Comparative study of total shoulder arthroplasty versus total shoulder surface replacement for glenohumeral osteoarthritis with minimum 2-year follow-up.

BACKGROUND: Compared with total shoulder arthroplasty (TSA), total shoulder surface replacement (TSSR) may offer the advantage of preservation of bone stock and shorter surgical time, possibly at the expense of glenoid component positioning and increasing lateral glenohumeral offset. We hypothesized that in patients treated for osteoarthritis with a sufficient rotator cuff, TSA and TSSR patients have comparable functional outcome, glenoid component version, and lateral glenohumeral offset. METHODS: We conducted a retrospective cohort study with a minimum of 2 years of follow-up. Patients in t

Allograft-Prosthetic Composite Reconstruction for Massive Proximal Humeral Bone Loss in Reverse Shoulder Arthroplasty.

BACKGROUND: Allograft-prosthetic composite (APC) reconstruction of the humerus represents an appealing surgical technique when implantation of a reverse prosthesis is considered in the setting of substantial proximal humeral bone loss1-4. Humeral APCs have been successfully performed in the past with a non-reverse shoulder prosthesis4,5. Reconstruction of the proximal part of the humerus with an allograft provides adequate support and fixation for the humeral component, allows restoration of length and lateral offset, and provides an opportunity for soft-tissue reattachment when needed. On the…

Quantitative evaluation of retrieved reverse total shoulder arthroplasty liner surface deviation and volumetric wear.

Polyethylene wear is a known complication in total joint arthroplasty, however, in vivo wear rates in reverse total shoulder arthroplasty (RTSA) remain largely unknown. This study aimed to quantify volumetric and surface deviation changes in retrieved RTSA humeral liners using a novel micro-computed tomography (μCT)-based technique. After IRB-approval, 32 humeral liners (single manufacturer and model) with term-of-service greater than 90 days were analyzed. Clinical demographics and surgical data were collected via chart review. Unworn liners were used as geometric controls. Retrieved and unwo

Avoiding pitfalls and complications in total shoulder arthroplasty.

To further minimize complications in the currently available arthroplasties and to improve results, the surgeon must understand all aspects of gleno-humeral joint replacement. Many of the important variables for successful results are under the surgeon's control and are discussed in this article.

The Role of Humeral Neck-Shaft Angle in Reverse Total Shoulder Arthroplasty: 155° versus <155°—A Systematic Review.

The aim of this study was to have updated scrutiny of the influence of the humeral neck-shaft angle (HNSA) in patients who underwent reverse shoulder arthroplasty (RSA). A PRISMA-guided literature search was conducted from May to September 2021. Clinical outcome scores, functional parameters, and any complications were reviewed. Eleven papers were identified for inclusion in this systematic review. A total of 971 shoulders were evaluated at a minimum-follow up of 12 months, and a maximum of 120 months. The sample size for the "HNSA 155°" group is 449 patients, the "HNSA 145°" group involves 14

Stemless humeral head hemiarthroplasty for multiple indications in younger patients aged ≤ 65 years : long-term results at a mean of 17 years.

AIMS: Stemless humeral head arthroplasty (HHA) offers a treatment option for various irreversible shoulder pathologies. This study aimed to report long-term results of HHA as a hemiarthroplasty for different indications in patients aged ≤ 65 years. METHODS: Patients documented in a prospectively collected local database who underwent stemless HHA with a minimum follow-up of 12 years and a maximum age at surgery of 65 years were included. In all, 88 shoulders (86 patients) were available for cumulative revision-free implant survival analysis according to the Kaplan-Meier method. The clinical e

Short-term clinical outcome of an anatomic short-stem humeral component in total shoulder arthroplasty.

BACKGROUND: Short-stem press-fit humeral components have recently been developed in an effort to preserve bone in total shoulder arthroplasty (TSA), but few studies have reported outcomes of these devices. The purpose of this study was to evaluate the short-term clinical outcomes of an anatomic short-stem humeral component in TSA. We hypothesized that the implant would lead to significant functional improvement with low rates of radiographic loosening. METHODS: A multicenter retrospective review was performed of TSAs using an anatomic short-stem humeral component (Apex; Arthrex, Inc., Naples,

Antibiotic Spacers in Shoulder Arthroplasty: Comparison of Stemmed and Stemless Implants.

BACKGROUND: Antibiotic spacers in shoulder periprosthetic joint infection deliver antibiotics locally and provide temporary stability. The purpose of this study was to evaluate differences between stemmed and stemless spacers. METHODS: All spacers placed from 2011 to 2013 were identified. Stemless spacers were made by creating a spherical ball of cement placed in the joint space. Stemmed spacers had some portion in the humeral canal. Operative time, complications, reimplantation, reinfection, and range of motion were analyzed. RESULTS: There were 37 spacers placed: 22 were stemless and 15 we

[Bony glenoid reconstruction in revision arthroplasty].

Glenoid pathologies are one of the main indications for shoulder arthroplasty revisions. In most cases, the glenoid bone defect is associated with insufficiency of the rotator cuff, so that only reverse total shoulder arthroplasty can be considered as a revision implant. For stable permanent fixation of the reverse glenoid component, to avoid inferior glenoid notching, and to achieve optimal function, physiological restoration of three-dimensional glenoid anatomy is necessary. In order to approach the individual glenoid pathologies in a differentiated and optimal manner, it is important to cla

Gorham-Stout syndrome of the shoulder.

INTRODUCTION: Gorham-Stout syndrome (GSS) is a rare but severe subtype of idiopathic osteolysis. There are no guidelines for the treatment of GSS. We analysed different diagnostic and therapeutic regimes and we describe the sucessful treatment of GSS considering individual patient factors. METHODS: We diagnosed three patients with shoulder-specific GSS using clinical, radiological and histopathological examinations. Two out of three patients with similar clinical appearances were treated non-operatively. One patient was treated by reverse shoulder arthroplasty. All patients were analysed retr

Role of Subscapularis Repair on Muscle Force Requirements with Reverse Shoulder Arthroplasty.

Concomitant repair of the subscapularis with reverse shoulder arthroplasty (rTSA) is controversial. To evaluate the biomechanical impact of subscapularis repair with rTSA, a cadaveric shoulder controller quantified the muscle forces required to elevate the arm during scapular abduction with the elbow flexed at 90°. The results of this study demonstrate that concomitant subscapularis repair with rTSA creates a biomechanically unfavorable condition during arm elevation. Specifically, repair of the subscapularis significantly increased the force required by the deltoid and posterior rotator cuff

Total reverse shoulder arthroplasty: European lessons and future trends.

In the late 1980s, Grammont introduced a new reverse total shoulder arthroplasty (TSA), with a hemisphere directly attached to the glenoid surface and with medial positioning of the center of rotation to overcome former shortcomings. Over the past few years, results from several mid- and long-term clinical studies of this new TSA have demonstrated that unexpectedly good functional outcomes and pain relief (Constant-Score, 60-69) could be achieved, even in patients with progressive superior migration, joint destruction, and rotator cuff deficiency. In all these studies, however, limited range o

Management of a pathological fracture in a rare case of Gorham Stout disease of the hip with a mega prosthesis.

Gorham-Stout disease (GSD) is a rare bone disease of unknown etiology, characterized by spontaneous and progressive resorption of bones. This disease occurs most commonly in males of the age group of fewer than 40 years and has no genetic predisposition. It most commonly affects the skull, spine, pelvis and shoulder girdle. The diagnosis of GSD is established based on the combination of clinical, radiologic, and histological features, after excluding other diseases. There is no standard treatment for it so far, and the therapy depends on patient's clinical condition and may include surgery, ra

Usefulness of diaphragmatic ultrasound in the early diagnosis of phrenic nerve palsy after shoulder surgery in the prevention of post-operative respiratory complications.

Phrenic nerve block is a complication that can occur after brachial plexus anaesthesia above the clavicle. The main consequence of this blockage is ipsolateral diaphragmatic paralysis, which can sometimes lead to the appearance of post-operative respiratory complications. A case is presented on a woman, who after having undergone a total shoulder prosthesis, presented with dyspnoea in the post-operative recovery unit. A diaphragmatic ultrasound was performed that enabled a rapid diagnosis to be made of a complete paralysis of the ipsolateral hemi-diaphragm. Given the suspicion of phrenic nerve…

[Ischemic ulcer in Madelung disease].

We describe a 50 years old man with multiple symmetric lipomatosis (Madelung disease) and two years history of intermittent claudication who was hospitalized because of persistent ulcer of left foot. Lipomata were located on the neck, shoulders and chest; they enlarged gradually over a few years, becoming huge in size. Ulceration of the foot appeared a few weeks before hospitalization and its ischaemic origin was uncertain because of atypical character (painless, ankle/brachial index > 0.5). After angiographic examination patient was assigned for conservative treatment. Intravenous infusions o

Replacement of the proximal humerus with a ceramic prosthesis: a preliminary report.

Insertion of a multicomponent ceramic prosthesis with extracortical fixation to the conically reamed shaft of the humerus was performed in 7 patients after resection of 4 malignant and 3 benign tumors of the proximal humerus. The length of the replacement ranged from 9 to 18 cm. Fixation was secure in all but one patient, who sustained a fracture through the proximal humerus. Subluxation occurred in 3 patients but posed no difficulty, except in one patient who required revision for a dislocation. Functionally, little active motion is achieved, but the implant provides a spacer to permit effici
