Changes in illicit, licit and stimulant designer drug use patterns in South-East Hungary between 2008 and 2015.

The aim of this work is to present the changes in classical illicit and licit drug, as well as stimulant designer drug (SDD) consumption of suspected drug users in South-East Hungary between 2008 and 2015. Urine and/or blood samples of 2976 subjects were analyzed for these groups of substances of which 1777 (59.7%) were tested positive. THC was the most frequent (32.2%) substance, followed by classical stimulants (amphetamine, metamphetamine, MDMA, cocain) (21.4%), SDDs (17.0%), benzodiazepines (15.5%), medical opiates (codeine without morphine, methadone, tramadol) (4.03%), and morphine with

Management of the pregnant substance abusing woman.

Operation PAR has been involved in prevention and treatment service development for drug-dependent women for nearly 40 years. Management starts with prevention programs. For opiate-addicted pregnant women, methadone is the treatment of choice and should be maintained throughout pregnancy, despite the manageable problem of neonatal opiate withdrawal. Women are actively engaged in innovative educational and therapeutic activities to help moms build skills to enhance their personal health and develop recovery orientation and drug refusal skills. Specific parenting skills are modeled and taught bo

Analysis of drugs of abuse in wastewater from two Canadian cities.

Several drugs of abuse, including amphetamines, cocaine and its metabolite, benzoylecgonine and several opioid prescription drugs were detected in wastewater from two Canadian cities, a small community (75,000 population) and a large urban center (1.6 million population). The objective of this study was to evaluate community use of these drugs in two cities with large differences in population size and demographics. In addition, we evaluated the use of the Polar Organic Chemical Integrative Sampler (POCIS) as a monitoring tool for drugs of abuse. Heroin was not detected at either location, pro

Using molecular similarity to highlight the challenges of routine immunoassay-based drug of abuse/toxicology screening in emergency medicine.

BACKGROUND: Laboratory tests for routine drug of abuse and toxicology (DOA/Tox) screening, often used in emergency medicine, generally utilize antibody-based tests (immunoassays) to detect classes of drugs such as amphetamines, barbiturates, benzodiazepines, opiates, and tricyclic antidepressants, or individual drugs such as cocaine, methadone, and phencyclidine. A key factor in assay sensitivity and specificity is the drugs or drug metabolites that were used as antigenic targets to generate the assay antibodies. All DOA/Tox screening immunoassays can be limited by false positives caused by cr

Anabolic-androgenic steroids: use and abuse in pediatric patients.

The "win at all costs" mentality fuels athletes to seek performance-enhancing substances, such as anabolic-androgenic steroids, to gain an advantage over their opponents. Nonathletes espouse this same attitude to "win" the battle of attractiveness. An enhanced understanding of anabolic-androgenic steroids and the motivations behind their abuse will arm pediatricians with the ability to engage their patients in a balanced discussion of the benefits and costly risks of anabolic-androgenic steroids and successfully deter further use.

Epidemiology of alcohol and other drug use among motor vehicle crash victims admitted to a trauma center.

The objectives of this research were to (1) determine the incidence and prevalence of alcohol and other drug use among motor vehicle crash (MVC) victims admitted to a regional Level-I trauma center, and (2) to examine the utility of using a rapid point-of-collection (POC) drug-testing device to identify MVC patients with drug involvement. Blood and urine specimens were routinely collected per clinical protocol for each MVC victim at the time of admission. Blood alcohol concentration (BAC) levels were determined per standard clinical protocol. Clinical urine specimens were routinely split so th

Deaths after intravenous misuse of transdermal fentanyl.

Fentanyl is a potent synthetic opioid used as a general anesthetic and analgetic. Fatal outcome from intravenous misuse of transdermal fentanyl is rare, and there are few such reports in literature. Here we report two cases of fatal intravenous injection of the content from fentanyl patches. Both were male drug addicts, found dead within a one week interval in the same apartment. Post-mortem femoral blood was screened for amphetamines, cannabinoids, cocaine, and opioids with immunological methods (EMIT II) and further with headspace gas chromatography for alcohol and with liquid chromatography

Sympathomimetic drug abuse masking an endogenous hyperadrenergic state, Graves' disease.

Sympathomimetic drug use or abuse in a thyrotoxic individual can be clinically devastating as a result of enhanced cellular sensitivity to adrenergic stimulation. Thus, it is critical to recognize thyrotoxicosis in a patient with coexistent sympathomimetic drug abuse. This case report describes a patient with Graves' disease and severe hyperadrenergic symptomatology that was thought to result from sympathomimetic drug abuse.

A green and non-invasive approach for quantitative analysis of psychoactive substances in exhaled breath.

Exhaled breath has emerged as a promising alternative biological matrix in forensic toxicology due to its non-invasive collection and its potential to reflect recent substance use. In this study, a custom-made breath collection device was developed using 3D printing technology, and its analytical performance for detecting psychoactive substances was evaluated. A quantitative analytical method for target analytes in exhaled breath was validated in accordance with ANSI/ASB guidelines and subsequently applied to samples collected from individuals undergoing substance use treatment. The method dem

Accuracy of Patient-Reported Exposure to New Psychoactive Substances and Other Illicit Drugs in Australian Emergency Departments: Findings From the Emerging Drugs Network of Australia.

INTRODUCTION: New psychoactive substances (NPS) present a unique challenge in clinical, public health and drug-policy contexts. Continued diversity, unknown potency and often unintentional exposure can limit the accuracy of self-reported data. This study examined the accuracy of patient-reported NPS and illicit drug exposure in Australian emergency departments (ED). METHODS: Patient-reported drug exposure, clinical and toxicology data were extracted from the Emerging Drugs Network of Australia Clinical Registry between 1 July 2021 and 30 June 2024 for patients presenting with severe and/or un

A descriptive analysis of substance use screening among youth involved in the legal system in eight counties.

BACKGROUND: Substance use disorders (SUDs) are more prevalent among youth involved in the legal system (YILS) compared to non-system-involved peers. Legal system involvement can serve as a gateway to health services, presenting an opportunity to identify YILS in need of SUD intervention. Here, aims included: (1) describe YILS patterns of substance use based on drug testing (e.g., urine) versus a self-report risk screener, (2) characterize how screening practices and results differ based on YILS demographic characteristics, and (3) compare how positive screening informs referrals to behavioral

[Standardized Operating Procedure «Model of Short-Term Motivational Intervention During Preventive Medical Examinations of Students for Early Detection of Narcotic Drugs and Psychotropic Substances Use»].

INTRODUCTION: The use of psychoactive substances (PAS) remains one of the key factors contributing to the development of non-communicable diseases and posing a threat to the national security of the Russian Federation. Preventive measures remain the most effective in minimizing negative impacts on public health, as supported by scientific research and enshrined in the *State Anti-Drug Policy Strategy of the Russian Federation until 2030* (Presidential Decree No. 733 of 23.11.2020). OBJECTIVE: To assess the effectiveness of implementing the standardized operational procedure (SOP) «Model of Sh

Polydrug fatal intoxication involving MDPHP: Detection and in silico investigation of multiple 3,4-methylenedioxy-derived designer drugs and their metabolites.

A drug-related fatality involving 3,4-methylenedioxy-α-pyrrolidinohexanophenone (MDPHP) is here reported. Belonging to the class of synthetic cathinones (SCs), MDPHP is a 3,4-methylenedioxy-derived designer (MDDs) drug with a pyrrolidine moiety and an alkyl portion with six carbon atoms. Other MDD pyrrolidine derivatives belong to the alkyl homologous series (C3-C5) and are known as 3,4-methylenedioxy-α-pyrrolidinopropiophenone (MDPPP), 3,4-methylenedioxy-α-pyrrolidinobutyrophenone (MDPBP) and 3,4-methylenedioxypyrovalerone (MDPV). MDDs are psychostimulant drugs of abuse that primarily act on

Trends in urine drug screening and positivity among patients with heart failure: Insights from a large health system.

BACKGROUND: Substance use is common in patients with HF and affects candidacy for advanced HF therapies. Rates and results of urine drug screening (UDS) are understudied particularly in the setting of increasing decriminalization of some substances. Substance use, particularly cannabis, has been increasing in older age groups who are most at risk of HF. However, while UDS assesses substance use, no guidelines recommend routine screening for patients with HF, outside of evaluation for advanced therapies. METHODS: We performed a retrospective study including data from the Epic electronic health

Changing trends in anabolic-androgenic steroid use within Scottish prisons: Detection, prevalence, and quantitation.

Anabolic-androgenic steroids (AASs) are a subclassification of image performance enhancing drugs (IPEDs). While AAS use is most prevalent among people in athletics, there is also high lifetime prevalence of AAS use among prisoners. This study reports the qualitative detection of AASs in seized samples from the Scottish prisons from 2019-2023. Additionally, methods were developed for the quantitative analysis of AASs using gas chromatography-mass spectrometry (GC-MS) and applied to 61 samples of tablets or powders seized from Scottish prisons between July 2022 and July 2023. Since 2022, there h
