[Infections in surgery of idiopathic scoliosis].

PURPOSE OF THE STUDY: In this retrospective study, we evaluated infectious complications in the patients undergoing surgical treatment for idiopathic scolions in order to identify risk factors for postoperative infections. MATERIAL: A total of 786 patients with idiopathic scoliosis were operated on during 24 years. In 754 (96%) cases, we used the posterior approach, involving posterior fusion and internal fixation, and subsequent immobilization in a brace. During that period, we recorded 15 (1.9%) deep wound infections in the area of fusion. Early infections were treated by debridement and la

The Eden-Hybbinette procedure is one hundred years old! A historical view of the concept and its evolutions.

INTRODUCTION: One hundred years ago, before Bankart, Latarjet or Bristow, Eden and Hybbinette developed a procedure to treat anterior shoulder instability and currently, this eponymous term is known by every shoulder surgeon. The purpose of this review is to summarise the historical "Eden-Hybbinette" procedure and its evolutions during the last century and discuss results. METHOD: On the centenary of the first publication on the "Eden-Hybbinette procedure", a search was conducted on Medline, Google Scholar and in the grey literature, to find its initial concept and description, and the evolut

Early results with use of the midfoot fusion bolt in Charcot arthropathy.

Charcot neuroarthropathy is a complicated condition affecting up to 1 in 680 diabetic patients that can rapidly cause severe destruction of the bony architecture in the foot, with resultant gross instability and frank deformity. Conservative care is not always successful at maintaining an intact soft tissue envelope of the foot; therefore, surgical reconstruction is often attempted in an effort to salvage the limb. The goal is to create a stable, plantigrade foot that can be placed in a shoe or simple brace. However, this effort is dramatically more challenging because of the pathologic bone b

Endoscopic neurosurgery "around the corner" with a rigid endoscope. Technical note.

Endoscopically "working around the corner" is presently restricted to the use of flexible endoscopes or an endoscope-assisted microneurosurgical (EAM) technique. In order to overcome the limitations of these solutions, endoscopic equipment and techniques were developed for "working around the corner" with rigid endoscopes. A steering insert with a 5 French working channel is capable of steering instruments around the corner by actively bending the guiding track and consecutively the instrument. A special fixation device enables strict axial rotation of the endoscope in the operating field. End

Stabilization of fractures in the hand and wrist with traumatic soft tissue and bone loss.

Open type III fractures of the hand or wrist with severe bone and soft tissue loss justify aggressive treatment to restore anatomy, assure healing, and maximize functional recovery. The techniques of modern wound excision used at initial surgery predictably result in a decompressed and surgically clean wound within a few days from injury in the vast majority of cases. This allows a safe application of delayed primary internal fixation and bone grafting for fracture restoration or joint arthrodesis as well as early wound closure or coverage. The immediate or early application of stable external

[The posterior lumbar interbody fusion with cages (PLIF) and transpedicular stabilization].

The development of intervertebral cages has significantly innovated the original technique of posterior lumbar interbody fusion (PLIF). In this study we present the results of patients treated for degenerative or postoperative segmental spinal instabilities by PLIF with cages and pedicular stabilisation (360 degrees-instrumentation). Between 1992 and 1999 we implanted either CFRP-, PEEK- or Titanium-cages in 86 patients. 78 patients were adequately followed up over a period of at least 12 months (average 2,6 years). 5 patients were stabilised over 2 segments, so that ultimately 83 fused segmen

Fracture-dislocation of the thoracic spine without neurologic lesion.

An 18-year-old man with a fracture-dislocation of the thoracic spine showed remarkable displacement, but without neurologic complications. The halo-femoral traction allowed a reduction, which has been only partially maintained. Despite this, the clinical result is satisfactory. The lesion appears to be more common than the few cases published in the literature would suggest.

Circular External Fixation as a New Offloading Standard of Treatment in Charcot Neuro-Osteoarthropathy Complicated by Midfoot Osteomyelitis: A Pilot, Prospective Case-Control Study.

After surgical treatment of Charcot neuro-osteoarthropathy (CNO) complicated by plantar ulcer and midtarsal osteomyelitis, offloading is mandatory to protect the surgical site. Total contact casting is, to date, the standard-of-care to offload the foot during the postoperative period. We have compared the application of external circular fixator, to the standard of care, with regard to surgical wound healing and time to healing. During the time period from January 2020 to December 2021, 71 consecutive patients admitted to our unit with diabetes and CNO complicated by plantar ulceration and mid

3D Printing Applications for Craniomaxillofacial Reconstruction: A Sweeping Review.

The field of craniomaxillofacial (CMF) surgery is rich in pathological diversity and broad in the ages that it treats. Moreover, the CMF skeleton is a complex confluence of sensory organs and hard and soft tissue with load-bearing demands that can change within millimeters. Computer-aided design (CAD) and additive manufacturing (AM) create extraordinary opportunities to repair the infinite array of craniomaxillofacial defects that exist because of the aforementioned circumstances. 3D printed scaffolds have the potential to serve as a comparable if not superior alternative to the "gold standard

Rehabilitation evaluation of the newly developed polymeric based passive polycentric knee joint.

Objective: The lower limb amputation is one of the major concerns for the amputee's daily life and the trans-femoral (TF) amputation is being paid a lot of attention because of its functional requirement in flexion-extension motion. Though significant progress has been made for the development of high end prosthetic knee joint, the affordability of the same is still a great concern. Thus, a passive polycentric knee joint was developed and the health related quality of life (HRQL) before and after the fixation of the prosthesis, and performance of the same were studied.Design: After 6 months of

Sagittal plane correction in pedicle subtraction osteotomy using the Xia 3 SUK Direct Vertebral Rotation System: technical note.

The Xia 3 SUK Direct Vertebral Rotation (DVR) System was developed for performing the vertebral derotation maneuver in scoliosis surgery. The author applied this device to sagittal plane correction in pedicle subtraction osteotomy for adult spinal deformity. The surgical procedure included 1) preparing secure proximal and distal foundations for correction using mutisegmental pedicle screw-rod fixation (to avoid stress concentration to a specific screw-bone interface), 2) decancellating only the posterior two-thirds of the vertebral column, 3) providing supplemental interbody fusion above and b

Failure of the Femoral Neck System in a Young Patient: A Case Report.

INTRODUCTION: Femoral neck fractures in young adults are rare but carry a high risk of complications, including non-union and implant failure. The femoral neck system™ (FNS) is a newer fixation device designed to enhance mechanical stability while minimizing surgical morbidity. Limited reports exist on its failure modes. The purpose of this case report is to describe a unique failure of the FNS and the challenges encountered during revision surgery. CASE REPORT: A 21-year-old female sustained a basicervical femoral neck fracture and underwent fixation with the FNS and an additional headless s

Effective treatment of non-union after internal fixation of femoral neck fracture using integrated traditional Chinese and western medicine.

BACKGROUND: Hip fractures, particularly femoral neck and intertrochanteric fractures, represent approximately 6.37 % of all fractures, with incidence rates showing an upward trend annually. CASE PRESENTATION: This report presents a case of a 55-year-old female with a femoral neck fracture who underwent internal fixation. One-year post-surgery, the patient continued to experience hip pain, and computed tomography (CT) scans revealed non-union of the fracture. The patient received an integrated treatment approach combining traditional Chinese and Western medicine, involving the alternating admi

[Surgical treatment of atlanto-axial dislocation. An introduction of two new methods].

In the past fourty-two months we have operatively treated twenty cases of several types of atlanto-axial dislocation (AAD) including an atlanto-axial deformity. A transoral retropharyngeal approach to the anterior arch of the atlas and the body of the axis was performed in two cases of AAD with dens fracture, 2 traumatic pure anterior AAD with relatively narrow spinal canal and 1 os odontoideum. The operative method we have devised is a modification of the transoral micro-surgical odontoid resection. In hyperextended position the fractured dens or os odontoideum comes to the position nearest t

[Study on Intraoperative Motion-Following Technology for a Novel Robot-Assisted Total Knee Arthroplasty System].

At present, joint surgical robots for total knee arthroplasty (TKA) still face certain technical limitations during the bone resection process. The conventional approach of using lower-limb fixators to immobilize the extremity suffers from two major drawbacks: increased risk of osteotomy error and prolonged operative time. To address these shortcomings, this study introduces an innovative real-time tracking motion-following technology for orthopedic surgical robots. This approach enables the robotic arm to dynamically monitor and precisely follow subtle movements of osseous structures intraope

Development of an endoluminal intestinal attachment for a clinically applicable distraction enterogenesis device.

PURPOSE: Previous methods of distraction enterogenesis have relied upon blind-ending intestinal segments or transmural device fixation, requiring multiple operations and potential bowel injury. We hypothesized that using a novel attachment would allow reversible device coupling to the luminal bowel surface, achieving effective endoluminal distraction. METHODS: A telescopic hydraulic device was designed with latex balloon attachments covered with high-friction mesh and a dilating fenestrated elastic mask (DFM attachment), allowing mesh-to-mucosa contact only with inflation. Mitral Valve Repair via a Minithoracotomy in a Patient With Pectus Excavatum.

Cardiac surgery in patients with pectus excavatum is challenging because of the difficulty associated with achieving optimal surgical exposure and postoperative sternal fixation by using standard instruments. To solve these problems, mitral valve repair was performed via a right minithoracotomy in a 48-year-old man with severe mitral valve regurgitation and pectus excavatum. With the use of conventional median sternotomy, an optimal surgical field was difficult to achieve because of his thoracic deformity. Therefore, surgical fixation via right minithoracotomy using particular equipment was pe
