Strongyloidiasis is one of the major intestinal infections in humans, and a neglected tropical disease whose diagnosis still poses a challenge. We hypothesized that diagnostic tests based on short peptides containing major epitopes may represent a promising strategy to improve strongyloidiasis detection due to reduced cross-reactivity and higher sensitivity. Our aim was to evaluate two synthetic peptides selected by phage display (C10 and D3) as potential tools for serodiagnosis of strongyloidiasis, and to predict their putative antigen target. To investigate their diagnostic potential, we hav…

Pulmonary strongyloidiasis is an uncommon presentation of Strongyloides infection, usually seen in immunocompromised hosts. The manifestations are similar to that of acute exacerbation of chronic obstructive pulmonary disease (COPD). Therefore, the diagnosis of pulmonary strongyloidiasis could be challenging in a COPD patient, unless a high index of suspicion is maintained. Here, we present a case of Strongyloides hyperinfection in a COPD patient mimicking acute exacerbation, who was on chronic steroid therapy.

BACKGROUND: Strongyloides stercoralis is a parasitic nematode found in humans, with a higher prevalence in tropical and sub-tropical regions worldwide. If untreated, the infection can progress to disseminated strongyloidiasis, a critical illness which may be fatal.
OBJECTIVE: To provide clinical guidance on the prevention, assessment and management of disseminated strongyloidiasis.
METHODS: A literature review was conducted to evaluate the current evidence and to identify any systematic reviews, case reports, guidelines and peer reviewed and non-peer reviewed medical literature. The Committe…

We compared the efficacy and applicability of a modified formalin-ether concentration technique (M-FECT) to the conventional FECT (C-FECT) a … of M-FECT has become comparable to APC and was much better than that of C-FECT for the diagnosis of strongyloidiasis.

IgG avidity assays have been developed for several parasitic diseases although there are no researches focused in strongyloidiasis diagnosis. Definitive diagnosis of strongyloidiasis is based on the presence of Strongyloides larvae in stool, but majority of cases involve low and irregular larval output. While limitations of serological assays for strongyloidiasis are well known, characteristics of persons who are misdiagnosed based on negative coproparasitological tests have been little explored. The aim of the present study was to evaluate the use of IgG avidity to detect patients with active…

We report a 49-year-old man who was a human T-cell leukemia virus type 1 (HTLV-1) carrier, born in Okinawa prefecture where both strongyloidiasis and HTLV-1 are endemic. He presented with fever, headache and urinary retention. On the basis of CSF examination and MRI findings, his condition was diagnosed as myelitis. He received methylprednisolone pulse therapy. He was transferred to our hospital due to severe paralytic ileus. Strongyloides stercoralis (S. stercoralis) was found in the duodenal stained tissue of a biopsy specimen. Ivermectin applied both orally and through enema were ineffectiv…

Hyperinfection with strongyloides stercolis occurs in the setting of chronic strongyloides infection in conjunction with immune suppression. Although malnutrition remains the major secondary cause worldwide in the developed world, iatrogenic immune suppression is an important precipitant. Autologous stem cell transplantation recipients are significantly immunocompromised albeit temporarily. Despite the increasing use of haemopoetic stem cell transplantation, hyperinfection with strongyloides has rarely been reported. We describe two cases of patients transplanted with chronic strongyloidiasis.…

A retrospective study of laboratory records in a 400-bed university hospital in Campinas city, SP (Southeastern Brazil) suggests that infection by Strongyloides stercoralis is widespread in the region. A prevalence of 10.81% was found in 37,621 stool specimens examined in a two-year period. Parasite-specific IgG antibodies were measured by ELISA in sera from 90 patients with strongyloidiasis. The ELISA detected antibody in 76 (84.44%) patients, indicating that serodiagnostic tests may be helpful in screening patients for strongyloidiasis.

An indirect agglutination test using recently developed gelatin particles was assessed to determine its applicability as a screening test for mass examination for strongyloidiasis. 1199 individuals in Sashiki Town, Okinawa Island, were screened by the test and 34.7% were determined to be antibody positive. Follow-up examination of the persons whose sera showed positive antibody responses demonstrated the presence of faecal larvae in 41.7%. The calculated infection rate (14.5%) was similar (14.1%) to that indicated by another survey using the micro-enzyme-linked immunosorbent assay (micro-ELISA…

Total IgE, specific IgE and IgG antibodies directed against Strongyloides stercoralis antigens were measured in the serum of 8 asymptomatic individuals with parasitologically proven strongyloidiasis, 10 patients with symptomatic strongyloidiasis and 8 with a severe form of the disease. No correlation was found between any of the immunological quantities evaluated and the clinical form of strongyloidiasis. We suggest that other responses may be involved in determining clinical manifestations of this parasitosis.

Antibodies to intact rhabditiform and filariform larvae of Strongyloides stercoralis were measured by indirect immunofluorescence using quan … unts of parasite-specific IgG. We conclude that the majority of patients with chronic uncomplicated strongyloidiasis develop specific IgG antibodies directed against the surface of filariform larvae, the invasive stage of the parasite. such antibodies may be involved in host defenses that prevent fatal hyperinfection in immunocompetent individuals. In addition, ou …

Total IgE serum levels were determined by the radioimmunosorbent method (RIST) in 18 patients with strongyloidiasis presenting mild or asymptomatic disease. Results showed elevated IgE serum levels in 7 out of 18 patients (38.8%), ranging from 150 to 3600 ng/ml with a mean concentration of 973 ng/ml.

Strongyloides stercoralis hyperinfection carries near-universal mortality if untreated. We present a 52-year-old man from Central America with advanced HIV (CD4 196 cells/μL) who presented with chronic diarrhea, vomiting, and weight loss. Cross-sectional imaging revealed small bowel thickening. Endoscopy demonstrated gastric and duodenal mucosal friability and ulceration. His course was complicated by polymicrobial Gram-negative bacteremia and meningitis. Retrospective identification of antecedent eosinophilia prompted suspicion for Strongyloides, confirmed on intestinal biopsies. The patient

INTRODUCTION: Strongyloidiasis, caused by the soil-transmitted helminth Strongyloides stercoralis, remains a neglected public health issue in Australia, particularly among remote Aboriginal and Torres Strait Islander communities. This study aimed to map the spatial distribution of strongyloidiasis and investigate associated socioecological factors to identify high-risk areas and guide targeted interventions in Australia.
METHODS: We used data from a previous nationwide pathology data survey conducted between 2012 and 2016, which included 81,131 individuals across 332 statistical area level 3 …
