarthrosis of the scaphoid: frequency, pathogenesis and course].

Eight-three scaphoid pseudo-arthroses were found amongst 1.104 scaphoid examinations. Sixty-seven were present at the first examination and 16 pseudoarthroses developed amongst 252 scaphoid fractures. Men were affected predominantly, particularly in the 20 to 40-year old group. Fractures in the proximal third of the scaphoid and vertical oblique fractures had a particular tendency to pseudo-arthrosis formation. The operative treatment of choice is a Matti-Russe bone graft. Only one patient in seven with definite scaphoid pseudo-arthrosis showed firm fusion.

Treatment for proximal pole scaphoid nonunion with capsular-based vascularized distal radius graft.

PURPOSE: We retrospectively reviewed the results of 89 patients with proximal pole scaphoid nonunion, 58 with avascular necrosis, treated with a capsular-based vascularized distal radius graft. METHODS: Seventy-one male and eighteen female patients with symptomatic nonunion at the proximal pole of the scaphoid were included in this study. No patient had a humpback deformity. In all patients, the vascularized bone graft was harvested from the dorsum of the distal radius and was attached to a capsular flap of the dorsal wrist capsule. After fixation of the scaphoid with a small cannulated screw
