Tethered spinal cord in association with diastematomyelia.

There is no good evidence that division of the filum terminale with simultaneous removal of the central spur in patients with diastematomyelia improves their eventual neurological status. We describe 5 patients in whom the filum was not divided and who were later reported upon because of lack of improvement or because of increasing neurological deficit. In 3 patients the division of a congenitally short filum terminale resulted in no improvement; in the other 2, symptoms were relieved following the division of adhesions at the site of previous surgery.

[Thermography in the diagnosis of spinal cord tumors].

Clinical studies of 22 patients with pathological processes in the spinal cord and vertebral column allowed a new method of diagnosing spinal tumors with the aid thermography on a Soviet thermovision "Rubin" set. According to the preliminary data this method facilitates the diagnosis and differentiation between tumors of the spinal cord. The paper contains thermograms of the back in normals and patients with extramedullar tumors of the spinal cord at the vertebral level D5--D6 and thermograms of the lower extremities in this pathological condition.

Hypercalcemia in children with spinal cord injury.

Eighty-seven children less than 16 years of age with recent spinal cord injury were studied during a 7-year period (1970--1976). Serum calcium concentration was determined at least once in each of 76 of these patients, and in 18 (23.6%) it was above 11 mg%. Fifteen of the 18 patients had quadriplegia and 3 had paraplegia. In 5 patients, the first symptoms of acute hypercalcemia simulated those of an acute abdomen. Two of the 5 patients underwent exploratory laparotomy, with negative findings. The incidence of urinary stones was 55% (10 of the 18), a difference from the control population of on…

Spinal cord injury in the newborn infant.

Birth injury to the spinal cord secondary to traction forces during delivery is a common but frequently undiagnosed disorder. The injury usually affects the cervicodorsal junction, with both extradural hematoma and direct cord damage at that level. Clinical findings of a paraplegic infant with abdominal breathing are sometimes obscured by secondary pneumonia and/or hypoxia. Radiologic manifestations include a bell shaped chest indicative of loss of the external muscles of respiration; spinal roentgenograms are usually normal. Myelography in neonatal spinal injury demonstrates a block in the su…

Transcutaneous Spinal Stimulation Modulates Spinal Reflex Circuit Excitability in Persons with Spinal Cord Injury.

Background: Transcutaneous spinal stimulation (TSS) is a noninvasive stimulation approach to modulate spinal reflex circuit excitability after spinal cord injury (SCI) Posterior root muscle (PRM) reflexes can be used to characterize the change in excitability of spinal reflex circuits after TSS; these responses are likely influenced by stimulus parameters. Methods: We compared PRM reflex responses to 3 TSS conditions: single-site continuous (SS-CONT), single-site burst (SS-BURST), and dual-site continuous (DS-CONT). Stimulation (frequency: 50 Hz, intensity: 80% soleus reflex threshold[RT]) was…
