The goal of the current study was to provide an easy to use criterion to diagnose neurocranial asymmetries. An anatomical survey of the external aspect of the skull base was planned to correlate with radiological studies. In a sample of 239 skulls, variations of the angular relationship between the structures seen on a basilar view of the cranium were analysed. An anterior and a posterior transverse line were plotted with respect to the midline. Departures from 90 degrees to the midline for either of the transverse lines were less than 5 degrees in normal skulls, between 5 degrees and 7 degree…

Intraoperative monitoring of cranial nerves is performed to minimize postoperative cranial nerve dysfunction. We performed electrophysiologic monitoring of motor cranial nerves with a NIM 2 unit from Xomed Treace and a patient multiplexer developed in our clinic. This multiplexer allows simultaneous monitoring of four cranial nerves and is additionally equipped with a bipolar stimulation mode. This intraoperative monitoring was used during 102 skull base operations. Of these, 44 operations were acoustic neuroma removals by translabyrinthine approach and 36 by a middle fossa approach. Various o…

In 1981, the senior author first reported the use of the pericranial flap for support of the brain following resection of the anterior skull base. Since the initial report, considerable experience has been gained with this flap. It is currently deployed via a bifrontal craniotomy and is placed primarily as a support beneath the frontal lobes. The pericranial flap may also be used to repair dural laceration and defects. The sinonasal surface is routinely grafted with split-thickness skin or dermis. This technique provides an excellent barrier between the sinonasal cavity and the cerebrospinal f…

Prognosis was once considered hopeless for many patients with malignant neoplasms of the anterior skull base. Craniofacial resection of these lesions has now been found curative for many patients. However, massive resections of the anterior skull base pose many potentially life-threatening complications. Cerebrospinal fluid fistula with meningitis is especially troublesome when previous radiation therapy compromises wound healing. In such circumstances local tissue flaps and skin grafting may not provide reliable separation of the brain from the nose. The pectoralis major myocutaneous flap was…

Our experience in 125 cases of traumatic frontal skull-base and mid-face lesions is discussed. 88 patients were operated on immediately after injury whereas 37 were operated on when the presented with complications due to the trauma some time after the injury. Small aeroceles seen on the computer tomogram are a definite and accurate sign of a dural rupture. The indications for an exploration of the frontal skull-base should be rather liberal since most dura lesions are difficult to verify. The current surgical techniques to repair the posterior wall of the frontal sinus and the cribriform plat

A case of traumatic thrombosis of the internal carotid artery at the base of the skull is reported, and 23 similar cases collected from the literature. This review shows that 85% of the patients either died or were left with major neurological deficit, and only 15% recovered completely. There were two primary sites for the onset of thrombosis. One in the petrous pyramid and usually associated with basal skull fracture, and the other immediately distal to the posterior communicating artery in patients with minor head trauma.

Jugular venography done to evaluate abnormalities at the base of the skull demonstrated three distinctly different patterns depending on whether there is occlusion, invasion, or growth with in the internal jugular vein. Improper technique results in a lack of intracranial dural sinus filling which may masquerade as venous occlusion. This problem is avoided by adequate neck compression along with proper volume and rate of delivery of contrast. Radiographically, an abnormal jugular vein at the base of the skull will show a concave defect in true occlusion, constriction or invasion of the vein by

PURPOSE: The brain lymphatic vessels (LV) drain fluids, metabolic waste, and immune cells. The abnormal accumulation of toxic substances causes neurodegenerative, cerebrovascular diseases, and brain tumors. Despite their intracranial importance, the precise extracranial exit routes of brain LVs remain poorly characterized. We evaluated the presence of LVs in the dural samples overlying the foramina/canals (superior orbital fissure, foramen rotundum, foramen ovale, Meckel's cave, internal acoustic meatus, foramen lacerum, jugular foramen, and hypoglossal canal) using immunohistochemistry.

BACKGROUND: Penetrating injuries involving foreign objects that transgress the entirety of the human skull present challenges in the management of vascular injuries, infectious risk, cerebrospinal fluid leak, and cosmetic repair. Ultimately, effective management of these injuries requires a multidisciplinary team and judicious surgical planning.
CASE DESCRIPTION: Here we present the treatment of a patient who experienced a penetrating injury from a railroad spike, with entry and exit points involving the left and right anterior aspects of the temporal fossa, respectively.
CONCLUSIONS: The ra

OBJECTIVES: Skull base meningiomas represent a distinct subgroup of intracranial tumours because of their proximity to cranial nerves, major vascular structures, and the brainstem. Although advances in imaging, neuroanaesthesia, and skull base approaches, marked improvement in illumination devices, provision of improved haemostatic agents, greater availability of more precise surgical instruments, and better modalities for skull base reconstruction have led to an inevitable evolution of safe skull base neurosurgery, complete resection remains technically demanding. This study evaluates operati…

Background: Sinonasal mucosal melanoma is a rare and aggressive malignancy arising from the nasal cavity and paranasal sinuses, characterized by high local recurrence rates and poor survival. Skull-base and orbital progression can occur rapidly, particularly when preoperative imaging underestimates local extension. This paper reports a case of sinonasal mucosal epithelioid melanoma with fulminant postoperative skull-base breach and orbital invasion, highlighting its clinical course, management challenges, and histopathological features. Methods: A 60-year-old woman with progressive unilateral

BACKGROUND: Traumatic skull base fractures carry a risk of cerebrospinal fluid (CSF) leak, predisposing patients to meningitis, neurological complications, and prolonged hospitalization. Predictive factors for posttraumatic CSF leak remain incompletely defined. The primary objective of this study is to identify clinical and radiological predictors of CSF leak among patients with traumatic skull base fractures and evaluate associated outcomes at a Level-1 trauma center.
METHODS: A retrospective cohort study was conducted at King Abdulaziz Medical City, Riyadh, including all patients with compu…

Preoperative planning for skull base meningioma surgery requires a detailed understanding of complex anatomical relationships and careful strategy development. Although three-dimensional (3D) simulation has been increasingly used for surgical planning, conventional simulations mainly visualize anatomical structures and rarely reproduce the sequential process of tumor removal. We developed a method to perform a near-complete digital 3D simulation of skull base meningioma surgery using a surgical simulation application. Preoperative imaging data in Digital Imaging and Communications in Medicine …

OBJECTIVES: Neurogenic symptoms in craniofacial fibrous dysplasia (FD) are typically caused by direct compression due to the lesion's expansile nature. However, in rare cases, atypical symptoms unrelated to direct compression and associated dural contrast enhancement have been reported. The aim of this study was to investigate the relationship between FD and adjacent dural contrast enhancement.
MATERIAL AND METHODS: This observational study included patients with FD localized to the skull base or calvarium. The location of the lesion, its axial dimensions, and the presence of destruction of t…
