Concurrent manometry and videofluoroscopy were utilized to examine tongue base function during swallowing in 3 patients with head and neck cancer. Subjects were instructed in four voluntary swallow maneuvers, including the supersupraglottic swallow, effortful swallow, Mendelsohn maneuver, and tongue-hold maneuver. Peak catheter pressures (mm Hg) at the tongue base-pharyngeal wall level were recorded and duration of tongue base to pharyngeal wall contact was measured for each swallow. This pilot study revealed that tongue base-pharyngeal wall pressures and contact duration increased with use of

BACKGROUND: Aspiration of food and liquid following supraglottic and supracricoid laryngectomy has been documented and found to be the most frequent major postoperative complication that extends hospitalization. The advantages as well as disadvantages of discharging a patient with percutaneous endoscopic gastrostomy (PEG) placement and home therapy versus an aggressive in-hospital dysphagia management program remain controversial. The present investigation examines an aggressive in-patient postoperative dysphagia management program following decannulation.
METHODS: Twenty-one patients partici

We report two cases of cerebral infarction in which swallowing function improved following swallowing rehabilitation. Patient 1 was an 82-year-old man, who was admitted due to rheumatoid arthritis and multiple cerebral infarction, suffering from aspiration pneumonia. The abnormality of swallowing was assessed by the water swallowing test and videofluorography. It has been reported that videofluorography is useful in the diagnosis of aspiration. Three weeks after the start of swallowing rehabilitation, the serum level of inflammatory markers and the chest X-ray had returned to normal. His score

We studied oropharyngeal function in patients with Chagas' disease and oesophageal disease. Twenty-four patients with Chagas' disease, 17 of them with oesophageal disease, were submitted to scintigraphic oropharyngeal transit. A single swallow of a 10-mL bolus of water labelled with 40 MBq 99m Tc-phytate, followed by image acquisition at 20 images s-1 yielded time-activity curves in which oropharyngeal dynamics was evaluated. Thirteen normal volunteers were used as controls. Oral transit was longer for chagasics with oesophageal disease than for controls (P=0.004) or for chagasics with a norma

Swallowing difficulties in isolated recurrent laryngeal nerve paralysis are rarely reported in the literature, although it is frequently mentioned in lesions of both recurrent laryngeal and superior laryngeal nerves. The aim of this prospective study was to evaluate the incidence of aspiration in patients with unilateral recurrent laryngeal nerve paralysis after head and neck or thoracic surgery. Eighteen patients were included and evaluated within the first week and eleven two months postoperatively. Position, tone and tension of the vocal fold as well as assessment of the glottic axis, aryte

This study investigates inter- and intrajudge reliability of a clinical examination of swallowing in adults. Several investigations have sou … l motor function were rated more reliably than were history measures or measures taken during trial swallows. There is a need to define more clearly the measures employed in clinical examinations and to be consistent in reporting reliability for clinical measures of swallowing function in future research.

Channel activators (potentiators) of cystic fibrosis (CF) transmembrane conductance regulator (CFTR), can be used for the treatment of the small subset of CF patients that carry plasma membrane-resident CFTR mutants. However, approximately 90% of CF patients carry the misfolded ΔF508-CFTR and are poorly responsive to potentiators, because ΔF508-CFTR is intrinsically unstable at the plasma membrane (PM) even if rescued by pharmacological correctors. We have demonstrated that human and mouse CF airways are autophagy deficient due to functional sequestration of BECN1 and that the tissue transglut

The objective of this study was to determine the inter- and intrarater reliability in evaluating videofluoroscopic swallowing studies (VFSS). Participants included 4 physicians (3 physiatrists and 1 internist) and 5 speech-language pathologists with at least 5 years experience in evaluating VFSS. The main outcomes of the study were reliability ratios of positive and negative tests in inter- and intrarater evaluations. Raters independently rated each of 20 VFSS on two separate occasions. Traits evaluated included oral stage impairment, aspiration, pharyngeal retention, and several functional co

The radionuclide salivagram was introduced as a simple physiologic technique to document the aspiration of saliva in patients with chronic lung disease. We previously reported positive studies to be prevalent in patients with neuromuscular incoordination, as with cerebral palsy, or after surgery involving the upper airway. Many patients referred for a salivagram, however, have an apparent intact swallowing mechanism and are better challenged with a tagged bolus.

This study examines the pharyngeal phase of swallowing after the resection of pharyngeal cancer, and focuses on the pharynx as a functional organ. The purpose of the study was to obtain information on both reconstruction and rehabilitation in cases of surgically treated pharyngeal cancer. The records of 21 consecutive patients who underwent surgical treatment of oropharyngeal and hypopharyngeal squamous cell carcinoma between 1990 and 1993 were reviewed. Functional results following surgery were graded on a numerical scale in three categories: pharyngeal swallow, laryngeal and lower airway pro

Fetal swallowing is one of the main mechanisms of amniotic fluid circulation. Swallowing mechanisms were studied in 34 healthy fetuses using modern ultrasound technology. The fetal lips and mouth, as well as tongue, pharynx, larynx, trachea and esophagus were surveyed in serial sagittal planes. Fetal swallowing movements were recorded and subsequently analyzed from the videotapes. All neonates were born at term in a satisfactory condition. Twenty-nine infants were breastfed, and five were bottle fed. No feeding problems were reported in these neonates. In 85% of fetuses, swallowing activity st

Because no detailed information exists regarding the topographic representation of swallowing musculature on the human cerebral cortex in health or disease, we used transcranial magnetic stimulation to study the cortical topography of human oral, pharyngeal and esophageal musculature in 20 healthy individuals and the topography of pharyngeal musculature in two stroke patients, one with and one without dysphagia. Our results demonstrate that swallowing musculature is discretely and somatotopically represented on the motor and premotor cortex of both hemispheres but displays interhemispheric asy

The aim of this study was to assess the incidence and severity of dysphagia following anterior cervical spine surgery for cervical spondylosis. One-hundred patients were contacted 12-22 months following cervical spine surgery. Those reporting persistent swallowing impairment were invited to attend for further investigation. Of 73 respondents, 33 (45%) experienced postoperative dysphagia. This persisted for longer than 6 months in nine (12% of respondents). Of five subjects attending for investigation, none had a definite radiological abnormality. In contrast, manometry suggested hyperactivity

In this study, dynamic imaging was used to track the movements of oral cavity and laryngeal structures during swallowing in 10 normal adults subjects. The movements of tiny lead pellet markers attached to the lips, tongue, mandible, and soft palate, as well as anatomic landmarks on the hyoid bone, were measured in relation to a reference pellet affixed to the upper central incisors. Sagittal views of the oral cavity were obtained using standard videofluorography. Each subject produced 10 swallows of 12 ml of tap water followed by 5 swallows with a bite block placed between the molars. The reco…

Twenty-two nondysphagic normal subjects were sonographically studied to define the average volume of a physiologic fluid bolus. Several varying volumes (5, 10, 15, and 20 ml) of water were given to assess the average size of the swallowed bolus. At the onset of oral deglutition, when placed between the dorsum of the tongue and the groove of the hard palate, the bolus has an ellipsoidal shape, thus permitting sagittal, axial, and coronal measurements. Our results indicate that swallowed bolus volume rises proportionally to water bolus given up to 15 ml and that the average size of a physiologic

Swallowing is an essential function of the upper alimentary tract. It is highly complex, requiring precise coordination of numerous nerves and muscles of the oral cavity, pharynx, larynx, and esophagus. Swallowing is integrated with other physiologic functions, including mastication and respiration. Impairments of swallowing may result from many different structural or physiologic disorders. Little is currently known about the direct effects of pollution on swallowing. Structures critical to swallowing, however, are vulnerable to damage by environmental hazards such as exposure to ionizing rad

In 9 young men, healthy volunteers, we studied the effect of dry swallows, liquid and paste swallows on the pharyngeal motility. The viscosities of liquids and past were 200 centipoise (cP), 300 cP and 60000 cP, respectively. The bolus volume was 10 ml. For pharyngeal manometry we used two strain-gauge manometric probes taped together, so that a total of six transducers were staggered at 1.5 cm intervals. After its passage through the nose, the assembly was positioned so that all its transducers faced posteriorly, and manometric activity was recorded from oropharynx, hypopharynx and upper esop…
