INTRODUCTION: Paroxysmal Sympathetic Hyperactivity (PSH) is a historically underrecognized yet increasingly acknowledged syndrome following traumatic brain injury (TBI), characterized by episodic surges in sympathetic nervous system activity. Despite increasing awareness, effective therapy remains unavailable due to diagnostic uncertainty and therapeutic heterogeneity.
AREAS COVERED: In this review, the authors synthesize the recent advances and emerging fronts in the treatment of PSH, encompassing mechanistic understanding, drug discovery, non-pharmacological treatment, and trials in progres…

Paroxysmal Sympathetic Hyperactivity (PSH) is a challenging and often underrecognized syndrome, commonly arising after a traumatic brain injury (TBI). Characterized by episodic bursts of heightened sympathetic activity, PSH presents with a distinct constellation of symptoms including hypertension, tachycardia, hyperthermia, and diaphoresis. While the exact pathophysiology remains elusive, current evidence suggests that the syndrome results from an imbalance between excitatory and inhibitory neuronal pathways within the central nervous system, leading to dysregulated autonomic responses. The un

Sympathetic skin responses (SSRs), a measure of sympathetic cholinergic sudomotor function, have been used in the assessment of autonomic dysfunction in patients with spinal cord injury (SCI). This review highlights the basic mechanisms underlying SSRs as well as their application to the SCI population. We address the utility of SSRs in assessing autonomic function, the relationship between autonomic and sensorimotor impairment, and the association between SSRs and the sequelae of autonomic dysfunction in SCI, particularly autonomic dysreflexia and orthostatic hypotension. Overall, SSRs are a

The sympathetic skin response (SSR) is a slow wave resulting from activation of the sudomotor sympathetic efferent fibers. Records are usually made with surface electrodes on hand or foot after the electrical stimulation. Either the amplitude or the latency of the response varies greatly on consecutive stimulations and there is also a remarkable tendency to habituation. Therefore SSR is considered abnormal if no significant responses are detected. SSR is well correlated with other autonomic function tests and its abnormality is documented in a variety of neurologic disorders such as diabetic n

The otolaryngologist-head and neck surgeon is aware that the cervical sympathetic nerves lie behind the carotid artery and should be avoided during neck surgery. To render this sketchy dictum more tangible, relevant anatomy and physiology of the autonomic supply to the head and neck is reviewed, as are aspects of site-of-lesion testing, with respect to Horner's syndrome. Examples of neck operations during which the cervical sympathetic chain--from the base of skull to the root of the neck--may be injured are illustrated.

What is the topic of this review? Chronic intermittent hypoxia (CIH), as observed in patients with obstructive sleep apnoea, is associated with the development of sympathetically mediated arterial hypertension. Nevertheless, the mechanisms underpinning the augmented sympathetic outflow in CIH still remain under investigation. What advances does it highlight? In this report, I present experimental evidence supporting the hypothesis that changes in the function of the respiratory network and coupling with the sympathetic nervous system may be considered as a novel and relevant mechanism for the …

The sympathetic nervous system is crucial for the regulation of visceral organ function. For instance, the activation of the sympathetic nervous system promotes glycogenolysis in the liver and modulates glucagon and insulin release from the pancreas, thereby raising blood glucose levels. A decrease in sympathetic nerve activity has the opposite effect. Although such acute effects of sympathetic activity changes have been studied, their long-term outcomes have not been previously examined. In this study, we removed the celiac/superior mesenteric ganglia, where sympathetic postganglionic neurons…

Many glial biologists consider glia the neglected cells of the nervous system. Among all the glia of the central and peripheral nervous system, satellite glia may be the most often overlooked. Satellite glial cells (SGCs) are located in ganglia of the cranial nerves and the peripheral nervous system. These small cells surround the cell bodies of neurons in the trigeminal ganglia (TG), spiral ganglia, nodose and petrosal ganglia, sympathetic ganglia, and dorsal root ganglia (DRG). Essential SGC features include their intimate connections with the associated neurons, their small size, and their

After recovering from COVID-19, a significant proportion of symptomatic and asymptomatic individuals develop Long COVID. Fatigue, orthostatic intolerance, brain fog, anosmia, and ageusia/dysgeusia in Long COVID resemble "sickness behavior," the autonomic nervous system response to pro-inflammatory cytokines (Dantzer et al., 2008). Aberrant network adaptation to sympathetic/parasympathetic imbalance is expected to produce long-standing dysautonomia. Cervical sympathetic chain activity can be blocked with local anesthetic, allowing the regional autonomic nervous system to "reboot." In this case

For 80 years the sympathetic nervous system has been implicated in certain human pains, notably causalgia and reflex sympathetic dystrophy. This assumption has led to the accepted concept of 'sympathetic dependent' pain. In this critical review, the evidence for this assumption is assessed. It is found that the clinical phenomena suggesting sympathetic nerve involvement may be more satisfactorily attributed to effects of neuropeptides released from afferent C-fibres. The evidence also demonstrates that the effects resembling sympathetic dysfunction relate neither to the pain nor to pain relief…

Amphetamine and methoxamine facilitated the flexor reflex in chronic spinal rats whose descending monoaminergic nerve endings were degenerated. Methoxamine, but not amphetamine, facilitated the flexor reflex in the chronic spinal rat 16-20 h after IV 6-hydroxydopamine treatment. These data indicate that amphetamine's spinal cord facilitative effects are mediated through two noradrenergic mechanisms. In the acute spinal rat, its facilitative effects in these experiments seem to be independent of the sympathetic nervous system and are probably dependent on the presence of the terminal axons of t…

Inferences about the association between sympathetic overactivity and insulin resistance have been drawn from the infusion of sympathomimetic amines in supraphysiological doses. We used the isolated perfused human forearm to investigate the effect of reflex-induced sympathetic nervous system activation on the peripheral utilization of glucose in the skeletal muscles of 14 healthy men. Local hyperinsulinemia in the forearm (132 +/- 25 microunits/mL for 90 minutes) induced a significant increase in the utilization of glucose from baseline (16.4 +/- 3.1 mg.dL-1.min-1 per 100 mL forearm volume) to…

BACKGROUND: Sympathetic innervation is fundamentally required for physiological aortic homeostasis. Though it plays a vital important role in aortic function, its specific role in aortic dissection (AD) remains unclear.
METHODS AND RESULTS: In this study, we explored the role of the sympathetic nervous system in AD by examining the effects of superior cervical ganglionectomy (SCGx) in mice. Using β-aminopropionitrile (BAPN) induced AD mice model at pre-lesional stage, the tyrosine hydroxylase (TH) expression, indicative of sympathetic activity, increased early in AD progression and correlated…

Renal denervation (RDN) is an interventional approach that targets the sympathetic nervous system with the goal of modulating its activity, an important contributor to the pathogenesis of many cardiovascular and metabolic diseases. Unlike pharmacologic therapies, RDN efficacy is independent of patient adherence to medications, which remains a major limiting factor in long-term blood pressure control. Initially developed to treat severe, treatment-resistant hypertension, RDN has demonstrated consistent results in recent sham-controlled trials across the spectrum of hypertension. As a result, RD…

Given the ubiquitous distribution of catecholamines in mammals, and their importance in a range of physiological processes pivotal to blood pressure regulation, the subject of catecholamines and essential hypertension has a broader context than simply consideration of sympathetic nervous system and adrenal medullary dysfunction. These further matters are the likely involvement in hypertension pathogenesis of the CNS catecholaminergic neurones influencing peripheral sympathetic outflow, the possible pathogenetic significance of adrenaline released as a cotransmitter in sympathetic nerves, and t…

Heart rate (HR) in vertebrates is determined by the discharge frequency of cardiac pacemaker cells, which are innervated by the autonomic nervous system. The portion of this system located within the heart, the intracardiac nervous system (ICNS), transmits impulses to cardiac effectors for control of cardiac output. Central autonomic neurons modulate the heart through sympathetic (acceleratory) and parasympathetic (inhibitory) axons in the vagosympathetic trunks. In the classical model of cardiac control, the balance between sympathetic and parasympathetic efferent signals determines HR. Recen
