Injuries of the cervical spine are re-evaluated at present. New surgical procedures and research on the stability, when these procedures are used, caused that surgical treatment is used more frequently. Attention is draw to provisions in the field as well as in hospitals, on the diagnostic approach and classification of these injuries, on the procedure of conservative surgical treatment. The domain of conservative treatment are injuries of the upper cervical spine (C1 and C2); injuries of the lower cervical spine are frequently unstable, and therefore frequently indicated for operation. Attent…

Out of 144 patients suffering from cervical osteochondrosis, single or multiple foci of chronic infection (ENT infection, bronchitis, pneumonia, cholecystitis, pyelonephritis, ect.) were found in 99. All of them had various combinations of symptoms indicating lesions of the radices spinales, cervical spine, insufficient circulation in the vertebrobasilar area. Literature and first-hand experience suggest a conclusion on the underlying infection and allergic processes in development of the disk ossification. Relevant treatment modalities are recommended.

The authors report 10 cases of spontaneous pyogenic spinal osteomyelitis encountered within a 3-year period. There were six women and four men, ranging in age from 60 to 84 years. Six cases occurred at the thoracic level, three at the lumbar level, and one in the cervical spine. No patient was diabetic, immunocompromised, or receiving steroid therapy, and none had a history of endocarditis or intravenous drug abuse. No patient had undergone previous spinal surgery. There were no instances of coexisting tuberculosis or malignancy. Contemporaneous cases with known predisposing factors have been …

Pathology affecting the spinal cord structures and/or vertebral segments at the TLJ is not uncommon. Trauma is the most frequent cause of dysfunction-compression at these levels and may result in permanent neurological disability and vertebral column instability. Patients with TLJ pathology must be evaluated carefully and thoroughly with serial neurological examinations and diagnostic radiographic studies as outlined. The treatment of patients who are documented to harbor TLJ pathology must be individualized and is determined by the type, extent, and level of the lesion, the presence of disloc

Spinal instrumentation surgery has come into wide use recently, especially for tumor of the spine. It is very effective for fixing the spine for relieving the patient from pain. But the decision about the indication of instrumentation surgery for tumor of the spine must be made carefully. In cases of double malignant tumors including spinal tumor, it is hard to decide how to treat tumor of the spine. We report a case of 78-year-old male with multiple myeloma of the cervical spine and cancer of the prostate. The patient had been in good health until one year previously, when he started to compl…

From 1975 to 1986, 2435 patients were admitted to the Northwestern University-Midwest Regional Spinal Cord Injury Unit. Of these, 220 patients (9.0%) had documented neck fractures from diving accidents, representing the largest series of acute diving injuries yet analyzed. The average age of these patients was 21 years, and males predominated. Two hundred twelve patients (96.4%) were admitted within 48 hours of injury. Associated injuries were rare: none had intracranial mass lesions or systemic injuries, and only nine were near-drowning victims who required endotracheal intubation. The most c

Functional methods utilising the effects of gravity on the upright lordotic spine were developed for both static and dynamic radiography. Static standing orthoradiography of the lumbar spine and weight-bearing joints of the lower extremities shows constitutional and functional abnormalities not visible in conventional recumbent radiography, e.g., postural scoliosis, hypo- or hyperlordosis, "kissing spine"-syndrome and signs of segmental instability. In dynamic traction-compression radiography segmental instability is provoked by axial traction and compression of the spine. With this method tra…

Recent advances and the technical options that have lately become available in the surgical treatment of spinal injuries mean that clear definition of the indications is essential. Precise observation and classification of each injury to the cervical spine are of paramount importance for this purpose, to allow the comparison of proposed methods of treatment and of the results obtained with them. There are a number of classification systems for injuries to the cervical spine, none of which, however, is generally accepted. These classification systems are based on different criteria, such as nat…

Four cases of cervical spine growth plate ("ring apophysis") injury are described. Two nonfatal injuries involved anterior displacement of a portion of the inferior physis and epiphyseal ossification center in the lower cervical spine. Neither injury was associated with neurological damage. Follow-up roentgenograms demonstrated healing, but with a spur-like prominence in one patient. A third patient sustained a fatal separation through the inferior end plate of C2. The fourth patient sustained a complete separation of C6 from C7. These injuries are correlated with pathologic specimens and norm…

The erect posture of man is not due to a static rigidity of the vertebral column, but to an extremely dynamic function of the whole locomoto … anging the small lumbar vertebral joints, beginning with the transition between thoracic and lumbar spine. In many cases this adaption is not finished and remains incomplete in the region of transition between lumbar and sacral spine. Only the erect posture has released the hands and made possible the use of tools and the evolutionary progress of the br …

The use of any implant system to promote arthrodesis of any segment of the spine is only as good as the technique of surgical arthrodesis. The success of the implant depends, in short, on the success of the arthrodesis. A successful arthrodesis depends on meticulous soft-tissue debridement, decortication, and copious iliac bone grafting or allograft bone grafting if autogenous graft is deficient. If the anterior approach is used, complete and thorough debridement of the intervertebral disc and cartilaginous end-plates with meticulous bone grafting of the interspace is essential for a successfu…

Metastatic bone disease in 322 patients was analyzed to assess the frequency and behavior of disseminated carcinoma to the vertebral column. Breast, lung, and prostate neoplasms were the most frequent tumors of origin in the 55% of patients who had vertebral lesions. The lumbar spine was the site of the greatest number of metastases. Back pain did not occur in 36% of the 179 patients with spinal disease. Cord compression occurred in 20% of the patients with vertebral involvement, and prostate tumors were the most frequent neoplasm to cause epidural spinal cord impingement. Hypernephroma was th…

Sonography has proved to be a valuable method for the examination of the spine in children with spinal dysraphism. It facilitates the diagnosis of meningocele, lipoma, and/or spinal cord tethering rapidly and noninvasively. It is suggested that sonography be utilized for screening infants with neurocutaneous signature of spinal dysraphism.

Little experience of computed tomography (CT) evaluation of spinal infection has been reported. We collected 20 patients with spinal infection studied with CT. Fifteen patients had pyogenic infections, four had tubercular lesions, and one infection was fungal. Predisposing factors for spinal infection were present in ten of 20 patients. Metrizamide was injected into the subarachnoid space prior to CT in nine of 20 patients. CT detected bony destruction in all patients with osseous involvement. In addition, CT diagnosed prevertebral abscess formation and/or extension of the process into the spi
