Benign communicating hydrocephalus in children.

A review was performed of the computed tomograms (CTs) of 500 children which had been reported as showing widening of the supratentorial subarachnoid spaces with normal cerebral substance. On the basis of this a radiological diagnosis of cerebral atrophy had been made in all but five, who were said to have megalencephaly. From these, the children with large or abnormally enlarging heads, but normal or only slightly enlarged ventricles, were selected; there were 40 such cases (8%). The clinical condition either improved or remained stable over a period of 2 years; in the majority the scan abnor…

Subarachnoid hemorrhage as the initial presentation of cerebral venous thrombosis.

Cerebral venous thrombosis presenting as subarachnoid hemorrhage (SAH) is very rare. We present a woman with thrombosis of the superior sagi … nsion with dilatation of the thin, fragile-walled cortical veins which eventually ruptured into the subarachnoid space.

Spontaneous occlusion of ruptured vertebral artery dissection at the extradural fenestration associated with extradural origin of the posterior inferior cerebellar artery--case report.

A 69-year-old female suffered from sudden onset of severe headache. Computed tomography showed subarachnoid hemorrhage primarily located in the posterior fossa. Initial angiography demonstrated a fenestration of the vertebral artery and an extracranial origin of the posterior inferior cerebellar artery. However, no bleeding points could be clearly detected. The operative findings revealed a massive clot in subarachnoid space, but no bleeding point. Serial angiography demonstrated dissection in one of the limbs of the fenestrated vertebral artery on the 25th day after the onset. On the 100th da…

Mortality in Parenchymal and Subarachnoid Neurocysticercosis.

Neurocysticercosis (NCC) is endemic in many parts of the world, carrying significant neurological morbidity that varies according to whether lesions are located inside the cerebral parenchyma or in extraparenchymal spaces. The latter, in particular subarachnoid NCC, is assumed to be more severe, but no controlled studies comparing mortality between types of NCC exist. The aim of this study was to compare all-cause mortality between patients with intraparenchymal NCC and those with subarachnoid NCC. Vital status and sociodemographic characteristics were evaluated in patients with intraparenchym

A modified technique for syringo-subarachnoid shunt for treatment of syringomyelia.

Syringo-subarachnoid shunting is a well-established procedure for the treatment of syringomyelia. However, the standard surgical procedure r … thick catheter into the syrinx and placement of the distal tip of the catheter in the anterolateral subarachnoid space. Using this technique there was no operative morbidity or mortality. Collapse of the syrinx, in the first post-operative month, was demonstrated by MRI in all cases. There were no relapses in the follow-up period. Although our experience is limited …

[Subarachnoid hemorrhage].

INTRODUCTION: The vascular structures in the central nervous system has a particular distribution in respect of other organs. This anatomic configuration and other conditions predispose this arteries and veins to bleed frequently in the subarachnoid space, resulting in a syndrome known as subarachnoid hemorrhage. DEVELOPMENT: We make a study of the bibliography published in the topic, beside our professional criteria concerning those themes, with regard to the elements that we have been able to evaluate in our daily practice. We carried out a wide review about etiology, physiopathology, clini…

Chronic post-traumatic erosion of the skull base.

Delayed post-traumatic erosion of the skull base is reported in three patients who presented as adults with cerebrospinal fluid fistulae and a history of recurrent meningitis. These skull defects were associated with herniation of the subarachnoid space into the diploe of the skull base, the paranasal sinuses and the orbit. This rare complication of head injury is assumed to have occurred as the result of a dural tear at the time of trauma. Its site probably determines whether a resulting meningocele widens the intradiploic space or broaches the cranial floor.

Optimal time for subarachnoid transplantation of neural progenitor cells in the treatment of contusive spinal cord injury.

This study aimed to identify the optimal neural progenitor cell transplantation time for spinal cord injury in rats via the subarachnoid space. Cultured neural progenitor cells from 14-day embryonic rats, constitutively expressing enhanced green fluorescence protein, or media alone, were injected into the subarachnoid space of adult rats at 1 hour (acute stage), 7 days (subacute stage) and 28 days (chronic stage) after contusive spinal cord injury. Results showed that grafted neural progenitor cells migrated and aggregated around the blood vessels of the injured region, and infiltrated the spi…

Intrasellar arachnoid cysts.

OBJECTIVE: To evaluate the clinical, endocrinological, and radiological presentation of nine cases of surgically verified intrasellar arachn … der mild pressure. In three patients, the contrast infiltrated along the pituitary stalk toward the subarachnoid spaces; in the other patients, it remained in the intrasellar compartment. Cyst membranes were removed as completely as possible with fenestration toward the subarachnoid spaces in communicating cysts. In spite of tight packing of the sella and sphenoid s …

[Suspected spontaneous subarachnoid hemorrhage: what diagnostic approach?].

Subarachnoid hemorrhage (SAH) is defined as sudden bleeding into the subarachnoid space. Although its incidence is low, mortality remains high. The most frequent cause of spontaneous SAH is aneurysm rupture. Cerebral CT scans are highly sensitive in ruling out SAH within the first 6 hours. Due to the recent improvement in imaging resolution, only a strong clinical suspicion can justify a cerebrospinal fluid analysis if the CT scan is normal after 6 hours. Cerebral MRI is also highly sensitive in both the acute and sub-acute phases. This article reviews the various clinical and paraclinical ele…

Histopathology of Endolymphatic subarachnoid shunt surgery for Meniere's disease.

The histopathological findings in the temporal bones of three patients who underwent endolymphatic subarachnoid shunt operation for Meniere's disease are reported. The postoperative follow-up period varied from three to five years. Two patients had a successful operation; the third was unsuccessful. The continuity between th mesothelial lining of the endolymphatic sac and the subarachnoid space was demonstrated in one case; no definite conclusions could be made for the other two cases. Although failure of shunt surgery was presumably due to fibrosis in the shunt area in the reported case, the …

Subarachnoidal pleural fistula after resection of intradural thoracic disc herniation and multimodal treatment with noninvasive positive pressure ventilation (NPPV).

Subarachnoid pleural fistula (SPF) is a type of cerebrospinal fluid (CSF) fistula that can arise as a complication following transthoracic resection of intervertebral disc herniation in the thoracic spine. It is an abnormal communication between the subarachnoid and pleural space. Negative intrapleural pressure promotes CSF leak due to a suction effect into the pleural cavity, with little chance of spontaneous closure. Due to the risk of severe complications with CSF leak into the thoracic cavity, early diagnosis and treatment are mandatory. However, management can be challenging. We report a

Intracranial extravasation of contrast medium during carotid angiography.

The results of 5184 carotid angiograms were reviewed and five cases of intracranial extravasation of contrast medium were found. In two cases an aneurysm ruptured during angiography and extravasation into the subarachnoid space was seen. In one case medium leaked from an angiomatous malformation into the subdural space. The fourth case was a patient who had suffered closed head trauma. Here extravasation was seen deep within the brain. The fifth case illustrates two forms which extravasation from meningeal arteries may take. The incidence and prognostic significance of this finding in differen…

Ion changes in spreading ischaemia induce rat middle cerebral artery constriction in the absence of NO.

In rats, cortical spreading hyperaemia is coupled to a spreading neuroglial depolarization wave (spreading depression) under physiological conditions, whereas cortical spreading ischaemia is coupled to it if red blood cell products are present in the subarachnoid space. Spreading ischaemia has been proposed as the pathophysiological correlate of the widespread cortical infarcts abundantly found in autopsy studies of patients with subarachnoid haemorrhage. The purpose of the present study was to investigate whether the extracellular ion changes associated with the depolarization wave may cause …
