Joint involvement in patients with early polymyalgia rheumatica using high-resolution ultrasound and its contribution to the EULAR/ACR 2012 classification criteria for polymyalgia rheumatica.

OBJECTIVE: To assess joint involvement and the contribution of musculoskeletal ultrasound (MSUS) to the novel European League Against Rheumatism/American College of Rheumatology (EULAR/ACR) 2012 classification criteria in patients with polymyalgia rheumatic (PMR). METHODS: MSUS was performed in 54 consecutive patients with recent-onset PMR. RESULTS: Biceps tenosynovitis of at least 1 shoulder has been observed in 70.4% of patients, and 64.8% had a bilateral biceps tenosynovitis. Subdeltoid bursitis (27.8% unilateral, 5.6% bilateral), glenohumeral synovitis (22.2% unilateral, 9.3% bilateral),…

Which inflamed tissues explain a positive metatarsophalangeal squeeze test? A large imaging study to clarify a common diagnostic procedure.

OBJECTIVES: The squeeze test of MTP joints is frequently used because it is easy and cheap. It is traditionally perceived as a test for synovitis. Besides classic intra-articular synovitis, also tenosynovitis and intermetatarsal bursitis (IMB) represent synovial inflammation, albeit juxta-articularly located. Both are frequently present in RA and occasionally in other arthritides. Therefore we hypothesized that tenosynovitis and IMB contribute to a positive MTP squeeze test. METHODS: A cross-sectional study design was used. A total of 192 early arthritis patients and 693 clinically suspect ar…

MRI and ultrasound predictors of progression to inflammatory arthritis in anti-CCP-positive individuals with musculoskeletal symptoms without clinical synovitis: a prospective cohort study.

OBJECTIVES: This study aims to evaluate the prevalence of magnetic resonance imaging (MRI) and ultrasound features in anti-cyclic citrullinated peptide (anti-CCP) positive individuals with musculoskeletal symptoms without clinical synovitis (CCP+ at-risk), and compare their prognostic value for progression to inflammatory arthritis (IA). METHODS: Overall, 130 CCP+ at-risk underwent baseline contrast-enhanced 3 T MRI of the dominant/most symptomatic hand and both feet. Images were scored for synovitis, tenosynovitis, osteitis, and erosions using Rheumatoid Arthritis Magnetic Resonance Imaging …

Sequence of joint tissue inflammation during rheumatoid arthritis development.

OBJECTIVE: Subclinical joint inflammation in patients with arthralgia is predictive for progression to rheumatoid arthritis (RA). However, the time course of progression for bone marrow edema (osteitis), synovitis, and/or tenosynovitis is unsettled. This longitudinal study assessed the course of magnetic resonance imaging (MRI)-detected subclinical joint inflammation during progression to RA. METHODS: Patients that progressed from clinically suspect arthralgia (CSA) to RA underwent 1.5-T MRI of the metacarpophalangeal (MCP), wrist, and metatarsophalangeal (MTP) joints at presentation with art…

Ankle extensor tendon synovitis due to a date palm thorn.

Palm thorn injury to tendons and joints is characterized in most patients by an acute inflammatory stage followed by a relatively asymptomatic period and finally by chronic synovitis or tenosynovitis. Once the inflammatory process has begun, detection and localization of the thorn can be very difficult. We present an unusual case of palm thorn tenosynovitis of the ankle extensor tendon which was detected and localized preoperatively by ultrasound. Removal of the thorn and synovectomy were curative.

Tendinitis Around the Wrist and Hand.

Tendinopathies around the hand and wrist are common. Most are diagnosed easily with a thorough history and clinical examination. Common conditions involving the hand and wrist include trigger finger, de Quervain tenosynovitis, intersection syndrome, third and fourth extensor compartment tenosynovitis, extensor carpi ulnaris tendinitis, and flexor carpi radialis tendinitis. Management strategies include nonsurgical treatments comprising immobilization, corticosteroid injections, or hand therapy, and surgical techniques including tendon release.

Comparison between 1.5T and 3.0T MRI: both field strengths sensitively detect subclinical inflammation of hand and forefoot in patients with arthralgia.

OBJECTIVE: Magnetic resonance imaging (MRI) of small joints sensitively detects inflammation. This inflammation, and tenosynovitis in particular, has been shown to predict rheumatoid arthritis (RA) development in arthralgia patients. These data have predominantly been acquired on 1.0-1.5 T MRI. However, 3.0 T is now commonly used in practice. Evidence on the comparability of these field strengths is scarce and has never included subtle inflammation in arthralgia patients or tenosynovitis. Therefore, we assessed the comparability of 1.5 T and 3.0 T in detecting subclinical inflammation in arthr…

Acute Peroneal Injury.

A high clinical suspicion and greater understanding of the anatomy and pathophysiology of lateral ankle injuries have enabled early diagnosis and treatment-improving outcomes of acute peroneal tendon tears. Multiple conditions can be the cause of lateral ankle pain attributed to the peroneal tendons: tenosynovitis, tendinosis, subluxation and dislocation, stenosing tenosynovitis, abnormality related to the os peroneum, as well as tears of the peroneal tendons. It is imperative for the clinician to maintain a high suspicion for peroneal tendon abnormality when evaluating patients with lateral a…

[Technique and diagnostic value of musculoskeletal ultrasonography in rheumatology. Part 6: ultrasonography of the wrist/hand].

Sonography of the hands is especially helpful in the diagnosis of early arthritis. Sonography allows for a very sensitive detection of small joint-effusion, tenosynovitis and small erosive bone lesions earlier than conventional radiography. Musculoskeletal sonography is also helpful in morphological analysis of changes of the median nerve in patients with carpal tunnel syndrome. The following standard scans are suggested for the sonographic evaluation of the wrist: 1. dorsal longitudinal scan along the radio-carpal joint, 2) along the ulno-carpal joint, and 3) dorsal transverse scan along the …
