[Use of immunomodulator derinat in the treatment of patients with surgical sepsis in traumatic shock].

The reparative effects of Derinat were studied in 15 patients with traumatic shock whose state was complicated by the development of sepsis. The immunomodulating effect of the drug lies in increasing the number of lymphocytes, reducing the total number of granulocytes and in elevation of the number of functionally valuable cells in them. Using Derinat was shown to facilitate the restoration of the initially reduced number of blood erythrocytes. The maximum effect of the drug was observed within 3-4 days. Readministrations of Derinat should be made twice a week.

Effects of ethanol and naltrexone in a model of traumatic brain injury with hemorrhagic shock.

BACKGROUND: Ethanol predisposes to traumatic injury and causes respiratory depression and cardiovascular compromise in models of traumatic brain injury (TBI) and hemorrhagic shock (HS). Endogenous opioids may play a role in ethanol intoxication and TBI. We studied the effects of ethanol and the opiate antagonist agent naltrexone (NTX) in a TBI/HS model. METHODS: Fifty-six pigs (20 kg) were anesthetized with isoflurane, intubated, instrumented, and subjected to fluid percussion TBI with concurrent 30 ml/kg hemorrhage over 30 min. Seven groups were studied: Control, EtOH, NTX, INJ, INJ/EtOH, IN

[Study of mechanisms of the antihypotensive effects of antishock suits in injured with traumatic shock].

The mechanisms of antihypotensive action of antishock overalls (ASO) were studied on the basis of comparison of 30 patients with traumatic shock at the prehospital stage of medical care; in 15 of these ASO was used, in the other 15 traditional antishock intensive care. Use of a portable device for noninvasive measurement of cardiac stroke volume at the prehospital stage permitted us detect the principal mechanisms of liquidation of shock-induced arterial hypotension with the use of ASO: increase of cardiac output at the expense of autohemotransfusion from the lower part of the body to the uppe

[Mechanisms of the beneficial effect of indomethacin and nimodipine in the treatment of traumatic shock in rats].

Aiming to study the pathogenetic mechanisms of the disturbed function of the central nervous system in traumatic shock we have determined the dynamics of changes of eicosanoids (PGF2 alpha, TxB2, 6-keto-PGF1 alpha, peptidoleukotrienes) in the brain structures (medulla oblongata, hypothalamus) of the experimental animals subjected to bilateral trauma of the hind legs (tourniquet trauma, LD50). Considering that our previous data have shown the importance of eicosanoid mediatory system in the pathogenetic mechanisms of shock, we have studied possible use of prostaglandin synthesis inhibitors (ind

[Effect of the antioxidant ionol on myocardial energy metabolism and the course of traumatic shock].

Energy metabolism was studied in experiments on rats with due regard for the individual features of the organism's reaction to trauma. The animals were divided into 2 subgroups according to the survival period during the experiment. Disorders of mitochondrial function were revealed in both subgroups, but they differed in degree according to the severity of the process. Preliminary injection of the antioxidant ionol prolonged the animals' survival and prevented disorders of mitochondrial function. The article discusses the possibility of preventing activation of free-radical oxidation of the me

[Functional state of the endogenous opioid peptide system in traumatic shock in rats].

The experiments have been performed on 120 Wistar rats and 546 CBWH mice. Met-enkephalin (ME) and beta-endorphin (BE) levels were determined to increase in blood and mid-brain 3 hours later after soft tissue compression of pelvic extremities. 48 hours later after six-hour compression BE and ME level in blood was increased, BE concentration in blood was also increased, and enkephalins' content in brain and blood was decreased as against the control. Opioid peptides influence on the host compression period has been studied. D-agonists were established to decrease, but mu-agonists were establishe

Efficacy of a combination thromboxane receptor antagonist and lipoxygenase inhibitor in traumatic shock.

The effects of a thromboxane receptor antagonist having lipoxygenase inhibitory activity, L-655,240 (3-[1-(4-chlorobenzyl)-5-fluoro-3-methyl-indol-2-yl]2,2-dimethylpropa noic acid) (1 mg/kg per h) were studied in a standardized model of traumatic shock. Pentobarbital (35 mg/kg) anesthetized rats subjected to Noble-Collip drum trauma were characterized by a 82 +/- 12 min survival time, a 20-fold increase in plasma cathepsin D activity, and a 6-fold increase in plasma myocardial depressant factor (MDF) activity. L-655,240 significantly attenuated the accumulation of MDF activity in the plasma (7

[Therapeutic effect of a saline solution with sodium lactate (lactasol) in traumatic shock].

Acute experiments on 67 adult dogs have demonstrated that infusions of lactasol in a dose of 25 ml/kg in the early period and in a dose of 50 ml/kg in the late period of shock are not efficacious enough. Injection of the increased amount of the saline in the early and late period of shock (up to 50 and up to 100 ml/kg, respectively) makes it possible to save from death almost all the animals. A conclusion is made that the therapeutic effect of lactasol depends on the amount of the saline injected and on the period of shock during which the saline is injected, as well as on the specific action

Influence of fibrinogen levels in dogs on mortality from hemorrhagic and traumatic shock.

1) Fibrinogen levels appear related to the outcome of hemorrhagic shock in dogs. 2) Fibrinogen levels can vary with different animals based on location, disease, stress, nutrition, or other factors. 3) Any experiments in hemorrhagic shock (or anything else) should utilize simultaneous and paired controls of animals from the same source or they may be subject to major error.

Decreased activity of NADPH oxidase in alveolar macrophages as a result of traumatic shock.

NADPH oxidase activity is significantly decreased in alveolar macrophages isolated from rabbits subjected to traumatic shock. In vitro studies indicate that activity of the enzyme is depressed in subcellular fractions of both resting and phagocytosing macrophages from shocked animals. Phagocytosis stimulates a twofold increase in NADPH oxidation in control alveolar macrophages, whereas NADPH oxidase activity is stimulated to a much lesser degree in macrophages from shocked animals. Results of this study suggest that the decreased activity of NADPH oxidase in alveolar macrophages from shocked a
