Iatrogenic Spinal Cord Injury in a Trauma Patient with Ankylosing Spondylitis.

BACKGROUND: The recommended practice for over 30 years has been to routinely immobilize patients with unstable cervical spinal injuries using cervical spinal collars. It is shown that patients with Ankylosing spondylitis (AS) are four times more likely to suffer a spinal fracture compared to the general population and have an eleven-fold greater risk of spinal cord injury. Current protocols of spinal immobilization were responsible for secondary neurologic deterioration in some of these patients. OBJECTIVE: To describe an iatrogenic injury resulting from the use of a rigid spinal board and ad

Nicotiana glauca-induced congenital deformities in calves: clinical and pathologic aspects.

Seven calves born to 7 cows fed Nicotiana glauca during portions of the 1st trimester of gestation were deformed at birth. Deformities increased in severity as the calves aged. At birth, calves typically had arthrogryposis of the forelimbs or curvature of the spine; severity varied among calves. In 4 calves necropsied at about 15 months of age, there was general malpositioning and misalignment of the distal ends of the radius and ulna and the proximal ends of the metacarpal bones. Carpal joints were severely affected, fetlock joints were moderately affected, and pastern joints were slightly af

Spinal deformity in a sandtiger shark, Carcharias taurus Rafinesque: a clinical-pathological study.

The present study investigated spinal deformity in a captive sandtiger shark and describes the clinical and histopathological features of the lesion. Clinically, the shark presented a marked spinal curvature in the region between the pectoral girdle and cranial dorsal fin. Radiographic studies revealed scoliosis and kyphosis of the vertebral column with a modification of the basic structure of five vertebrae. Post-mortem examination confirmed the results of radiographic studies and histological examination showed remodelling and cartilaginous proliferation of the vertebral bodies together with

Curves of the achondroplastic spine: a new hypothesis.

The profile of the achondroplastic dwarf reflects abnormalities in the shape of the spine, particularly in the lumbosacral area. We hypothesize that these abnormalities and the resultant symptoms are secondary and avoidable rather than heritable and inevitable. Because the heads of achondroplastic children are enlarged (n = 50), loading of the vertical spine is increased. Unusually lax ligaments and misshapen vertebrae predispose the spine to yielding under vertical stress. Children and adults uniformly have hip flexion contractures (n 105); these are hypothesized to be the result of uncorrect

[Microsurgery of acquired degenerative lumbar spinal stenosis. Bilateral over-the-top decompression through unilateral approach].

Degenerative lumbar spinal stenosis is diagnosed with an increasing incidence. Clinical and radiological signs of progression often result in a significant decrease in the patient's quality of life. The indication for surgical treatment follows the spectrum of clinical symptoms. In cases with "structural" spinal stenosis and predominant "leg symptoms" such as neurogenic claudication and/or radicular symptoms, decompressive types of surgery are indicated. However, due to the multimorbidity of the mostly elderly patients, the surgical risk should be as low as possible whereas the surgical effica

Factors associated with rod fracture following surgery for adult spinal deformity: a single-center retrospective study.

PURPOSE: Adult spinal deformity (ASD) has a significant impact on trunk balance and quality of life in the elderly. Postoperative rod fractures pose significant challenges, but the mechanisms of their occurrence are underexplored compared to other complications such as proximal junctional kyphosis. This study investigated factors associated with rod fracture in patients with ASD. METHODS: A retrospective single-center study analyzed 110 adult patients who underwent spinal deformity correction between 2012 and 2020. Comparative analysis and univariate and multivariate Cox regression analyses w

The efficacy of combined physiotherapeutic scoliosis-specific exercises and manual therapy in adolescent idiopathic scoliosis.

BACKGROUND: Adolescent idiopathic scoliosis (AIS) is a pathological condition characterized by vertebral curvature and associated trunk deformities in adolescents. The clinical efficacy of conservative treatment in alleviating spinal curvature of AIS remains a topic of ongoing debate. The objective of this study was to investigate the impact of combined physiotherapeutic scoliosis-specific exercises (PSSE) and manual therapy (MT) on trunk deformity, spinal function, mobility, and mental health in patients with AIS. METHODS: Thirty-one participants who were diagnosed with AIS whose Cobb angle

Relationship of total sagittal spinal alignment index of thoracic kyphosis and lumbar lordosis with physical function in community-dwelling older adults.

BACKGROUND: Overall spinal curvature is evaluated by calculating the difference between the angles of lumbar lordosis (LL) and thoracic kyphosis (TK) and is expressed as LL minus TK (LL-TK). It is unclear whether LL-TK is associated with physical function in community-dwelling older adults and whether it is more relevant than TK or LL alone. OBJECTIVE: This study aimed to identify whether LL-TK is associated with physical function in community-dwelling older adults, and whether it is strongly associated than TK or LL alone. METHODS: The participants comprised 1,674 community-dwelling older a

Spine kinematics during gait in paediatric Hereditary Spastic Paraparesis.

BACKGROUND: Many studies already addressed specific gait abnormalities in children affected by Hereditary Spastic Paraparesis (HSP). Some authors investigated the contribution of the upper body to walking pattern, but simplifying trunk and pelvis as two hinged rigid bodies. Recently, we developed a method to detail spinal kinematics in terms of anatomic curvatures and length; we were thus interested in applying such protocol to HSP. RESEARCH QUESTION: how HSP influences spinal kinematics during gait? METHODS: we enrolled ten HSP patients (5-17 years, 8 males) and twelve Healthy Children (HC,

Characterizing spinal curves: addressing variability beyond identical cobb angles.

Traditional methods for evaluating spinal curves, such as the Cobb angle, are limited in their ability to capture the full complexity of spinal deformities. This manuscript presents a novel method that builds on Cobb's principle and Ferguson's approach to provide a more comprehensive assessment of spinal curves. The method involves identifying the centroid-based endpoints of the curve to define its "radius of curvature" and calculating the true curve length through a three-point measurement, including the apical vertebra. The curve's type and sharpness are determined using ratios of distances

Does Preoperative Spinal Alignment Influence Surgical Outcomes When Postoperative Alignment and Fixation Strategies Are Matched in ASD Correction?

STUDY DESIGN: Retrospective study. OBJECTIVE: To evaluate whether the extent of correction influences outcomes following adult spinal deformity (ASD) surgery using a matched-pair analysis. SUMMARY OF BACKGROUND DATA: Alignment targets are often based on achieving absolute values. However, the amount of correction may also influence outcomes. Existing studies are limited by methodological flaws, including multicollinearity and oversimplified modeling of correction. METHODS: We included ASD patients who underwent fusion (≥5 levels) with a UIV between T1 and L1 at a single center (2013-2023),

Pedicle Screw Placement in Pediatric and Adolescent Spinal Deformity Surgery: Does Tapping of the Pedicle Screw Tract Increase Safety?

INTRODUCTION: Since the development of pedicle screw fixation in the spine, safe placement has remained a crucial component in maximizing patient outcomes and mitigating pedicle screw-related complications. The purpose of this study is to investigate the utility of pedicle tapping in identifying pedicle breaches. METHODS: A pediatric spine surgery database was queried to identify a consecutive series of patients who underwent spinal deformity surgery utilizing pedicle screw fixation between May 2019 and October 2022. A "breached pedicle" was defined as a pedicle with intact walls after initia

An analysis of the usage and limitations of the T1 pelvic angle.

AIMS: The T1 pelvic angle (T1PA) provides a consistent global measure of sagittal alignment independent of compensatory mechanisms and positional changes. However, it may not explicitly reflect alignment goals that correlate with a lower risk of complications. This study assessed the value of T1PA in achieving sagittal alignment goals in patients with an adult spinal deformity (ASD). METHODS: Patients aged ≥ 18 years who had undergone surgery for ASD and had complete baseline data and at least two-year postoperative, radiological, and health-related quality of life follow-up were included. Lower lumbar spine significantly affects the dynamics of pelvic incidence after corrective surgery for adult spinal deformity.

PURPOSE: Pelvic incidence (PI) is a fundamental parameter influencing sagittal spinal alignment. Recent studies indicate that PI is not a fixed value and may change after surgery for adult spinal deformity (ASD). However, research on this topic is very limited, and no consensus has been reached. The aim of this study is to assess postoperative PI changes in ASD patients and identify the factors influencing these changes with long-term follow-up. METHODS: We reviewed 65 ASD patients who underwent corrective surgery with S2-alar-iliac screws and pedicle subtraction osteotomy. Radiographic param

Robotic-assisted versus free-hand techniques in adult spinal deformity surgery: a comparative analysis of postoperative outcomes.

Adult spinal deformity (ASD) represents a challenging area in spinal surgery due to its complexity and potential for postoperative complications. While robotic systems have promised enhanced precision for pedicle screw placement, improved alignment, and potentially fewer complications, the comparative efficacy and morbidity of robotic-assisted (RA) versus free-hand (FH) techniques remains underexplored in the context of ASD. This study aims to compare postoperative outcomes in patients undergoing RA techniques versus FH surgery to manage ASD. A retrospective review was performed and included p

Preservation of Distal Mobile Segments Reduces Risk of Rod Breakage in Adult Spinal Deformity Surgery.

STUDY DESIGN: Retrospective multicenter observational study. OBJECTIVE: To analyze whether avoiding fusion to the pelvis and preserving distal mobile segments reduces the risk of rod breakage (RB) after adult spinal deformity (ASD) surgery. SUMMARY OF BACKGROUND DATA: RB is a major complication in ASD surgery, mostly associated with age, malalignment, and pelvic fixation. However, the impact of different lower instrumented vertebrae (LIV) possibilities as an independent risk factor remains unclear. METHODS: We included ASD patients (aged 50-75) who underwent fusion of more than six levels,
