Pulmonary adenocarcinoma mutation profile in smokers with smoking-related interstitial fibrosis.

Cigarette smoking is an established cause of lung cancer. However, pulmonary fibrosis is also an independent risk factor for the development of lung cancer. Smoking-related interstitial fibrosis (SRIF) has recently been reported. We hypothesized that adenocarcinomas in lungs with SRIF might show distinct molecular changes and examined the molecular phenotype of 168 resected lung adenocarcinomas in lungs with and without SRIF. The diagnosis of SRIF was determined by histological examination, based on the presence of alveolar septal thickening, due to pauci-inflamed, hyalinized, "ropy" collagen,

Randomized trial comparing mindfulness training for smokers to a matched control.

Smoking continues to take an enormous toll on society, and although most smokers would like to quit, most are unsuccessful using existing therapies. These findings call on researchers to develop and test therapies that provide higher rates of long-term smoking abstinence. We report results of a randomized controlled trial comparing a novel smoking cessation treatment using mindfulness training to a matched control based on the American Lung Association's Freedom From Smoking program. Data were collected on 175 low socioeconomic status smokers in 2011-2012 in a medium sized midwestern city. A s…

Cortical activity differs during nicotine deprivation versus satiation in heavy smokers.

RATIONALE: Research suggests that nicotine deprivation among smokers is associated with lesser resting cortical activity (i.e., greater power density in theta and alpha-1 EEG bands and lesser power in beta bands). These changes in cortical activity may be indicative of withdrawal-related cognitive deficits, yet the markers of differences in cortical activity are not well-established. OBJECTIVE: The objective of the study was to clarify the EEG frequency bands affected by nicotine deprivation and assess prospective moderators. METHOD: One hundred twenty-four heavy smokers visited the laborato…

Coronally Positioned Flap for Root Coverage: Comparison between Smokers and Nonsmokers.

BACKGROUND: Gingival recession is significantly more common among smokers, while cigarette smoking has been shown to negatively influence healing following periodontal therapeutic procedures as compared to non-smokers. The objective of this study was to evaluate the influence of cigarette smoking on the outcome of coronally positioned flap (CPF) in the treatment of Miller Class I gingival recession defects. MATERIALS AND METHODS: Ten current smokers (≥10 cigarettes daily for at least 5 years) and 10 non-smokers (never smokers), each with one 3 to 4-mm Miller Class I recession defect in an upp…

Hyperhomocysteinaemia predicts the decline in pulmonary function in healthy male smokers.

Hyperhomocysteinaemia is associated with chronic obstructive pulmonary disease. However, the relationship between plasma homocysteine levels and spirometric measures has not been investigated in a general population. We aimed to determine whether homocysteine levels are predictive for a rapid decline in lung function among healthy current smokers. Blood sampling and spirometry were performed on subjects participating in a community-based annual health check in Takahata, Japan, from 2004 to 2006 (n=3257). Spirometry was re-evaluated in 147 male current smokers in 2009. On initial assessment, fo…

Early detection of central airway lung cancer in smokers with silicosis.

BACKGROUND: Smokers with silicosis are at increased risk of lung cancer. OBJECTIVE: To evaluate the feasibility of using autofluorescence bronchoscopy after sputum examination for early detection of large airway lung cancer and factors associated with the presence of cancerous and pre-cancerous lesions among smokers with silicosis. METHODS: Subjects at the pneumoconiosis clinic were recruited if they fulfilled the following criteria: 1) age ≥40 years, 2) smoking history of ≥20 pack-years and 3) confirmed diagnosis of silicosis. Sputum specimens were collected for cytology/cytometry examinati…

Brain reactivity to emotional, neutral and cigarette-related stimuli in smokers.

Addiction has been described as the pathological usurpation of the neural mechanisms normally involved in emotional processing. Event-related potentials (ERPs) can provide a non-invasive index of neural responses associated with the processing of emotionally relevant stimuli and serve as a tool for examining temporal and spatial commonalities between the processing of intrinsically motivating stimuli and drug cues. Before beginning a smoking cessation program, 116 smokers participated in a laboratory session in which dense-array ERPs (129 sensors) were recorded during the presentation of pictu…

Association of secondhand smoke exposure with nicotine dependence among Black smokers.

INTRODUCTION: Exposure to secondhand smoke (SHS) is influenced by norms and regulations, socioeconomic status and immediate personal interactions. SHS exposure may occur in various settings, including the living space, workplace, and other social environments. This study examines the association between exposure to SHS and nicotine dependence among current smokers. METHODS: A cross-sectional sample of 246 Black (60% male and 40% female) current smokers age 40 and older, from Baltimore, Maryland and Washington, D.C, responded to an interviewer-administered questionnaire. We examined nicotine d…

Lung cancer in never smokers: clinical epidemiology and environmental risk factors.

More than 161,000 lung cancer deaths are projected to occur in the United States in 2008. Of these, an estimated 10 to 15% will be caused by factors other than active smoking, corresponding to 16,000 to 24,000 deaths annually. Thus lung cancer in never smokers would rank among the most common causes of cancer mortality in the United States if considered as a separate category. Slightly more than half of the lung cancers caused by factors other than active smoking occur in never smokers. As summarized in the accompanying article, lung cancers that occur in never smokers differ from those that o…

Comparison of CYP1A2 and NAT2 phenotypes between black and white smokers.

The lower incidence rate of transitional cell carcinoma of the urinary bladder in blacks than in whites may be due to racial differences in the catalytic activity of enzymes that metabolize carcinogenic arylamines in tobacco smoke. To examine this, we compared cytochrome P4501A2 (CYP1A2) and N-acetyltransferase-2 activities (NAT2) in black and white smokers using urinary caffeine metabolites as a probe for enzyme activity in a community-based study of 165 black and 183 white cigarette smokers. The paraxanthine (1,7-dimethylxanthine, 17X)/caffeine (trimethylxanthine, 137X) ratio or [17X+1,7-dim…

Adolescent smokers rate delayed rewards as less certain than adolescent nonsmokers.

This research compared adolescent smokers (n=45) and nonsmokers (n=35) on ratings of certainty about receiving delayed rewards during a delay discounting procedure. Consistent with a previous finding [Patak, M., Reynolds, B., 2007. Question-based assessments of delay discounting: do respondents spontaneously incorporate uncertainty into their valuations for delayed rewards? Addict. Behav. 32, 351-357] participants generally rated the delayed rewards as increasingly uncertain with longer delays, and ratings of certainty were correlated with delay discounting (r=.37). Also, the adolescent smoker…

A process evaluation model for patient education programs for pregnant smokers.

OBJECTIVE: To describe and apply a process evaluation model (PEM) for patient education programs for pregnant smokers. METHODS: The preparation of a process evaluation plan required each program to define its essential "new" patient assessment and intervention procedures for each episode (visit) of patient-staff contact. Following specification of these core implementation procedures (p) by each patient education program, the PEM, developed by the Smoke-Free Families (SFF) National Program Office, was applied. The PEM consists of five steps: (1) definition of the eligible patient sample (a); …
