Nocardia asteroides necrotizing scleritis associated with subtenon triamcinolone acetonide injection.

PURPOSE: To report a case of Nocardia asteroides infection after subtenon triamcinolone acetonide injection. DESIGN: Case report. RESULTS: An 80-year old female received a subtenon triamcinolone acetonide injection for postcataract surgery cystoid macular edema. Shortly after, the patient developed a chronic red eye that progressed to a fulminant necrotizing scleritis. Eventual scleral biopsy revealed Nocardia asteroides infection, which improved after 5 months of systemic antibiotic therapy. CONCLUSIONS: Subtenon triamcinolone acetonide injection should be recognized as a cause of serious …

Immunologic and clinical manifestations of infectious scleritis after pterygium excision.

PURPOSE: To present the immunologic status and clinical manifestations of patients who had infectious scleritis after pterygium excision. METHODS: This prospective noncomparative study of immunologic status and clinical manifestations involved 18 eyes of 18 patients with infectious scleritis (16 bacterial and 2 fungal infections) with a history of pterygium excision from 1999 to 2001. RESULTS: The period between pterygium excision and scleritis ranged from 1 to 36 years. None of the 18 patients had any history of systemic autoimmune disease. Elevated erythrocyte sedimentation rates were foun…

Surgically induced necrotizing scleritis occurring 48 years after strabismus surgery.

We present a 52-year-old woman who was diagnosed with surgically induced necrotizing scleritis 48 years after strabismus surgery. This case demonstrates the importance of a high index of suspicion for this sight-threatening condition, even many years after strabismus surgery.

Scleritis and other ocular side effects associated with pamidronate disodium.

PURPOSE: To evaluate reported cases of scleritis and other ocular side effects associated with pamidronate disodium, with emphasis on previously unreported cases of scleritis. DESIGN: Observational case series. METHODS: Case reports from the National Registry of Drug-Induced Ocular Side Effects (Casey Eye Institute), the Food and Drug Administration, the World Health Organization, and the literature were reviewed to determine possible adverse ocular side effects associated with pamidronate disodium. The World Health Organization's Causality Assessment Guide was used to categorize an adverse …

Magnetic resonance imaging characteristics of posterior scleritis mimicking choroidal mass.

We present imaging findings in a case of posterior scleritis, which may mimic tumoral mass lesion resulting in unnecessary enucleation. Magnetic resonance imaging was remarkable for a subretinal mass hypointense on T2 and hyperintense on T1 weighted images. A peripheral rim of hypointensity was noteworthy, suggestive of sclerouveal thickening. There was an ill-defined area of increased T2 signal intensity adjacent to globe at the site of nodular lesion implying an inflammatory process. A linear contrast enhancement was seen within the bulbus oculi which may represent detached retina by exudati…

Early successful treatment of postoperative necrotizing pseudomonas scleritis after trabeculectomy.

PURPOSE: The authors describe a patient who developed pseudomonas scleritis after a routine trabeculectomy. METHOD: The patient underwent trabeculectomy for poorly controlled intraocular pressure and progressive visual field loss. On the second postoperative date he developed severe pain, significant anterior chamber reaction, and hypotony. Scleral cultures taken at the time of surgical choroidal drainage grew pseudomonas aeruginosa. RESULT: Surgical reconstruction of the necrotic scleral area and intensive antibiotic treatment lead to a successful outcome. CONCLUSION: Early recognition and…

Serum interleukin (IL-1 beta) in patients with diffuse scleritis.

Scleritis is a painful, destructive inflammatory eye disease. We measured serum IL-1 beta in ten patients with diffuse scleritis and in control subjects matched for age and sex. Patients with diffuse scleritis were found to have significantly elevated serum concentrations of IL-1 beta in comparison to normal subjects (P less than 0.02). The results suggest that IL-1 beta is associated with diffuse scleritis and may influence scleral breakdown in diffuse scleritis.

Peripheral corneal ulcers, conjunctival ulcers, and scleritis after cataract surgery.

After undergoing uneventful cataract extractions, four patients developed an inflammatory reaction that included scleritis and peripheral corneal infiltrates and ulcers. Two of these patients also developed conjunctival ulcers. The inflammatory reaction in all four patients responded to topical corticosteroids.

Infectious scleritis: a review of etiologies, clinical features, and management strategies.

Infectious scleritis is a severe and potentially vision-threatening inflammation of the sclera caused by microbial invasion. Unlike autoimmune scleritis, infectious scleritis is less common with a prevalence of 5%-10% of all cases of scleritis. The initial clinical presentation may often resemble autoimmune scleritis, thus resulting in a delayed diagnosis and poorer outcomes. A high level of suspicion is required in such cases and risk factors such as with a history of trauma or surgery and worsening or refractory to immunosuppressive medications as these may well be infectious. While in estab…

[Peripheral ulcerative keratitis with necrotizing scleritis, manifestation of granulomatosis with polyangiitis].

BACKGROUND: Granulomatosis with polyangiitis (GPA) is an antineutrophil cytoplasmic antibody (ANCA)-associated autoimmune disorder that typi … nvolvement has been show to occur in around 30% percent of cases in several studies. It could cause scleritis as well as conjuntivitis, episcleritis, necrotizing keratitis, corneoscleral perforation, posterior uveitis, and optic neuritis. C-ANCA and high positive PR3 autoantibody have strong association and sensitivity with GPA. Rituximab has been prove to …

Idiopathic Anterior Scleritis in Pediatric Patients: A Report of Two Cases.

This study presents two cases of idiopathic anterior scleritis in young female patients, one of whom progressed to necrotizing scleritis. Both cases posed significant diagnostic challenges and ultimately required immunomodulatory therapy after their respective inadequate responses to the initial standard treatments. Despite comprehensive laboratory evaluations, no underlying systemic etiology was identified in either patient. Pediatric anterior scleritis is extremely rare, and these cases underscore the importance of its early recognition and appropriate management to prevent sight-threatening…

Nodular scleritis-a rare presentation of COVID-19& variation with testing.

PURPOSE: - To report a rare case of patient presenting with nodular scleritis and SARS-CoV2. OBSERVATIONS: This case highlights a unique presentation of SARS-CoV2 positive patient with nodular scleritis as a presenting feature. Patient initially had ocular symptoms and developed only mild systemic features subsequently which did not require hospitalization. COVID testing done at different time points showed variable results which correlated with the ocular features. This patient was followed up during quarantine using tele-ophthalmology. CONCLUSION AND IMPORTANCE: This case highlights a poss…

Nodular Scleritis Revealing Metastasis of Breast Cancer: Diagnosis Not to Be Neglected.

We report the case of ocular metastasis in a 48-year-old woman presenting left eye redness and pain. The patient was followed in another health institution for nodular scleritis and received systemic corticosteroids with moderate improvement. Medical history revealed that she was diagnosed three years ago with invasive ductal carcinoma of the left breast treated by tumorectomy with ganglion dissection. An ophthalmological examination found a hard scleral nodule with vascular architectural disorganization. A biopsy was performed, and the histopathological study revealed the presence of secondar…
