Determination of Normal Values of the Basal Angle in the Era of Magnetic Resonance Imaging.

BACKGROUND: The flexion of the skull base (basal angle [BA]) is the inclination between the anterior fossae and the basilar ramp of the occipital and sphenoid bones. An increased BA, termed platybasia, is usually associated with basilar invagination. BA reference values in the magnetic resonance imaging (MRI) era and the diagnosis of platybasia are of clinical importance. The transnasal approach has been the surgical technique of choice to remove the odontoid process in cases of ventral brainstem compression in patients with platybasia. The knowledge of normal BA values has been influenced by

Reconstructive techniques in skull base surgery after resection of malignant lesions: a wide array of choices.

PURPOSE OF REVIEW: In this manuscript, we will provide a panorama of the current status of skull base reconstruction following endoscopic endonasal or open resection of malignant lesions, focusing on novel options and recent modifications of previously described techniques. RECENT FINDINGS: New developments in skull base reconstruction continue to arise, mostly as in the form of vascularized pedicled flaps that can be used following endoscopic and open resections of the skull base. SUMMARY: Resection of skull base tumors, often creating large dural defects that couple with extensive intradur

Sinonasal morbidity following endoscopic endonasal skull base surgery.

Open transcranial surgery has long served as the traditional approach for resecting tumors and other lesions in the skull base. However, endoscopic endonasal skull base surgery (EESBS) has emerged as a credible alternative. This paper provides insight on the sinonasal morbidity in patients undergoing EESBS. A literature review was performed by searches of MEDLINE database to provide further insight on sinonasal morbidity associated with EESBS, with a particular focus on published incidence rates and patterns of complication. We identified only articles that reported the incidence of sinonasal

The clivus: anatomy, normal variants and imaging pathology.

The clivus is one of the most central parts of the skull base. As diseases of the clivus evade clinical evaluation imaging plays a pivotal role in establishing a diagnosis. This article combines the description of anatomy and pathology with an extensive pictorial essay. Starting with the development and normal macroscopic anatomy of the clivus the reader is then introduced to a large variety of normal variations and developmental disorders some of them with clinical significance. Typical examples of non-neoplastic and neoplastic masses of the clivus and their differential diagnoses are provide

[Craniomaxillofacial fibrous dysplasias].

Thirty cranio-maxillofacial fibrous dysplasia patients were treated. There were 21 patients with monostotic and 9 patients with multiple lesions both jaws and bones of the skull. Most of the patients were under forty years of age. The lesions caused facial asymmetry in all patients, in two of them was visual disturbances and orbital dystopia, in one patient was hearing loss. The lesions were evaluated histologically that enabled appropriate diagnosis. The principles of surgical treatment were based on the zones of involvement. Treatment was aimed at correcting functional problems and achieving

Cryptic osteoid osteoma of the cranium: case report.

Osteoid osteoma is an uncommon benign tumor of bone that rarely involves the cranium. If often causes local pain that is worse at night, characteristically relieved by aspirin, and does not correlate with the size of the lesion. The diagnosis depends on skull radiographs and computed tomography. This report describes a case in which radiographs showed nothing abnormal. The diagnosis and operative localization were accomplished with radionuclide bone scanning. Thus, the possibility of radiographically cryptic lesions must be considered in the evaluation of atypical head pain syndromes.

[Aneurysmal bone cyst of the skull -a case report- (author's transl)].

Aneurysmal bone cyst of the skull is rarely seen. There are 29 reported cases in the literatures as far as we could collect. We reported an additional case of aneurysmal bone cyst of the skull which originated from the right parietal bone of 4-year-old boy. We have summarized these 29 cases. The age incidence in this series is from 14 month-old to 31 year-old. There are 11 cases under the age of 10. In most cases clinical symptoms are palpable mass or headache and exophthalmos. Eyeball displacement and proptosis are also the symptoms when this disease occurs at the orbital roof. According to t

[Enlarged transbasal approach by mobilization of the medial fronto-orbital ridge].

Tumors involving the base of the skull are best approached by the transbasal route as described by Derome. However this approach can be improved by mobilization of the medial part of the supra-orbital ridge according to the technical principles used in treatment of craniosynostosis. This allows a wide exposure of the ethmoido-sphenoidal area with minimal retraction of the frontal lobes. The axis of working becomes parallel and even inferior to the plane of the cribriform plate and gives a better access to the upper part of the clivus. Rhinologic complementary approach (rhinoseptal or transfaci

[3D imaging and pathology of the base of the skull in children].

Three dimensional computed tomography was performed in 150 children suffering from a bone lesion of the base of the skull and face. These lesions were either malformative, neoplastic, infectious or traumatic. 3D CT provided an essential anatomical approach to the lesions facilitating the choice of the most appropriate treatment. 3D CT is also indicated in the follow-up of these operated children, particularly of the growth of the operated bone segment and its relations with adjacent structures.

Endoscopic resection of esthesioneuroblastoma.

Esthesioneuroblastoma (olfactory neuroblastoma) is a rare malignant neoplasm originating from neuroepithelial cells. Treatment for this tumor has traditionally required a craniofacial resection through an external approach. The safety and efficacy of transnasal endoscopic resection is uknown. A series of five patients with Kadish stage A or B lesions of the anterior skull base were resected and immediately reconstructed endoscopically over a nine-year period. There were 2 males and 3 females with a mean age of 64 years. Three patients were treated primarily and two were treated secondarily for

[Endoscopic Anterior Transpetrosal Approach].

The anterior transpetrosal approach (ATPA) is the standard technique for treating lesions around the petrous apex. Conventional microscopic ATPA is associated with limited linear visibility and enhanced risks related to temporal lobe retraction. This article reviewed endoscopic ATPA (eATPA) and addressed some of the limitations. The eATPA utilizes a panoramic endoscopic view through a small craniotomy, significantly reducing the need for brain retraction and eliminating blind spots in the ventral brainstem. The procedure centers on precise extradural drilling of Kawase's triangle. The primary

Risk Factors for Postoperative Brain Contusion and Early Outcomes in Patients with Anterior Skull Base Meningiomas Undergoing Unifrontal Surgery: Single Center Experience with 110 Patients.

BACKGROUND: Though frontal lobe contusion is a major cause for morbidity and prolonged hospitalization following excision of anterior skull base meningiomas, there is only limited literature on this complication. This study aimed to find out the incidence of postoperative frontal lobe contusion, identify the risk factors for its development and its impact on early postoperative outcome. METHODS: Data from 110 patients who underwent excision of anterior skull base meningiomas through a unilateral supraorbital craniotomy from 2001 to 2018 were retrospectively analyzed. The risk factors analyzed

Two step approach for surgical removal of petroclival meningiomas with large supratentorial extension.

The treatment of petroclival meningiomas is still a matter of controversy in literature. In the last decades, many approaches have been introduced. Our strategy for the treatment of such tumors having large supratentorial extension with encasement of the internal carotid artery or compression of optic and oculomotor nerves has evolved in the attempt to improve the outcome. Currently, we favor a surgical technique consisting of two steps. As first step, we perform a retrosigmoid suprameatal approach in order to resect the posterior part of the tumor and obtain brainstem decompression. In the se
